Preliminary study of argon fluoride (193 nm) excimer laser trabeculectomy. Scanning electron microscopy at five months.

There is an increasing interest in the use of the excimer laser in ophthalmology, particularly in refractive surgery. The argon fluoride (193 nm) excimer laser ablates tissue with a high degree of precision and without any mechanical or thermal damage to surrounding structures. In this study, the argon fluoride excimer laser was experimentally used in the rabbit model to perform a trabeculectomy. Clinical and scanning electron microscopic evaluations showed a successful and patent filtering procedure. At the five-month follow-up the trabeculectomy was smooth and regular and there was no evidence of inflammation or thermal damage on the surrounding structures.

YAG laser treatment in a series of failed trabeculectomies.

YAG laser treatment was used in 22 consecutive cases of failed or failing trabeculectomy. Seven treatments were considered to be successful with a mean decrease in intraocular pressure of 9.8 mm Hg (6 to 13 mm Hg) over a mean follow-up period of 10.7 months (two to 18 months). In the successful cases the trabeculectomy was the first intraocular surgery that the eye had undergone and the average time from trabeculectomy to YAG laser treatment was relatively short, 6.7 weeks, compared to 1.1 years for the failures. The major complication was an increase in intraocular pressure of 4 mm Hg or more in nine (60%) of the failed cases two hours after laser treatment.

Glare disability secondary to YAG laser intraocular lens damage.

Neodymium:YAG laser intraocular lens damage results in opacities in the pseudophakos. When located in the visual axis, these opacities may cause glare and image degradation. In this paper, we report a patient who experienced disabling glare sufficient to require the explanation of the YAG-damaged intraocular lens. The patient, her management and clinical course, and the pathology of the explanted lens are discussed.

Adjuvant chemotherapy in ocular malignant melanoma. Study of 20 cases.

The frequency of hematogenously propagated hepatic metastases occurring from ocular melanoma led us to treat 20 patients with adjuvant chemotherapy, 19 patients starting chemotherapy during the month following enucleation and 1 patient, a year after enucleation. With a median follow-up of 6 years, 17 patients (80%) are disease-free. Three patients developed hepatic metastases at 24, 24 and 30 months, respectively. The results suggest that adjuvant chemotherapy is effective in preventing metastases from ocular melanoma.

[Hypoplasia of the optic nerve head].

Optic nerve hypoplasia is not a rare eventuality in children. This report discusses the clinical features of 10 new cases on a multiple point of view including optic canal tomography, fluorescein angiography and cat's examination. Optic nerve hypoplasia is not always accompanied by decreased visual acuity: sector fields are often identified in these cases.

Tolosa-Hunt syndrome.

Ten cases of painful ophthalmoplegia are reported. In all cases pain marked the onset of the disease; the condition was mostly unilateral. Recurrence and dramatic response to steroid therapy were more constant features than angiographic findings which may be completely normal. CAT permitted us to eliminate a tumor of the cavernous sinus in 7 cases, but in one case, which was operated, an aggravation followed surgery. Despite complete investigations of all patients, some questions remain unanswered. Is it an inflammatory or an allergic inflammatory process? (And the presence of antinuclear factor in an only case does not permit it to respond.) Why is the process confined to the superior orbital fissure? Mathew and Chandy tried to find a similarity between this syndrome and Bell's palsy although the 2 syndromes have not been shown to coexist. Is the Tolosa-Hunt syndrome comparable to the pseudotumor of the orbit? Although the dura mater is inserted on the border of the sphenoidal cleft, some patients may altern the 2 syndromes.

[Two cases of chondrodysplasia punctuate are reported].

The first one was a 13 months old lewish boy with typical features of the rhizometic type; clinicaly the patient demonstrated cataract, esotropia, nystagmus, malnutrition, micromelia, contracture, multiple stippled calcifications about the periphery of the cartilagenous structures of the vertebral bodies, sternum, wrists, knees, heels, ribs; and profound mental retardation, After cataract operation, the head of the optic nerve demonstrate to be narrower than usualy and white. Pathologic studies of the lens' fragments after surgery showed the cataract to be without any specific characters. The child now 2 yrs and 1/2 old is still alive but in poor condition. The second was a 16 months old Arabic boy without typical features but with a cataract and stippled calfications of a limited number of epiphysae. Pathologic studies of the lens after cataract surgery showed the cataracts to be without any specific characters, the optic nerves were normal. The child now 3 yrs old is normally developed and in good conditions. It is remarquable to note that in both cases there was an history of consanguinity (mother and father were first cousins).
