Coordinating a large multicentered pediatric HIV clinical research study is particularly challenging for nurses. The cohort of patients and their families face multiple stressors, which make recruitment and compliance difficult. Additionally, when many departments within the institution are involved, coordination is often problematic. Ethical issues sometimes arise. Nurses must find ways to adapt what they learn from the study to their nursing practice. The nurse coordinators of the Pediatric Pulmonary and Cardiovascular Complications of Vertically Transmitted HIV (P2C2HIV) study discuss the unique challenges of such a project and the various strategies for successful completion.
A survey of the clinical research nurses (CRNs) associated with the Pediatric Pulmonary and Cardiovascular Complications of Vertically Transmitted HIV Infection Study was conducted to delineate the nurses' roles. The diversity of activities reported by the CRNs are presented, categorized, and compared to those stated in the protocol job descriptions. Additional dimensions of the multifaceted CRN role are identified. Aspects of the nurses' role associated with special needs of the study cohort and related implications are discussed.

Starting from the cyclicity of the anginal attack in variant angina, the authors point out the role of alkalosis, besides the richness in alfa-adrenoceptors of the great coronary trunks. At the level of the muscular cell there is a competition between H+-ions and Ca++-ions. The diminution of H+-ions as a result of alkalosis brings about the penetration of Ca++ into the cell and the appearance of the coronary spasm. So, we worked out an original method for the provocation of the spasm (the cold and hiperpneea test) and an original therapeutical procedure (by acetozolamid). We present, herein the first results scored in 16 patients.
