Upper buccal sulcus approach to reduction of zygomatic fractures.

Thirty selected patients with simple zygoma fractures over a 15-year period have been treated by elevation and reduction with the use of a zymgomatic elevator from an intraoral upper buccal sulcus approach. No patient suffered a recurrence of the fracture of deformity or required an open reduction. This little publicized procedure is easy, fast, and effective in carefully chosen cases of zygoma and zygomatic arch fractures in the first 72-96 hours postinjury. It should be considered by the surgeon along with the more complex procedures.

Deltopectoral flap with a segmental dermal pedicle in head and neck reconstruction.

Eight cases of extensive oromandibular resections have been reconstructed by the use of deltopectoral shoulder flaps with a segment denuded for deep placement. The operative technique is described, and the various uses of this flap are outlined. This technique has allowed us to extend the versatility of the deltopectoral flap, and to avoid the occurrence of the often annoying and occasionally dangerous orocutaneous fistula.

Surgical management of impotence: progress report.

Our experience utilizing a new design Lash-Maser silicone implant in the surgical management of impotence has been detailed. The present series includes twenty-five patients, twenty-three of whom achieved satisfactory intercourse without problems or complications. The implant's simplicity of design, function, and surgical implantation is appealing.

Argon laser management of cutaneous vascular deformities. A preliminary report.

Twenty-two patients with cutaneous vascular malformations were treated with an argon laser. Promising results were obtained in patients with port-wine stains, hemangiomas, telangiectasia and varicose veins. The argon laser shows promise as an effective clinical tool in the treatment of all such cutaneous vascular abnormalities. However, further study of clinical as well as laboratory data is necessary over a long period of time before this modality can be definitely recommended as the treatment of choice for vascular deformities of the skin.

Breast reconstruction for malignant or premalignant disease.

In ten patients breast reconstruction was done after surgical treatment for a premalignant or malignant breast disease. In six of these, prophylactic subcutaneous mastectomy and implant reconstruction were carried out, and in the remaining four reconstruction was done after simple or modified radical mastectomy. It is suggested that these procedures should be considered by those physicians and surgeons who undertake evaluation and treatment of breast disease in women. Breast reconstruction should be considered and offered to patients who suffer from the severe personal and emotional trauma attendant to surgical operation for breast disease.

High-pressure silicone injection injury of the hand.

A case of room-temperature vulcanizing (RTV) silicone injection of the hand is presented, and principles of management of high-pressure hand injuries are outlined. Early debridement and decompression are urgent in the acute stage of injury. Intermediate stages require adequate coverage and mobilization, and late stages involve reconstructive procedures. As in all high-pressure hand injuries, safety measures and prevention at the industrial level are very important.
