[Prognosis of cancer of the corpus uteri].

A statistical study of 240 cases of cancer of the body of the uterus treated throughout at the Institute Gustave-Roussy has led us to the following conclusions. The number of patients who survive drops rapidly after the first year of follow-up and this phenomenon is particularly marked in women who are more than 60 years of age. Mortality linked to other causes of death does not account for the total falling off in the number of those who survice among the older patients. A study of the clinical spread of the disease shows that most cancers stay confined to the body of the uterus and this really lessens the value of a classification based on spread of the disease for prognostic purposes. A systematic study of hysterograms taken before any treatment is started does on the other hand give us two prognostic pointers. The first is the size of the endometrial cavity and the second the maximum diameter of the tumour. These two radiological characteristics allow us to make an interesting division of cancers that are limited to the body of the uterus. The overall survival rate after 5 years is 61 per cent. When we take into account the dimensions of a tumour we can place patients into two groups; those with tumours that are less than 45 mm in diameter have a 71 per cent survival rate and those with tumours of greater than 45 mm diameter have a survival rate of 45 per cent.

Quality assurance in brachytherapy: the role of the ICRU in achieving uniformity in dose and volume specification for reporting.

A quality assurance programme in brachytherapy implies a general consensus on the method of dose and volume specification for reporting. This in turn implies a consensus on certain definitions of terms and concepts. For several decades, the ICRU (International Commission on Radiation Units and Measurements) has been actively involved in an effort to reach a consensus between different brachytherapy centres worldwide, and to improve uniformity in reporting. The ICRU has prepared two reports containing recommendations for reporting brachytherapy treatments. The first, report #38 published in 1985, deals with intracavitary therapy in gynecology. The second deals with interstitial therapy, and is now in press. A summary of these two ICRU reports is presented here. Some definitions of terms and concepts are first recalled and discussed: Total Reference Air Kerma (TRAK), gross tumor volume (GTV), clinical target volume (CTV), treated volume, mean central dose, dose uniformity parameters, etc. Specific recommendations for reporting interstitial and intracavitary brachytherapy are then presented.

[Rendu-Osler disease treated by endonasal radiotherapy (evaluation of 62 cases)].

Two groups of patients were treated by plesiocurietherapy. The first group of 17 patients received only one exposure to radium (between 1949 and 1960). The second group received one or more doses of irridium 192 (between 1960 and 1973). The average period for stabilization of the disease was 13,2 months with radium and 20,4 months with irridium. Unfortunately, this does not give a permanent cure and, for irridium 192, the percentage of successes is only 30 p. 100 after 5 years. Logically, radiotherapy with irridium 192 should only be used when other methods have failed. The progressive accumulation of radio-lesions may, in fact, make the use of other techniques impossible.
