Major clinical diagnoses found among patients with Raynaud phenomenon from the general population.

This study of 62 individuals with Raynaud phenomenon (RP) drawn from the general population of South Carolina shows a profile of associated diseases that is different from the one reported from hospital based studies. We found that connective tissue diseases affect a much smaller fraction of patients with RP than previously reported. Compared to RP negative controls, our RP positive group was found to suffer more frequently from a variety of diseases, both RP related and not RP related.

Fibroblast heterogeneity in scleroderma: Clq studies.

Fibroblast heterogeneity has been the basis of a pathogenetic theory of scleroderma in which one subpopulation of high collagen producing cells is expanded by selective growth. In studies by others using gingival cells, fibroblast membrane Clq receptor affinity correlates positively with collagen production. We have demonstrated distinctly higher amounts of Clq binding of fibroblasts from 2 patients with early scleroderma compared to late scleroderma and controls. These observations are consistent with the overgrowth of a normally occurring subpopulation of cells and provide support for clonal selection in the pathogenesis of scleroderma.

Serum alkaline phosphatase in diabetes mellitus.

Elevation of serum alkaline phosphatase concentration in patients with diabetes mellitus has been observed for several years, but the source and reasons are unknown. We report our experience with 39 diabetics, 38% of whom had an unexplained elevation of serum alkaline phosphatase. Isoenzyme determinations revealed bone fraction as the predominant species. Mean fasting serum glucose was significantly higher in the group with elevated alkaline phosphatase, supporting an association between the severity of diabetes and diabetic bone disease.

Theophylline-induced hypercalcemia.

Sixty patients with theophylline toxicity were hospitalized during a 2-year period. Eleven patients had hypercalcemia; their calcium levels returned to normal as theophylline levels fell to therapeutic or subtherapeutic levels. Serum calcium levels also fell significantly in three additional patients with theophylline toxicity, although the initial serum calcium concentration was not outside normal limits. A significant increase in serum calcium levels associated with therapeutic levels of theophylline in normal volunteers was reversed by propranolol. It appears that theophylline causes elevation of serum calcium by a system subject to beta-adrenergic regulation.

Determination of combined residues of metalaxyl and 2,6-dimethylaniline metabolites in urine by gas chromatography/mass spectrometry.

A gas chromatographic/mass spectrometric (GC/MS) method for determination of combined residues of the fungicide metalaxyl and its metabolites in urine containing the 2,6-dimethylaniline moiety is described. The method is a modification of a method of Balasubramanian and Perez for analysis of metalaxyl in vegetables. Noted modifications include replacement of steam extraction with extraction by methylene chloride and use of electron impact ionization GC/MS in the selected-ion mode. The method is linear over the range of 0.1-5 micrograms 2,6-dimethylaniline/g urine and has a detection limit of 0.025 microgram/g.
