[Clinical study of Supranettes pads in the treatment of seasonal or perennial allergic conjunctivitis in children].

PURPOSE: Ocular allergy is a common disease that is on the rise. Seasonal and perennial conjunctivitis are the most common forms of ocular allergy. Itching, hyperemia, foreign body sensation, irritation, blurred vision or ocular dryness sensations, tearing, and photophobia are the clinical signs observed in patients. A topical antiallergic treatment is prescribed associated with ocular cleaning to contribute to an antigen washing of the ocular surface. Our study aimed to evaluate the use of Supranettes pads as supplementation treatment in children for seasonal or perennial conjunctivitis. MATERIAL AND METHODS: Our prospective study was done in children with a diagnosis of seasonal or perennial clinical ocular allergy. Every symptom was rated from 0 to 4 according to the intensity described by the children and their parents. Twenty children (range, 4-10 years) presenting an ocular allergy were included in the study. A topical antiallergic treatment membrane stabilizer or antihistamine and Supranettes pads were prescribed for ocular cleaning of one randomized selected eye four times a day. Patients were seen in consultation 1 month later to evaluate the treatment. The ocular comfort of each eye, one treated with Supranettes pads and the other not treated by Supranettes pads, was evaluated by the child helped by parents on a graduated self-evaluation scale (range, 0-10). The Mann-Whitney statistical test was used to differentiate the two groups. RESULTS: Of the 20 patients, eleven presented seasonal conjunctivitis and nine perennial conjunctivitis. Six of them presented conjunctivitis with a slight to moderate superficial punctuate keratitis. At the consultation on day 28+/-3, patients who were first treated showed clear improvement of clinical signs with the prescribed treatment. The statistical comparison of the two groups confirmed a significant difference (p<0.05) to the advantage of the group using the Supranettes pads. DISCUSSION: Ocular washing is essential in the treatment of allergic conjunctivitis. The new galenic form of ocular cleaning by physiological serum-impregnated pads is simple to use for children. An objective significant difference is difficult to observe in 1 month. CONCLUSION: This study underlines the importance of the comfort of Supranettes pads evaluated in the treatment of children's conjunctivitis. It confirms the advantage of developing evaluation studies including patient satisfaction and quality of life in ophthalmology.

[Conjunctival provocation test: recommendations].

The diagnosis of ocular allergy, which has become a real public health problem occurring in 25% of the general population and continues to rise, requires allergic testing. Skin and blood tests combined with interviewing the patient demonstrate sensitivity to one or more antigens. However, while allergic testing and interviews generally reveal the cause of the symptoms, only the conjunctival provocation test (CPT) provides clear evidence of a functional relation between the conjunctival pathology and exposure to the antigen. Since there is still no validated consensus regarding the criteria for positivity of the CPT in routine practice, the working group attempted to establish the major guidelines for conducting the test, to standardize approaches and pinpoint areas of uncertainty. The group established a consensus regarding the conditions for using the test (patient selection, allergens to be tested, usable concentrations, the test protocol, the signs and symptoms to be assessed, interpretation of results, follow-up) and identified two distinct patient groups (outpatients and hospitalized patients) in whom test-taking conditions are different. The outpatient group includes patients suffering from benign forms of conjunctivitis (acute, seasonal, or chronic) and those sensitive to the usual allergens (mainly domestic and airborne). In these cases, the test is given in a medical setting with an allergologist or an ophthalmologist who instills the drops relatively rapidly. The criterion for positivity is the extent of the pruritus. On the other hand, in the hospital population, which is composed of patients suffering from vernal conjunctivitis, the attempt to establish the triggering factor requires a more elaborate protocol carried out in an ophthalmologic setting and based on the association of a range of clinical and biological factors. The deliberations of the working group should lead to the standardization of the CPT procedure for diagnostic purposes.

[Lasik correction of compound myopic astigmatism with positive versus negative cylinder ablations].

PURPOSE: To compare the predictability, efficacy, and safety of two methods of Lasik correction of compound myopic astigmatism: positive cylinder ablation versus negative cylinder ablation. METHODS: Twenty nine eyes of 19 patients were retrospectively analyzed. They had undergone Hansatome or ALK-e flap keratectomy, and a Technolas Keracor 217c laser ablation. Group 1 (14 eyes) was corrected with the positive cylinder program, group 2 (15 eyes) with the negative cylinder program. Spherical equivalent (SE), cylinder, vector analysis, and best corrected and uncorrected visual acuities (BCVA, UVA) were compared in the two groups. Minimum follow-up was 6 months. RESULTS: The preoperative mean SEs for groups 1 and 2 were, respectively, - 7.09+/-3.36 D and - 8.05+/-2.27 D (NS). Mean cylinders were 1.73+/-0.88 (group 1) and +/-0.74 (group 2) D. Visual acuities were not statistically different in the two groups. Postoperative mean SEs were, respectively, - 0.57+/-1.58 D and - 0.68+/-0.88 D. Mean cylinder was, respectively, 0.57 +/- 0.54 D and 0.7 +/- 0.46 D in groups 1 and 2. Astigmatism induced by surgery, calculated by vector analyses was, respectively, 1.36 +/- 0.66 D and 1.54 +/- 0.56 D. Percentages of UVA above 20/25 were, respectively, 50% and 35%. None of those differences was statistically significant. One eye lost one line of BCVA. CONCLUSION: The positive cylinder ablation method makes a larger optical zone of ablation possible with the same central deepness of ablation. We found that the predictability, efficacy, and safety of this technique compares well with the negative cylinder ablation in compound myopic astigmatism.

[Evaluation of the quality of life in ophthalmology].

THE IMPORTANCE OF VISUAL FUNCTION IN THE ASSESSMENT OF QUALITY OF LIFE: The notion of visual function is related to visual acuity but also to many other parameters such as the visual field, perception of colour, contrasts, and the resistance to blinding. These factors are difficult to measure during routine ophthalmic examination but can be assessed during questionnaires on quality of life. MARKERS OF QUALITY OF LIFE IN OPHTHALMOLOGY: Various quality of life questionnaires have been developed in ophthalmology because the non-specific generic instruments appear inadequate. The SF 36 and SF 20 scales, which are generic instruments widely used in many fields, do not adequately explore the problems related to vision. The first efficient instrument is the VF 14, which is a questionnaire specific to ophthalmic diseases, developed by C. Mangione in 1992. This self-administered questionnaire permits calculation of a 0 to 100 score exploring 5 dimensions (long-sight acuity, near-sight acuity, unclear sight, and driving during the day and at night) during 14 activities with 18 questions. It was translated and adapted into French by Gresset in 1997. Today it is the most commonly used questionnaire in ophthalmology, particularly in the assessment of efficacy and impact in surgery. Along the other specific questionnaires developed, there is the NEI-VQF which was created in 1995 to assess vision and the impact of visual problems on the quality of life of patients, independently of an ophthalmic pathology. Many studies have been conducted on various diseases that affect vision, such as diabetes or hypertension. THE LIMITS OF EXISTING INSTRUMENTS: The specific scales appear far more sensitive and specific than generic scales with regard to ophthalmic problems, but they provide less information on the general status of the patient, except for the NEI-VQF. They are limited in some parameters and do not, unfortunately, take into account the patient's age. No specific scale has been developed for children or adolescents, although the impact of vision on daily life is fundamental. The complexity of vision means that the elaboration of an ideal instrument is difficult. However, in the meantime, it is essential that the practitioners continue to use and to test the instruments available in order to improve with regard to pathologies, or in particular sub-groups of the population.

The lateral inframalleolar fat pad: a poorly recognized anatomical structure.

The authors carried out an anatomical histological study of the lateral inframalleolar region in order to improve knowledge of a fat pad of the lateral aspect of the ankle which is not mentioned in most anatomical texts. Twenty-four ankles, from 12 cadavers, free of any local pathology or malformation, were studied. Twenty ankles were dissected, of which, five had samples taken for histological examination. Colored latex was injected into the joint cavities of the last four ankles before anatomical cuts were made in three planes. This study allowed the description of the fat pad which we have called the lateral inframalleolar fat pad (LIMFP). It is oval and made up of a classical unilocular fatty tissue which is clearly distinct from the subcutaneous plane. We have defined its relationships and in particular, the neurovascular ones. The sural nerve which supplies cutaneous sensation to part of the 5th toe runs over the surface of the fat pad. It is accompanied on this part of its course by the short saphenous vein which gives off a medial perforator which traverses the LIMFP. Because of these neurovascular relationships, the LIMFP may play a role in the pathophysiology of neuralgias of the lateral aspect of the 5th toe or of the 4th digital interspace. It should be recognized before any operations on the lower limbs, in particular, before any plastic surgical liposuction, in order to optimize the resection volume to prevent inaesthetic over correction or under corrections which are often confused with residual edema.

One year clinical results of photoastigmatic refractive keratectomy for compound myopic astigmatism.

PURPOSE: To evaluate the efficacy, predictability, and safety of excimer laser photoastigmatic refractive keratectomy (PARK) to correct compound myopic astigmatism. SETTING: Departments of Ophthalmology, Robert Debré Hospital and Rothschild Foundation, Paris, France. METHODS: This retrospective study included 27 eyes with compound myopic astigmatism treated with a Nidek EC 5000 excimer laser. The refractive results were measured at 1 year, and the cylindrical component was analyzed by the Alpins method. Mean preoperative myopia was -4.50 diopters (D) (range -0.75 to -4.00 D) and mean preoperative cylinder, -1.64 D (range -0.75 to -4.00 D). RESULTS: At 1 year, the spherical equivalent was -0.47 D (range +1.00 to -3.00 D) and residual subjective astigmatism, -0.40 (range -0.25 to -1.50 D). Uncorrected visual acuity of 20/40 or better was obtained in 22 of the 27 eyes; 21 eyes were within +/- 1.0 D of emmetropia. Vector analysis showed a mean coefficient adjustment of 1.50 D +/- 0.53 (SD), a mean axis shift of 2.64 +/- 12.10 degrees, and a mean magnitude of error of 0.45 +/- 0.56 D. Haze was absent in 22 eyes and grade 1+ in 5 eyes. Five eyes gained 1 line of best corrected visual acuity and 3 lost 1 line. No patient lost more than 1 line. CONCLUSION: Excimer laser PARK successfully corrected low and moderate myopia combined with up to 4.0 D of astigmatism with a low mean angle of error. To increase the accuracy of toric ablation, specific algorithms for the cylinder component are needed.

The pterygoid venous plexuses.

The pterygoid venous plexuses are closely adjacent to the lateral pterygoid m. and occupy a considerable part of the infratemporal fossa (ITF). We have conducted a study after injection of colored latex in 10 unemblamed subjects in order to define the arrangement of the venous network and to assess the frequency of the different types of plexuses found. In all the subjects symmetry was noted between the right and left plexuses, with predominance of the venous layer in the upper part of the ITF. The pattern most often encountered was plexiform and complex. The development of these plexuses follows that of the masticatory muscles, whose contraction promotes drainage and represents an alternative venous pathway for drainage of the cranial cavity.

Computation of eyeball growth by magnetic resonance imaging (26.11.93).

The computation of eyeball growth has a wide field of applications in orbital and ocular pathology. An anatomic and MRI study to compute eyeball growth has been done. The anatomic study was made to determine the best MRI parameters to calculate the ocular surface; then this surface was computed by MRI in the neuro-ocular plane in fetuses and children aged from 0 to 13 years. The curve of eyeball growth shows rapid growth in utero and until 18 months, followed by a phase of slower growth. This method allows measurement in two dimensions, which we find complementary to ultrasound in the antenatal search for certain congenital malformations.

[Oculo-orbital anatomy and nuclear magnetic resonance: use in ocular growth and fetal pathology].

The computation of eyeball growth has a wide field of applications in orbital and ocular pathology. An anatomic, macroscopic, histologic and RMI study has correlated the different oculo-orbital signals with the anatomy: a study of compute eyeball growth has been done. The anatomic study was made to determine the best MRI parameters to calculate the ocular surface. The surface was computed by MRI T2 weighted in the neuro-ocular plane in children aged from 0 to 13 years. The curve of eyeball growth shows rapid growth in utero until 18 months and then a phase of slower growth. A second study with fetuses RMI of pregnant women showed the neuro-ocular plane of fetus. Application in antenatal pathology are interesting. Two cases of orbital tumor, with fetal diagnosis are analysed and histologically explained.

[Micro-endoscopy of the lacrimal apparatus combined with Erbium YAG laser: an anatomical study. New methods of imaging].

PURPOSE: To study the anatomic structure and the endoscopic aspect of the lacrimal drainage system and to evaluate the efficacy of the Erbium laser with microendoscope in lacrimal obstruction. METHODS: Twenty lacrimal ducts from ten fresh cadaver heads were used to lead a special rigid probe of 1.1. mm diameter (Endognost Schwind). This probe combined a flexible endoscopic probe of 500 mu, an Erbium laser fiber of 375 mu and an irrigation. The probe was introduced through the inferior canaliculus. Progressing in the lacrimal system, all the images were analysed. Then, we used the laser fiber to open the lacrimal sac mucosa and the lacrimal bone. RESULTS: The anatomo-endoscopic analysis was performed using endoscopic data. We appreciated essentially the walls of valves and sinuses that appeared only functional when increasing the irrigation flow. The laser could easily open the mucosa and the lacrimal bone but not the maxilla. DISCUSSION: The small size of the cannula allows to use the system for the diagnosis of lacrimal obstructions. The combined laser allows to treat at same time canalicular obstructions and lower obstructions. CONCLUSION: The endocanalicular study of the lacrimal drainage system can diagnose the type of obstruction and can treat it. A clinical study is requested to confirm these results.
