[Epithelial lesions associated with gallbladder carcinoma. A methodical study of 32 cases].

Using mapping techniques, 32 consecutive gallbladder carcinomas diagnosed in our department were studied. The defined lesions were dysplasia, carcinoma in situ and superficial spreading carcinoma. A total of 2257 lesions were recorded from 1072 inclusions. All the defined lesions were present in about 10% of all sections. However, if each case is considered separately, dysplasia was present in 81.3% of cases, carcinoma in situ in 65.6% of cases and superficial spreading carcinoma in 68.8% of cases. An isolated focus of atypical epithelium without anatomic relation with the main tumor was demonstrated in only 3 cases and a close relation between invasive carcinoma and dysplastic lesions was observed in the remainder. Infiltration of Rokistansky-Aschoff sinuses was observed in 40% of cases. We were not able to demonstrate, on histological basis, differences between superficial spreading carcinoma and carcinoma in situ. We conclude that atypical lesions of the gallbladder are not frequently seen and that their presence must prompt the search of a carcinomatous lesion by means of serial sections of all the histological sample.

[Diagnosis and percutaneous drainage guided by ultrasonics in retroperitoneal abscess: experience with 16 cases].

The diagnosis of retroperitoneal abscess was made by ultrasound studies in 16 patients. Ultrasound guided percutaneous drainage followed in 10 patients confirming the diagnosis in all. A catheter was used in 8 and needle aspiration in 2. The 6 remaining patients were operated on. Patients with percutaneous drainage had better bacteriologic diagnosis and significantly shorter hospital stays than those who were submitted to immediate operation. Thus, ultrasound is an effective means for diagnosis and treatment of retroperitoneal abscess.

[Acute plasma cell leukemia].

Plasma Cell Leukemia is a very rare form of plasmocytic dyscrasia, whose clinical and pathological characteristics warrant its recognition as a distinct subentity. We report the case of a 60 years old man who presented a rapidly fatal acute plasma cell leukemia, with multiple osteolytic lesions, hipercalcemia, renal and cardiac failure.

[Ultrasonics in the diagnosis of choledocholithiasis].

The value of ultrasound in the diagnosis of choledocholithiasis is controversial, sensitivity figures ranging from 22 to 75%. We correlated the ultrasound study with operative findings in 38 patients. A diagnosis of common bile duct stone was made upon finding an echogenic image with or without acoustic shadow in the common bile duct; no attention was paid to the dilatation of the biliary tree. The sensitivity was 68% and the positive predictive value was 85%. Therefore, our results approach the highest reported figures for the value of this diagnostic technique.

[Ultrasound in the diagnosis of congenital malformations of the brain].

After the work published by Kossof (1974), on the use of ultrasound for the study of trans-sectional cuts in the brain of newborns, the technic has been widely used in the exploration of this organ. This study evaluates the use of ultrasound in the diagnosis of congenital malformations of the brain in 11 patients seen during a period of 3 years at a regional hospital of southern Chile. The classification introduced by Beberly and Zimmerman was used in order to ascribe each case to a correspondent phase of organogenesis. Recent studies over the advantages of ultrasound for the detection of congenital malformations of the brain have resulted in the replacement of traditional invasive methods by ultrasonography for the evaluation of these problems.
