[Preoperatively asymptomatic left adrenal incidentaloma].

Since the introduction of new, very sensitive imaging procedures (ultrasonography, CT, MRI) the incidence of incidentally discovered, asymptomatic adrenal masses has clearly increased. These tumours are a new challenge to develop therapy and preoperative evaluation strategies, as the following case report shows. In a 29-year-old asymptomatic female patient a large adrenal tumour on the left side was discovered on abdominal ultrasonography during pregnancy. The further endocrinological examinations were normal. The patient was operated on post partum. During intraoperative manipulation of the tumour we observed an excessive increase in blood-pressure and heart rate. Shortly after removal of the tumour and immediate sympatholytic treatment the situation was under control. The intraoperative clinical situation and histological work-up of the tumour led to the diagnosis of phaeochromocytoma. The problem of adrenal incidentalomas is discussed on the basis of the current literature and an algorithm for preoperative evaluation is presented.

Medical coverage analysis for Wisconsin's Olympics: the Badger State Games.

The objective of this analysis is to determine the prevalence and severity of injuries encountered during the 1994-96 Badger State Summer Games Finals. Allocation of available medical personnel can be determined with this information. Medical contact with an athlete required an evaluation form to be completed by the health care professional covering the event. Information was compiled and analyzed to determine injury frequency and severity. Of the 31,580 athletes competing over the three year period in the 11 sports provided with medical personnel, 285 suffered a reportable injury. Soccer and basketball had the highest number of reported injuries with 68 and 65 injuries respectively. Basketball (2.00%), cycling (1.59%), wrestling (1.50%) and roller hockey (1.24%) had the highest injury rates. Severity of injury determined by the number of injuries transported to a medical facility found wrestling (23), soccer (22), basketball (11), and cycling (6) with the highest numbers of severe injuries. Wrestling (1.27%), basketball (.34%), soccer (.32%), and cycling (.21%) had the highest rate of severe injury. The most common sustained injuries were found to be sprains, strains, skin wounds, and contusions. These four types of injuries made up 70.18% of the injuries sustained. In conclusion, non-physician medical presence may be adequate coverage in most venues at multi-sport athletic competitions like the Badger State Games because of the relatively low frequency of severe injuries.

[Nanofiltration in production of Beriplex P/N: increasing the capacity of virus elimination while maintaining product quality].

For the manufacture of the PCC Beriplex P/N, nanofiltration was introduced into the production process of Beriplex HS providing an additional means to heat treatment for the clearance/inactivation of viruses. By nanofiltration, large enveloped viruses (HSV-1, HIV-1) were completely eliminated by a factor of more than 7 log10. While medium-sized enveloped viruses (HBV, BVDV) were cleared by a factor of approximately 4 log10, small non-enveloped viruses (poliovirus) were not removed. The product profile remained, no thrombogenic activities were detected.

Inactivation of HIV, HBV, HCV related viruses and other viruses in human plasma derivatives by pasteurisation.

Laboratory studies were made to verify the capacity and efficacy of the pasteurisation step (heat treatment at 60 degrees C for 10 hours in stabilized aqueous solution) in inactivating various pathogenic viruses (e.g. HIV, HBV, HCV related viruses, HAV, HSV, poliovirus). These experiments proved that pasteurisation effectively inactivates a broad spectrum of viruses with a range of different physicochemical properties. The results, summarized in inactivation kinetics, demonstrate that complete inactivation of high quantities of both enveloped (e.g. HIV, HCV related viruses) and non-enveloped (e.g. poliovirus) viruses takes place during pasteurisation. In clinical trials on the virus safety of plasma derivatives no case of AIDS, hepatitis B or C occurred. From these data we conclude that heat treatment at 60 degrees C in a stabilized aqueous solution confers a high margin of safety on human plasma protein preparations regarding the transmission of diseases such as AIDS or hepatitis.

Degradation of dioxane, tetrahydrofuran and other cyclic ethers by an environmental Rhodococcus strain.

By enrichment and isolation techniques bacterial strains with the capacity to grow on aliphatic cyclic ethers (dioxane, tetrahydrofuran, 1,3-dioxolane) have been isolated. Six strains that degrade tetrahydrofuran were classified as belonging to the genus Rhodococcus. One of two strains that degrade dioxane instead of or in combination with tetrahydrofuran was further characterized and a hypothetical catabolic pathway comprising an initial 2-hydroxylation and several oxidation steps is postulated.

[Transient normalisation of CK blood levels in patients susceptible to malignant hyperthermia by oral administration of dantrolene (author's transl)].

Five subjects, all relatives of two patients who suffered from malignant hyperthermia, were tested for blood CK activity before and after a course of dantrolene sodium. (4 mg/kg/day orally for three days.) In all the cases, the CK levels which were significantly elevated prior to the drug administration, dropped to normal values after three days of treatment returning to the original levels after eight days. The effect of the drug is most likely related to a temporary normalisation of muscle metabolism which may prevent the development of malignant hyperthermia in susceptible patients. The blood CK activity can be a valuable indicator in determining the effective dose of dantrolene. Prophylactic drug therapy should be considered even when special anesthesia techniques are used.

[Creatininphosphokinase isoenzymes in anaesthesia-induced myoglobinuria (AIM) (author's transl)].

During a three year period, pediatric patients 4 to 16 years old were post-operatively screened for anaesthesia-induced myoglobinuria. All investigated cases had general anaesthesia and received succinylcholine prior to intubation. Anaesthetics were predominantly performed for tonsillectomies and/or adenoidectomies. Of a total of 1704 anaesthetics, 7 (0.4%) resulted in postoperative myoglobinuria without hyperthermia. Anaesthesia-induced myoglobinuria occurs more often than previously presented. This abnormal reaction to succinylcholine is not harmless in all cases. The possible genetic influence on total CPK and its isoenzymes is discussed.

[Studies on the relationship between 2-iminothiazolidine-4-carboxylic acid and the thiocyanate metabolism in the guinea-pig (author's transl)].

In vitro and in vivo experiments have been carried out to elucidate the metabolism of 2-iminothiazolidine-4-carboxylic acid (ITC). By using 35S-ITC, the formation of 35SCN as well as of 35S-containing ITC metabolites could be excluded. As compared to the findings from control animals, the serum thiocyanate levels determined in guinea-pigs after oral administration of ITC were unchanged.

[Extra-cardial rise of the CPK in the screening of urgent diagnostic (among others, a case of anaesthesia induced myoglobinuria and a case of malignant hypertermia) (author's transl)].

Tests are being carried out on 37 patients and healthy volunteers, to differentiate the various, from heart independent, creatin-phosphokinase (CPK) increases, in order to recognize anaesthesia-endangered patients. A rise of the serum-CPK-level, dependent on trauma is insignificant for the anaesthesia. A significant rise of the CPK during a gener al anaesthesia and particularly extreme high values after 24 hours, (example, more than 10 000 U/l) is reason to think of a course of a malignant hyperthermia or myoglobinuria in a subclinical form. Relatives of hyperthermia- or myoglobinuria suspected families are in all cases anaesthesia-endangered, even with a very small rise of CPK-values. The knowledge of these connections is highly important also for internal medical spheres.

[Effect of cordycepin on the activation of RNA and protein synthesis at the early inhibition stages of Agrostemma githago embryos].

In embryos of Agrostemma githago pre-rRNA, ribosomal, heterogeneous, and transfer RNA is transcribed at 0 to 3 hrs of inhibition. An essential amount of hnRNA/mRNA is polyadenylated and can be prepared by affinity chromatography on poly(U)-sepharose columns. At very low concentrations of cordycepin synthesis of poly(A)+-RNA is especially inhibited. At increasing concentration of cordycepin the synthesis of ribosomal RNA is inhibited too. According to these results cordycepin is a suitable inhibitor to study the function of post-transcriptional polyadenylation of mRNA at the regulation of early protein synthesis of imbibing Agrostemma embryos. Incorporation of [3H]-leucine into protein occurring from 0 to 3 hrs of inhibition is not remarkably inhibited by cordycepin. These results may indicate that neither synthesis of mRNA nor its polyadenylation nor the polyadenylation of stable mRNA is involved in the regulation of early protein synthesis.

[Reaction of synovial membranes in knee joint with primary extra-articular systemic diseases (author's transl)].

Postmortal histologic examinations of the synovial membrane of both knee joints were conducted in 85 selected cases. The following conditions were given priority: diabetes mellitus, advanced renal insufficiency, liver cirrhosis with ascites, chronic insufficiency of the right heart, vericose syndrome of the lower extremities, tumours compressing organs of the pelvis minor as well as a few rare infectious and tumerous diseases. Comparison of the histomorphologic findings revealed consistancies such as pronounced ultravillous branching of the synovial membrane with villous hyalinosis in diabetes mellitus, increased, coarse villi formation in renal insufficiency and liver cirrhosis, edema of the synovial membrane in chronic insufficiency of the right heart and renal insufficiency as well as a number of nonspecific reactions of the synovial membrane in obstruction of venous drainage. In the context of generalized fibrotic processes such as in Ormond's disease and scleroderma, similar reactions of the synovial membrane are pronounced. Arthralgic complaints and secondary arthroses in those systemic diseases not primarily involving joints can be at least partially clarified by histomorphologic findings.

[Cogan's syndrome with angitis of cranial nerves, aortitis, endocarditis, and glomerulonephritis (author's transl)].

A 34-year-old woman died of left-heart failure due to combinated aortic-valve disease three years after manifestation of Cogan's syndrome characterized by sudden inner-ear deafness, loss of equilibrium, interstitial keratitis, and progressive loss of vision during pregnancy. At necropsy there was evidence of recurrent endocarditis of the aortic valves with stenosis and regurgitation, severe angitis of the thoracic aorta with marked secondary sclerosing changes as the cause of the heart failure. Angitis within the optical fasciculus and stato-acoustic nerve was the cause of the vestibular and optical defects. Primary changes in the visual and auditory cortices, the retina and inner ear were excluded as causes. There was also acute membrano-proliferative glomerulonephritis, which may have been coincidental.

Investigations on the suitability of mice for potency testing of FMD vaccines.

Potency testing of FMD vaccines by a three-point method was elaborated. Mice myotropic FMD virus was used as challenge virus. With this test the immunogenicity of FMD vaccines based on BHK, calf kidney or swine kidney was examined. The results are reproducible and allow quantification of the antigenic value of FMD vaccines within statistical limits.
