[Echographic studies of the thickness of the endometrium during spontaneous and induced cycles: its application to in-vitro fertilization].

The authors have estimated the thickness of the endometrium by measuring it ultrasonically between the 5th preovulatory and the 6th post-ovulatory day. This study, which was carried out as part of an in vitro fertilisation programme (IVF) was performed during 162 cycles of which 22 were spontaneous and 140 were stimulated by Clomiphene and HMG. The endometrium is significantly thicker and grows more rapidly when the cycles are stimulated. There is a correlation between the levels of progesterone in the blood and the thickness of the endometrium on the 2nd post-laparoscopy day. On the other hand there was no relationship between the thickness of the endometrium and with the development or otherwise of a pregnancy after replacing embryos.

Longitudinal division of small intestine: a surgical possibility for children with the very short bowel syndrome.

Surgical approach to short bowel syndrome has been dealing with two major problems: lack in absorptive surface and dysfunction of the peristalsis of the widely distended loop above the anastomosis. In those children having a very short intestine, one is reluctant to either resect or reduce the diameter of this loop. Bianchi, followed by Boeckman and Traylor, described a procedure of loop lengthening by dividing it longitudinally. Their procedure has the advantage of restoring normal peristalsis without losing any absorptive surface. A modification of the original procedure of Bianchi is described. We report on its application in a child born with laparoschisis and intestinal atresia; she had in fact 25 centimetres of duodenum and proximal jejunum anastomosed with left colonic angle. This child was referred to us with functional occlusion related to distension above an intact and unobstructed anastomosis. She was operated on at six weeks of age. Postoperatively oral feeding could be started after one month. Broviak's catheter for parenteral nutrition was removed at six months. In conclusion we believe that this technique offers a chance of better and faster adaptation to children born with short bowel syndrome.

[Characteristics of pregnancies obtained by in vitro fertilization].

193 patients had 212 cycles induced for in vitro fertilisation between May 1983 and May 1984. 20 pregnancies of which 10 are still in progress were obtained with 6 spontaneous abortions. There were 2 so-called biochemical pregnancies and 3 extra-uterine pregnancies. The figure of 17% pregnancies where embryos were replaced totals less than 10% if one considers those pregnancies that really carry on. The number of extra-uterine pregnancies is disturbing. Finally, 2 multiple pregnancies continued, one of them being a quadruple pregnancy. The authors have made a special study to see the parameters of the conditions under which the pregnancies were obtained and the prognosis for such pregnancies.

[Extrauterine pregnancy following in vitro fertilization, 2 of these associated with a developing intrauterine pregnancy].

Five cases of extra-uterine pregnancy occurred after in vitro fertilisation and embryo transfer. The case histories are given. All pregnancies occurred in the ampullary portions of the tubes. In two of the cases there was an on-going intra-uterine pregnancy as well as the ectopic. After the extra-uterine pregnancy had been operated on the intra-uterine pregnancies continued normally to term. In considering the tubal physiology the authors attempt to draw conclusions about the factors causing ectopic pregnancies after in vitro fertilisation.

[Posterior urethral valves in the very young infant: rupture by balloon catheter].

A simple treatment of valves obstructing the posterior urethra in male neonates consists of breaking them by pulling a balloon catheter. This technique is preferable to instrumental manoeuvres, always delicate in such small urethras. In the case reported here a 28-months follow-up showed satisfactory results with regard to both renal function and urinary continence.

[In vitro fertilization].

The results of 36 follicular punctures for in vitro fertilization (IVF) performed at the Maternity unit of Hôpital Tenon, Paris, between January and June 1981 are reported. To collect a pre-ovulatory ovocyte by follicular puncture through laparoscopy requires extremely accurate monitoring of ovulation to make sure that the ovocyte collected has completed its nuclear and cytoplasmic maturation in vivo. The criteria for puncture time are the beginning of LH plasma peak level in spontaneous cycles and injection of HCG in stimulated cycles. In this series the ovocyte collection rate was 60% on average. Failures were mostly due to the local status of the genital tract in the women selected for attempted IVF, since all of them had permanent tubal sterility consecutive to severe pelvic inflammatory lesions. The percentage of ovocytes found to be mature was higher in spontaneous than in stimulated cycles. Out of 13 pre-ovulatory ovocytes collected, 8 were fertilized in vitro. Four could be transferred into the uterus, resulting in two pregnancies which had to be interrupted at an early stage. IVF demands full availability of the surgical and laboratory teams concerned.

[Repair of post-traumatic recto-vesical fistulae. A new technique (author's transl)].

Surgery for post-traumatic recto-vesicle fistulae involves two major risks : recurrence and residual sphincter disturbances. It would appear that a solution may be offered in the form of transanal pre-rectal colonic lowering. This technique, which is easy to perform, and which involves no suture, has been used successfully in an 11 and a half year old child.

[Can we define the indications for oocyte donation?].

The outline of oocyte donation's indications is defined in France by the law of the 29 july 1994. A great number of situations do not pose any problem with regard to this principles: gonadal dysgenesis, premature ovarian failure, ovariectomy, castration following chimiotherapy or radiotherapy. However, others indications drive us progressively outside legal limits. What is the normal age of physiological menopause? Which threshold to justify oocyte donation for low responders? Is oocyte donation an alternative for other treatments failures? Ovocyte donation for genetic reason presents a risk of eugenic drift. Answering these questions is uneasy, though it is urgent for our patients to offer them a better uniformity.

[Oocyte donation: psychological and sociological aspects, the clinician's viewpoint].

Among the treatment for medical assisted procreation, oocyte donation presents the more complex implications on psycho-sociological aspects. French law of the 29th July 1994, tries to prevent risk for the principal actors of oocyte donation, but has created new problems: 1) the shortage of donors lengthens the waiting list and reduces the succes's chance, 2) the absence of uniformitybetween different countries opens the door to an international market of the oocyte. Medical progress give new possibilities for situations that presented no problem in the past, for instance, pregnancies after menopause. Natural markers are questionned, it is too soon to conclude and to mesure all the consequences of our treatment on the well-being of the child, the recipient's couple, and the donor.
