Response variations of neurons in the lateral geniculate body of the freely moving rat.

The influence of the motivational state on the discharge rate and flash evoked activity of 57 single units of the dorsal lateral geniculate body (LGB) was investigated using extracellular microelectrodes. Flashes were presented either alone (control group) or followed by water reinforcement (trained group). With decreasing thirst motivation the background activity of most of the cells of both groups changed. This change consisted mainly in a decrease of the discharge rate. After satiation two thirds of the cells exhibited response variations to light stimuli. In trained thirsty animals in 10 out of 19 cells these changes were of very high degree, as the usual response pattern consisting in a sequence of excitatory and inhibitory phases was lost. Under such conditions the light acted as a conditioned stimulus in the thirsty animal. Nonvisual stimuli preceding the flashes and evoking inhibiton of the reflex behavior in trained rats produced small response modulations in about two thirds of the cells of either animal group. The assumption is made that the motivational state of the animal or environmental stimuli not belonging to the original conditioned stimulus - unconditioned stimulus (CS-US) paradigm are less effective in producing response changes of cells to physically equal stimuli than the biological relevance of the stimulus.

Granulopoietic activity in Felty's syndrome.

The levels of urinary and serum granulopoietic factor have been determined in nine patients with Felty's syndrome and compared with similar levels in patients with rheumatoid arthritis without Felty's syndrome and patients with neutropenic disorders of other causes. These studies have shown that urinary and serum levels of colony stimulating activity (CSA) in patients with Felty's syndrome are low (mean urine CSA 10.0 and serum CSA 4.0) when compared to patients with neutropenic disorders without rheumatoid arthritis (mean urinary CSA 26.8 and serum CSA 9.4). These findings suggest that Felty's syndrome may be, in part, the result of decreased production or activity of granulopoietic factors rather than due simply to increased granulocyte destruction.

Minimally invasive catheter implantation for regional chemotherapy of the liver: a new percutaneous transsubclavian approach.

PURPOSE: Development of a percutaneously implantable catheter system for regional chemotherapy of liver metastases and its application in patients with surgically implanted but dislocated catheters. METHODS: Thirty-three patients with liver metastases of colorectal tumors were submitted to percutaneous puncture of the subclavian artery and insertion of a catheter whose tip was placed in the proper hepatic artery and whose end was subcutaneously connected with an infusion pump. RESULTS: The mean duration of therapy via the percutaneously inserted catheter was 27 weeks (+/-14 weeks). The most frequent complication was disconnection of the therapy catheter from the tube of the infusion pump. Eighty percent of all complications were corrected by reintervention. The therapy drop-out rate due to catheter-associated complications was 9%. CONCLUSION: Percutaneous insertion of a catheter for regional chemotherapy of the liver is a relatively uncomplicated method with high patient acceptance and simple access for reintervention.

[Facilities offered by community-oriented professional rehabilitation centres for mentally challenged persons in the Federal Republic of Germany].

OBJECTIVE: Vocational rehabilitation is an important component of community-oriented mental health care. How is the situation of supply with vocational rehabilitation facilities in Germany offering their services to mentally challenged persons? METHOD: Search of a Germany-wide rehabilitation database, supplemented by information from the Federal Working Groups on Vocational Rehabilitation. Cartographical classification of the identified facilities. Analysis of the results in a comparison of the situation in the 16 German States (Länder). RESULTS: The Länder of North Rhine Westphalia, Bavaria, Baden-Wuerttemberg and Lower Saxony offer the greatest number of vocational rehabilitation facilities to mentally challenged persons. In all East German and in some of the West German Länder vocational rehabilitation facilities are restricted to some areas and to certain vocational preconditions. Many of the facilities in these areas are structured as centres and situated in rural areas. Hence, they lack community-orientation. Only North-Rhine Westphalia offers country-wide community-oriented vocational rehabilitation facilities to mentally challenged persons, which are not restricted to certain vocational preconditions. DISCUSSION AND CONCLUSIONS: The supply with vocational rehabilitation facilities for mentally challenged persons has improved during recent years. Nevertheless, the situation in many of the German Länder needs to be improved, particularly in respect of the availability of rehabilitation facilities for younger mentally challenged persons without prior working experience. The implementation of decentralised community-oriented facilities should be favoured in centres, especially in larger sparsely populated areas.

[Objective determination of the shape of the anterior chest wall using moiré topography. Method and development of dimension-free indices for the evaluation of funnel chest].

Stereophotogrammetric measurement of body surface results in data, which are dependent on shape, posture and positioning of the patient. In order to establish a valid description and characterization of funnel chest measured by stereophotogrammetric methods, index numbers must be defined, which are independent of posture und positioning of the patient. This procedure is exemplified in the analysis of moiré topograms from 29 patients with funnel chest and 21 normal persons. The sagittal cross section is characterized by the index TI I, which in essential is the sum of absolute values of angles in a polygon along the section (cf. fig. 7). The funnel in the coronal cross section is characterized by the index TI II, which gives the quotient (in %) of the mean funnel width and funnel depth (cf. fig. 8). Application of the index numbers shows a good differentiation of TI I, independent of sex, whereas TI II might be most useful in follow up measurements.
