Long-term survival after chemotherapy with cisplatinum, adriamycin and cyclophosphamide for carcinoma of the ovary.

Forty six patients with FIGO Stage II, III, and IV ovarian cancer received treatment with cisplatinum, adriamycin and cyclophosphamide. After a minimum follow-up time of 60 months 13 patients remain free of recurrence. The overall 5-year survival is 39% and the median survival is 39 months. The 5-year survival for Stages II, III, and IV is 64%, 40% and 0% respectively. The 5-year actuarial survival for those who had less than 2 cm residual disease (including those with complete debulking) is 72%, while for those who had greater than 2 cm residual disease (including those who had biopsy only) is 15%. None of the patients have developed any second neoplasm.

Electron microscopic findings in benign deoxycorticosterone and 11-deoxycortisol-producing adrenal tumor.

The ultrastructural characteristics of a benign adrenal adenoma that produces deoxycorticosterone and 11-deoxycortisol were examined by transmission electron microscopy. Both large, round mitochondria with a few cristae in the peripheral portion and spherical or oval mitochondria with sacrotubular cristae were observed in adenoma cells. The development of agranular or granular endoplasmic reticulum varied from cell to cell. In some cells, many vacuoles and collagenous fibers were also seen. Thus, the adrenal tumor we examined was composed of the cells derived from the fascicular zone, which are associated with Cushing's syndrome, and the cells of the glomerular zone observed in association with primary aldosteronism.

Gastric DNA synthesis rate in different experimental models of duodenal reflux.

The aim of our investigation was to study the gastric mucosa DNA synthesis pattern in different experimental models of duodenal reflux. The following operations were performed on male Wistar rats: (A) sham-operation; (B) Roux-Y gastrectomy; (C) Billroth I; (D) Polya-type partial gastrectomy; (E) Billroth II; and (F) simple gastroenterostomy. Fifteen weeks later rats were sacrificed and histological and in vitro 3H thymidine autoradiographic studies were carried out. Gastric mucosal DNA synthesis rate was also estimated. Besides group A, all other groups showed variable degrees of gastritis mainly located at the anastomosis or at the gastrostomy suture line. Severe gastritis was not present in group B rats and most commonly occurred in group F rats. Group C, D, E and F showed an intense epithelial proliferation with a labeling index significantly higher than in the controls. However, mucosal DNA synthesis rate appeared to decrease with the progression of gastritis and in group F, the group more exposed to duodenal reflux, was significantly lower than in the controls. It is postulated that progressive exposure to duodenal reflux and secondary atrophic gastric changes following gastric surgery is accompanied by a reduction in the mucosal layer of the DNA synthesis rate, due to the loss of epithelial elements and the increase in connective and inflammatory elements, together with an enhanced turnover of the residual epithelial cells.

An ultrastructural study of early morphogenetic events during the establishment of fetal hepatic erythropoiesis.

Morphogenetic events are described which characterize early stages of the interaction between mesenchyme and expanding epithelial cell cords derived from the hepatic endodermal diverticulum in the C57BL/6J mouse. This interaction culminates in the differentiation of hepatic epithelial and hematopoietic tissues. No basement membrane separates the presumptive hepatic epithelial cells from the adjacent mesenchyme, while intercellular attachments, both adherent junctions and desmosomes, are established transiently between heterologous cell types across this epithelio-mesenchymal interface. Yolk sac-derived erythroblasts found in the primitive liver are distinguished morphologically from endogenous hepatic erythroid cells; they are confined to the vascular compartment and are not, apparently, precursors for hepatic erythropoiesis. The earliest recognizable endogenous hepatic hematopoietic cells appear, extravascularly, among those mesenchymal cells in intimate contact with the endodermal epithelium between the 10(1/4) and 10(1/2) gestational day. Definitive erythropoiesis commences between the 10(1/2) and 11th fetal days. The ultrastructure of these primitive hepatic erythroid cells (proerythroblasts) and their transition to more mature forms (basophilic and polychromatophilic erythroblasts) are described.
