Transition of myelofibrosis to polycythaemia vera.

A case of typical myelofibrosis with a huge spleen is described in a 62-year old man. During the subsequent 2-year follow-up, a clinical picture of polycythaemia vera with pancytosis and disappearance of the marrow fibrosis was observed. The pancytosis necessitated treatment with busulphan and frequent phlebotomies. The transformation was associated with prednisone treatment for a suspected haemolytic state. During this treatment, the spleen no longer became enlarged on clinical palpation, although it was still enlarged at post mortem examination, but much less than 2 years earlier.

Haemostatic effect of prednisolone in thrombocytopenia.

In a short-time study, 8 patients with thrombocytopenia and clinical signs of bleeding were experimentally treated with prednisolone as the only therapeutic measure to control the bleeding tendency. 2 patients had idiopathic thrombocytopenic purpura and 6 acute non-lymphocytic leukaemia. 1 of the leukaemic patients was thrombocytopenic on admission. 5 had thrombocytopenia due to antileukaemic chemotherapy. The capillary fragility test, evaluated blindly, and bleeding time determination were performed just before treatment and repeated once after 2-4 d of treatment. Platelet counts were carried out simultaneously. Capillary resistance, determined by the capillary fragility test, improved greatly during the administration of prednisolone, irrespective of the platelet count, and no serious bleeding occurred. The bleeding time remained prolonged.

Virus antibodies in the serum of patients with liver disease.

Two hundred and twenty-six patients suspected of having liver disease were examined clinically, by a liver biopsy and laboratory test, according to a prospective scheme. Blood samples obtained just before the liver biopsy were coded and subsequently examined blindly, using the complement fixation test (CFT). The antigens were influenza A and B viruses, parainfluenza 1, 2, and 3, respiratory syncytial viruses, varicella, morbilli, cytomegalo and herpes simplex viruses. The sera were also examined by the CFT against Mycoplasma pneumoniae antigen. Antibodies against rubella virus were determined in a haemolysis-in-gel test. HBsAg and HBsAb were determined by a staphylococcal radioimmunoassay, and sera from patients with chronic active hepatitis (CAH) and chronic persistent hepatitis (CPH) were also examined for antibodies against hepatitis A virus by radioimmunoassay. Highly significant antibody titres against morbilli virus were found in patients with CAH and CPH. Patients with CPH or liver cirrhosis also had significantly higher titres against rubella virus than other groups. Some patients with liver granulomas had high titres against rubella virus. Only in one patient with CAH was a positive test for HBsAg found, and in one a positive test for HBsAb. Seven patients in the CAH and CPH groups had very high titres against both rubella and morbilli viruses.

Meningococcal disease in the Norwegian Armed Forces 1967-1979. Some epidemiological aspects.

The epidemiology of meningococcal disease in the Norwegian Armed Forces has been studied for the years 1974-79 and compared with the situation in the years 1967-73. The clustering of cases was examined with special reference to vaccination. The annual incidence rate increased from 24 to 43 cases per 100,000 men. However, this is only half the relative total increase in Norway in the same period. When 1975-79 figures were weighted according to the military age distribution, the combined rate for conscripts was four times the civilian rate. The weekly risk during the three month recruit training was three to ten times greater than later on. A malignancy change was demonstrated by a fourfold increase in the proportion of septicaemia and a fivefold increase in the case fatality.
