Passive avoidance training results in increased responsiveness of voltage- and ligand-gated calcium channels in chick brain synaptoneurosomes.

A temporal cascade of events has been described from a number of biochemical investigations of passive avoidance training in day-old chicks. Among these, within minutes of training, there is a transient, enhanced release of glutamate and increased agonist and antagonist binding to N-methyl-D-aspartate-sensitive glutamate receptors in the intermediate medial hyperstriatum ventrale of the forebrain. Some 6.5 h later, alpha-amino-3-hydroxy-5-methyl-4-isoxazo lepropionate binding to glutamate receptors is also increased in the same region. These processes might be predicted to affect the uptake of calcium via voltage-sensitive calcium channels or glutamate receptor-associated channels, thereby changing the intracellular calcium concentration. To test this possibility, we have measured the calcium concentration in synaptoneurosomes, containing both pre- and postsynaptic elements, prepared from left and right intermediate medial hyperstriatum ventrale at various times following training, using Fura 2-AM as the indicator of intracellular calcium concentration. Synaptoneurosomes, prepared immediately and 5 min after training, were stimulated with 70 mM potassium chloride in the presence of 2 mM calcium, resulting in a significantly enhanced increase in calcium concentration in synaptoneurosomes from the left hemisphere of trained chicks. This effect was absent in samples obtained at later times after training. N-Methyl-D-aspartate (0.5 mM) induced a significant enhancement in the increase in calcium concentration in intermediate medial hyperstriatum ventrale from both left and right hemispheres 10 min and 30 min after training. At 3 h and 6 h after training, alpha-amino-3-hydroxy-5-methyl-4-isoxazo lepropionate (0.5 mM) induced a significantly enhanced increase in calcium concentration in samples from either hemisphere. These results suggest that immediately after training there is an engagement of both pre- and postsynaptic voltage-sensitive calcium channels, followed by an increased reponse to N-methyl-D-aspartate receptor stimulation, and coinciding with the enhanced calcium-dependent glutamate release and an increase in N-methyl-D-aspartate-sensitive glutamate receptor binding that has been reported previously. The alpha-amino-3-hydroxy-5-methyl-4-isoxazolepropionate-sensitive mechanisms are activated at a later stage of memory formation, when increased alpha-amino-3-hydroxy-5-methyl-4-isoxazolepropionate binding to glutamate receptors has been reported. Thus, responsiveness of calcium channels to agonist stimulation is implicated in temporally diverse stages in the cascade of events involved in memory formation following passive avoidance training in the chick.

Prolonged pyrexia in a diabetic due to systemic aspergillosis.

Immuno-compromised patients are susceptible to a variety of opportunistic infections. Systemic aspergillosis is one such common infection in neutropenic subjects. A case of primary cutaneous aspergillosis with fungimia in a diabetic is reported.

Infections in the elderly.

Ageing of the immune system is a complex process involving both humoral and cell mediated immunity. Along with decline in immunity, morphological changes in various organ makes the elderly especially vulnerable to infection. In clinical practice, infections of respiratory tract and urinary tract, endocarditis, septicaemia and tuberculosis are commonly encountered in elderly subjects Atypical clinical presentation, slow response to treatment and high mortality are hall marks of infection in order patients. Antibiotic therapy in elderly needs to be early, empirical and broad spectrum through parenteral route, with early change over to oral therapy. Aminopenicillins and cephalosporins are safer drugs in old age as compared to aminoglycosides. Pneumococcal and influenza vaccines are recommended in elderly subjects with medical conditions with higher risk of mortality and complications.

Biochemical studies on acid phosphatase in human placenta and fetal liver during development.

Activity and a few physiochemical properties of acid phosphatase (ACP) have been studied in different subcellular fractions of developing human placenta and fetal liver. The enzyme activity is discernible in all subfractions of each tissue already in the 6th week of gestation. The activity of the hydrolase in 20,000 g pellet, 105,000 g pellet and in 105,000 g supernatant of the placenta and in all fractions of the liver increases significantly with advancement of pregnancy until 26 weeks and declines in term placenta. However, 800 and 10,000 g pellets of the placenta show a gradual decrease in ACP activity with progression of gestation. Experiment with different inhibitors reveals that each tissue contains at least two types of ACP--a soluble one and a sedimentable one. It appears from the Lineweaver-Burk plot of kinetic study that sedimentable form has low Km and high Vmax values. Ag+, Cu++, Hg++, Co++ and Zn++ are inhibitory, while Fe++ and EDTA are stimulatory to the enzyme. The effect of different detergents, particularly with sodium deoxycholate and Triton X-100, shows the increase of the hydrolase activity, indicating 'latent nature' of the enzyme.

Haemodynamic advantage with single chamber rate responsive pacemakers over dual chamber pacemakers during exercise in chronotropic incompetence.

Sequential cardiac chamber activation and chronotropic response are amongst the important determinants of cardiac performance. This study compared VVIR mode with DDD and VVI mode to assess the contribution of these two factors in the determination of cardiac performance during exercise. Ten patients with a mean age of 68 +/- 5 years were studied during 1988-90, who had complete heart block and sinus node dysfunction with a mean LV ejection fraction of 51.5 +/- 6.25%. Five patients had DDD pacemakers and the rest had VVIR pacemakers. Patients were exercised first in VVI mode followed by either DDD or VVIR mode. Exercise duration, maximum rate pressure product, exercise capacity and cardiac output were measured with graded treadmill test in Bruce protocol. There was remarkable increase in the exercise duration (P < 0.001) and the maximum rate pressure product (P < 0.001) in patients with VVIR as compared to DDD and VVI mode. In DDD mode, as compared to VVI mode, this increase was less remarkable, though statistically significant (P < 0.005). Therefore, it is concluded that patients with chronotropic incompetence as well as complete heart block do better during dynamic exercise when they have VVIR mode compared to VVI or DDD mode.
