Efficacy and tolerability of once-daily tazarotene 0.1% gel versus once-daily tretinoin 0.025% gel in the treatment of facial acne vulgaris: a randomized trial.

Tazarotene 0.1% gel and tretinoin 0.025% gel are both effective in the treatment of acne vulgaris. Results of a multicenter, double-blind, randomized, parallel-group study that compared the efficacy and tolerability of these drugs are presented here. A total of 143 patients with mild-to-moderate facial acne vulgaris were randomized to receive tazarotene 0.1% gel or tretinoin 0.025% gel once daily for 12 weeks. Tazarotene 0.1% gel was more effective than tretinoin 0.025% gel in reducing the open comedo count (P < or = .05), the total noninflammatory lesion count (P < or = .05), and the total inflammatory lesion count (not statistically significant). At some time points, tazarotene was associated with increased irritation, but peeling, erythema, dryness, burning, and itching never exceeded trace levels. We conclude that tazarotene 0.1% gel is more effective than tretinoin 0.025% gel in reducing noninflammatory lesions and similarly effective in reducing inflammatory lesions.

High-pressure directed water jets as a cause of severe bilateral intraocular injuries.

PURPOSE: To draw attention to the characteristic features of eye injuries caused by high-pressure water jets. METHOD: We examined three patients with bilateral eye injuries caused by directed high-pressure water jets. RESULTS: All three patients had reduced visual acuity bilaterally, extensive eyelid ecchymosis, subconjunctival hemorrhages, hyphema, iris sphincter rupture, transient increase in intraocular pressure, and inferior commotio retinae. These injuries were confined primarily to the lower parts of the eyes. CONCLUSIONS: High-pressure water jets may cause bilateral eye injuries affecting primarily the anterior and inferior parts of the eyes. Because of concern about late effects of injuries, long-term follow-up is recommended.

Identification of digitalis-like compounds in human cataractous lenses.

Human cataractous lens nuclei extract inhibited, in a dose-dependent fashion, [3H]ouabain binding to rat brain synaptosomes and microsomal Na(+)- and K(+)-dependent adenosine triphosphate (Na+, K(+)-ATPase) activity and interacted with anti-digoxin antibodies. The compounds responsible for these activities, termed digitalis-like compounds (DLC), were also detected in bovine, rat, cat and rabbit, normal, transparent lenses, but the levels were only 0.7-5.4% of the average levels in the cataractous human lenses. DLC from the human cataractous lenses were purified by a procedure consisting of organic extractions and batch chromatography followed by filtration through a 3000 Da cut-off filter and subsequent separations using reverse-phase high-performance liquid chromatography. The presence of DLC in the different fractions obtained in the chromatograms was monitored by their ability to inhibit [3H]ouabain binding and Na+, K(+)-ATPase activity. Based on chemical ionization mass spectrometry together with ultraviolet spectrometry and biological characterization, it is suggested that new bufodienolides, 19-norbufalin and 19-norbufalin peptide derivatives are responsible for the endogenous DLC activity. It is proposed that these compounds may regulate Na+, K(+)-ATPase activity in the lens under some physiological and pathological conditions.

Retinal detachment with high myopia in the Rubinstein-Taybi syndrome.

A case of rhegmatogenous retinal detachment with high myopia is presented in a 17 year old boy with the typical characteristics of the Rubinstein-Taybi syndrome. Although multiple eye anomalies are known to occur in this syndrome, the occurrence of retinal detachment has not been reported up to now. The importance of including a thorough fundus examination in the routine eye examination of these patients is emphasized.

Optic disc drusen and hypermetropia.

We report the case of a 41 year old lady with high hypermetropia and optic disc drusen (ODD) in both eyes. A recent study has shown that ODD has a greater tendency to form in eyes with a small scleral canal. It would therefore seem probable to find a higher rate of ODD in hypermetropic eyes. Some of the current literature denies this connection. In this article we discuss the pathogenesis of ODD and the possible connection between this entity and "high" hypermetropia.

Essential thrombocythemia: a rare case of central retinal artery occlusion.

Essential thrombocythemia is a myeloproliferative disorder with an increased amount of abnormal platelets, causing both hemorrhagic and thrombotic pathology. Some of its systemic complications include deep vein thrombosis, pulmonary emboli, myocardial infarcts, and renal vessel thrombosis among others. We present the rare case of a woman suffering from essential thrombocythemia with the sudden loss of vision in her right eye, caused by central retinal artery occlusion. A possible connection between these two disorders is discussed.

A metabolic analysis of high myopia and senile macular degeneration.

Abnormal zinc and copper metabolism have been described in several retinal disorders affecting the retinal pigment epithelium. An infrequent presentation of senile macular degeneration in high myopia is well known. We examined the blood for zinc and copper and the urine for copper of three groups of patients, one consisting of high myopia, another with high myopia and primary retinal detachment and the other a group of patients with senile macular degeneration. When comparing the above groups of patients, we found statistically elevated values of serum zinc (p = less than 0.005) in the group of patients with senile macular degeneration. The significance of these findings, and a correlation with the pathogeneses of high myopia and senile macular degeneration is discussed.

Changes in the zinc and copper metabolism after panretinal photocoagulation in the rabbit.

In a previous work on a group of patients with diabetic retinopathy, the effect of laser photocoagulation on the serum zinc and copper levels was evaluated. In many of those patients, an increase in the serum zinc and copper levels was found one day after the laser treatment. In the present work, changes in the serum concentration of these metals after xenon arc photocoagulation were assessed in the rabbit. A significant rise was found one day after the treatment in the level of serum copper (P less than .005), and one week thereafter in the level of serum zinc (P less than .005). The significance of these results and their connection to the retinal pigment epithelium and choroid is discussed.

Chorioretinitis and extrapulmonary tuberculosis.

A case of tuberculous uveitis that presented 34 years after a primary articular focus is evaluated. The pathogenesis of the infection and the role of the tuberculin skin test in the diagnosis are discussed. Isoniazid was used with good therapeutic results. Fluorescein angiography was found to be essential in the follow-up of the choroidal lesions. Subretinal neovascularization was managed with argon laser photocoagulation.

Zinc and copper metabolism in oculocutaneous albinism in the Caucasian.

Abnormalities in the zinc and copper metabolism have been described in some pigmentary retinopathies. In these eye disorders changes in the Retinal Pigment Epithelium are a common finding. Albinism is a manifestation of a metabolic disorder with abnormalities of pigment formation which also occurs in this mentioned retinal layer. Recently, we described abnormalities in the metabolism of these metals in black albino patients. In them, elevated values of ceruloplasmin were found. In the present study we performed a similar work among Caucasians. When both the Caucasian albino and control groups were compared, no statistical difference in the ceruloplasmin, serum zinc and copper and their concentration in urine was present. However, in 7 patients of the Caucasian albino group the serum zinc was distinctly elevated. The mean concentration of ceruloplasmin was high as well. No definitive correlation in regard to the positivity or negativity of tyrosinase was observed. Our findings may support our previous observations regarding albinism as a possible primary abnormality of zinc and copper metabolism.

Copper and zinc metabolism in high myopic patients with retinal detachment evaluation of the Cu/Zn ratio.

In a previous work abnormalities in the copper and zinc metabolism in high myopic patients were found. Primary Retinal Detachment is a common complication of High Myopia. In order to establish further their relationship, a study of the metabolism of these metals was performed in myopic patients with Retinal Detachment. A significant high copper and zinc concentration in the urine of these patients was found. An inverse ratio of copper and zinc between their concentration in the subretinal fluid and their concentration in serum was present as well. The significance of these findings and their connection to the Retinal Pigment Epithelium is discussed.

Copper and zinc levels after external damage by laser photocoagulation to the retinal pigment epithelium. Preliminary reports.

Several reports have appeared in the literature linking abnormalities in the Retinal Pigment Epithelium (RPE) related with changes in the metabolism of copper and zinc. It has not been yet elucidated if the changes in the metabolism of these metals are secondary to a progressive disease that affects the RPE, or the changes found in this retinal layer are a manifestation of a basic metabolic disease. In the present study a group of 22 diabetic patients with different degree of retinopathy was evaluated. Eleven of these patients underwent laser treatment, and blood and urine copper and zinc were assessed prior to the treatment and at intervals of one day, one week and one month post treatment. All these patients were compared with a control group. We found a rise in the mean concentration of copper and zinc in serum, and zinc in the urine, in those patients who underwent more than 600 laser applications. The significance of these findings and their relationship to the RPE is discussed.

Treatment of ocular inflammation with diclofenac sodium: double-blind trial following cataract surgery.

A controlled trial of diclofenac sodium was carried out in 100 patients undergoing intracapsular cataract extraction. All patients were treated with dexamethasone eyedrops twice daily, plus either diclofenac sodium 50 mg tid or matching placebo tid, started 24 hours before surgery and continued for four weeks. In comparison with those in the placebo group, patients in the diclofenac group showed significant decreases in most signs of ocular inflammation. Diclofenac therapy was well tolerated, except in one patient, and may safely be used to reduce intraocular inflammation.

Zinc and copper metabolism in patients with senile macular degeneration.

Anatomic and functional studies in patients with senile macular degeneration show changes in this retinal layer. We examined the blood and urine of two groups of patients, one consisting of patients with senile macular degeneration and the other a control group, for zinc and copper in serum and urine. In the former group we found elevated values of serum zinc and serum copper that were highly significant when compared with the control group. The significance of these findings and their connection to the retinal pigment epithelium is discussed.

A metabolic aspect of high myopia.

Abnormal zinc and copper metabolism has been described in some retinal disorders affecting the retinal pigment epithelium. Anatomic studies in high myopic eyes, show important changes in this retinal layer. We examined the blood for zinc and copper and urine for copper in different groups of high myopic black and white patients, and high myopic white patients with primary retinal detachment. In the white myopic groups, many of the patients showed high serum zinc and low serum copper values. In the high myopic patients with primary retinal detachment, the serum zinc and copper were found to be distinctly elevated and of statistical significance. A relative inverse copper/zinc ratio between the concentration of these metals in serum and their concentration in the subretinal fluid was noted as well. The significance of these findings and their connection to the retinal pigment epithelium is discussed.
