[Prevention of mental disorders after the loss of a sibling in childhood].

Reaction to the loss of somebody close is determined, not only by cultural, historical, social and family factors, but above all by the stage of individual development that has been reached. Children behave differently from adults following "object loss". Their ego and object relations are developing rapidly. The classical phenomena of mourning are usually manifested only after adolescence. When their psychological balance has been upset by bereavement children try to restore it through reparative and adaptive measures. If they are not given adequate support from the parents and paediatrician, or psychotherapeutic help when this is called for, then the loss of a significant person frequently disturbs children's further development. There is an important field for preventive psychohygienic measures open in relation to fatal illness in childhood.

Gender-specific differences in the process of coping in families with a parent affected by a chronic somatic disease (e.g. multiple sclerosis).

Based on the investigation of 52 families and their 87 offspring by means of semistructured interviews, we evaluated the coping abilities of the parent affected by multiple sclerosis (MS), the healthy parent and their children. The results show that the gender of the child significantly influences his or her coping behaviour. Daughters cope better than sons, independently of the gender of the MS-affected parent. Only the daughter's coping is positively affected by age and disease variables. The correlation between the coping behaviour of parents and children is significant between children and their healthy parents and even stronger between children and healthy parents of the same gender. Healthy mothers and daughters cope better with the increasing disability of the father. This is not the case for healthy fathers and sons. Gender seems to be an important moderating factor in chronic parental disease and it has complex effects on the coping capacity of children.

[Couple relationship dynamics in transition to parenthood].

In an prospective longitudinal study of family development, 39 healthy couples were examined by the Giessen-Test 1-3 months before and 1 month and 1 year after the birth of their first child. The self-assessment and the assessment of the other spouse were recorded from both partners. By this procedure information relating to intrapsychic and interpersonal dynamics of the couple was obtained. It was evident, that during the transition from the preto the postnatal period the assessments at the personal as well as at the couples' level remained very stable. Our sample showed considerable differences compared to a representative couple sample. These differences decreased by the time of the third survey one year after the birth of the child. Although pregnancy and the birth of the first child seem to cause specific, but transitory changes in the parents self-images and in the dynamics of their relationship, our results support the notion of a high stability during the transition to parenthood with regard to the self-concepts of mother and father and especially to the structure of the parental dyad.

[Operationalized psychodynamic diagnosis in childhood and adolescence--a contribution to quality assurance].

Adopting a therapeutic view on diagnosis in child and adolescent psychiatry a psychodynamic diagnostic manual seems to be necessary, that may provide reliability in the assessment of intrapsychic processes. Multiaxial diagnosis of children's and adolescent's psychiatric disturbances may thus be completed with additional psychodynamic assessment procedures. In adults an operationalized procedure of psychodynamic assessment (OPD) already exists. In child and adolescent psychiatry a working group for psychodynamic diagnostic (OPD-KJ) has been founded. In 4 areas (subjective experiences of disturbances, relationships, intrapsychic conflicts and psychic structure) diagnostic manuals are being worked out. Fundamental questions of developmental psychopathology and diagnostic procedures have to be addressed. The basic proposition takes into account that children should not be psychodynamically compared with adults, but normally have a basic capacity to adapt to environment on a high level of psychic organisation in all developmental stages. Reports of the ongoing work are being given.
