Contact allergy to Bronopol.

A total of 8149 patients were patch tested with the preservative Bronopol in 7 European contact clinics. The majority of patients (6507) were investigated in London. Reactivity was low, with a total of 10 irritant (0.12%) and 38 allergic reactions (0.47%). In only 17 cases (0.21%) was the patch test reaction to Bronopol considered to be of current or past clinical relevance. Concomitant sensitization to formaldehyde was present in about 1/3 of patients. Based on these figures, the present sensitization rate to Bronopol in Europe seems to be quite low, which may be related to its less frequent use as a preservative in cosmetics and medicaments in comparison to parabens or isothiazolinones.

Erythropoietic protoporphyria. The problem of a suitable screening test.

Erythropoietic protoporphyria (EPP) is characterized by increased red cell protoporphyrins and is included in the differential diagnosis of children presenting with photosensitivity. In the past 20 years, using the traditional solvent extraction qualitative screening test for blood porphyrins, the diagnosis of EPP had been missed in 9 out of 10 patients but recently, using fluorescence microscopy of erythrocytes, no patients with EPP have been missed. All 14 patients in Northern Ireland known to have EPP were recalled and it was found that fluorescence microscopic determination was positive in all cases. We recommend fluorescence microscopy as the screening test of choice for the detection of increased red cell porphyrins.

The effects of mental and physical stress on the incidence of skin disorders.

The effect of civil unrest and terrorism in Northern Ireland from 1969 on skin diseases which are generally accepted to be aggravated by stress and strain has been studied (1) by reporting cases making claims against the government for skin diseases developed as the result of violence and (2), studying changes in the pattern of skin diseases at skin clinics in Northern Ireland over the last 10 years. There has been little change in the pattern of skin diseases which could be attributed to mental or physical stress.

Industrial dermatitis in Northern Ireland.

Industrial injuries and disablement benefits paid for industrial dermatitis (PD 42) in Northern Ireland are analyzed by age, industry and substance responsible. The most significant trend is a decrease in the number of cases due to cement and an increase in the number of nickel cases.

Contact dermatitis in animal feed mill workers.

Three cases of contact dermatitis due to additives in animal feed substances are described occurring in animal feed workers. There were two cases of sensitivity to ethoxyquin and one to halquinol.

Aglossia-adactylia syndrome.

Aglossia-adactylia is described in two male patients, aged 31 and 21 years old. Including a previous reported case (Nevin, Dodge, and Kernohan, 1970) there are three patients with this syndrome in Northern Ireland. The aetiology is unknown but in spite of the extreme variability of the clinical manifestation, a dominant mutant gene cannot be ruled out.

Reactivation of scabies rash by methotrexate.

A 13-year-old female was on maintenance therapy for acute lymphoblastic leukemia. On three occasions she received methotrexate orally and each time this was associated with reactivation of scabies rash. The mechanisms for this phenomenon are discussed.
