[Hyperviscosity syndrome and eye involvement].

10 patients with serum hyperviscosity syndrome of different etiologies were examined by fluorescein angiography of the ocular fundus. There was no exact correlation between the pathological findings in the fundus of cases with plasma cell dyscrasia and blood viscosity. Only papilledema and retinal venous engorgement were improved by lowering serum viscosity by plasmapheresis. The abnormal circulating immunoglobulins are an important factor in the genesis of the pathological changes in the retina of patients with plasma cell dyscrasia.

Tissue plasminogen activator levels in different types of polycythemia.

The plasma level of tissue plasminogen activator antigen (t-PA-Ag) was examined in 86 patients with polycythemia (29 polycythemia vera, 11 secondary polycythemia and 46 with spurious polycythemia) and 24 healthy volunteers. Tissue plasminogen activator antigen was significantly decreased in patients with polycythemia vera in comparison with healthy controls. On the other hand, in patients with spurious polycythemia and secondary polycythemia t-PA-Ag concentration was significantly increased. There was no significant difference in t-PA-Ag levels in polycythemic patients with or without thromboembolic disease. A significant correlation was detected between t-PA-Ag level and hemoglobin or hematocrit concentration in patients with polycythemia vera (p = 0.02, r = 0.43). However, in patients with secondary polycythemia and spurious polycythemia, no significant correlation between t-PA-Ag and hemoglobin level was found. Plasminogen activator inhibitor (PAI) levels in patients with polycythemia vera and healthy volunteers did not differ significantly.

Inhibition of platelet aggregation by contrast media.

The effect of 30% Urografin on platelet aggregation induced by adenosine diphosphate (ADP), epinephrine, and arachidonic acid was examined. In vivo and in vitro experiments in 10 nondiabetic subjects and in vitro experiments in 7 diabetic-uremic patients showed a statistically significant decrease in platelet aggregation 20 minutes after in vitro incubation or in vivo infusion with Urografin. Urografin concentration was about 2.4-2.5%. The possibility of bleeding induced by contrast media and the connection with acute renal failure in diabetic-uremic patients are discussed.

Effect of vitamin E on platelet aggregation in diabetic retinopathy.

The effect of vitamin E on platelet aggregation was investigated in a group of 10 patients with diabetic retinopathy. Adenosine diphosphate induced platelet aggregation was inhibited in the patients' group as well as in the controls in the presence of vitamin E. An increased platelet aggregation was obtained with arachidonic acid in both groups when platelet-rich plasma was incubated prior with vitamin E. The clot retraction inhibition test parallels the findings of aggregation obtained with the platelet aggregation meter.

Long-term electrode implantation for recording cochlear electrical activity in guinea pigs.

Comparative measurements of the cochlear electrical activity over prolonged periods require long-term implantation of electrodes. During this time, changes in the electrode recording ability and/or impairment of the sound-conducting system, could erroneously be attributed to the experiment. To avoid this risk, an easy and reliable technique is described. The procedure uses the Fallopian canal as a route by which to approach the cochlear potentials' generators, while leaving the middle ear untouched.

Decreased calcium content in patients with digoxin intoxication.

The platelet calcium content, the plasma calcium level and the serum digoxin were examined in 90 individuals divided into the following four groups : I -- patients with digoxin intoxication; II -- treated with digoxin; III -- treated with digoxin and furosemide, and IV -- control subjects. The patients with digoxin intoxication showed a lower platelet calcium content as compared to subjects of the other groups (p less than 0.001), whereas the serum digoxin level was significantly higher than in the patients of groups II and III (p less than 0.001). Since the examination of the platelet calcium level is a relatively simple and rapid procedure, with reliable results, its use as a routine method in the determination of digoxin intoxication is suggested.

The effect of pH on platelet and coagulation factor activities.

After observing that lavage of the stomach with alkaline solutions has a beneficial effect on the control of gastric hemorrhage, we examined the platelet and coagulation factor activities under conditions of lower and elevated pH. The results showed that change of the pH from acidosis to 7.0 and even to slight alkalosis induces platelet aggregation, platelet calcium and serotonin release, as well as platelet factor II availability. The prothrombin and partial thromboplastin times approached normal levels, whereas the fibrinogen level did not change significantly. The results obtained may serve as an explanation for the control of the intragastric bleeding in patients treated by maintenance of their gastric pH at 7.0.

The danger of "yellow dyes" (tartrazine) to allergic subjects.

Oral administration of 50 mg tartrazine to 122 patients with a variety of allergic disorders caused the following reactions: general weakness, heatwaves, palpitations, blurred vision, rhinorrhoea, feeling of suffocation, pruritus and urticaria. There was activation of the fibrinolytic pathway as shown by reduction of plasminogen with high pre-kallikrein and low kallikrein values. Reduction in complement activity (CH50) was seen in three out of sixteen reactions.

Platelet aggregation in diabetic retinopathy.

Platelet aggregation in diabetic retinopathy was investigated in a group of 25 patients. An enhanced activity induced by epinephrine and arachidonic acid was found in this group as compared with the controls, whereas in adenosine diphosphate (ADP) platelet aggregation no differences were observed. Spontaneous aggregation was found in 8% of the diabetic retinopathy cases.

Bilateral retinal vasculitis associated with clotting disorders.

We found distinct alterations of the clotting and fibrinolytic mechanisms in a 27-year-old man who suffered from typical bilateral vasculitis, clinically and fluoroangiographically manifest as obstruction of the central retinal veins. The partial thromboplastin time was slightly increased, a partial clot lysis occurred after one hour, the plasminogen level was markedly low, and alpha2-macroglobulin was decreased, as were factor XII and IgG. Kallikrein was absent. Similar changes, though not so pronounced, were also found in the patient's son and in his brother.

Haemostatic and complement changes in a family with 'allergic' disorders.

A family with allergic manifestations, haemostatic disturbances, total absence of haemolytic activity of the complement system and low IgG levels is described. It is suggested that this functional abnormality of the complement system and the decreased level of immunoglobulin G may be due to immune complex reactions.

The fibrinolytic pathway in the human fetus.

The fibrinolytic pathway and the protease inhibitors in 12 to 24-and 24-to 36-week-old fetuses were examined and compared with full-term newborns. The activity of the fibrinolytic system, as well as the values of the protease inhibitors approach the normal values progressively with the embryonic maturation.

Hereditary complement defect and kinins involvement in asthma.

Relationship between an inherited qualitative defect of the complement system and intrinsic asthma, and the direct participation of the kinin system in producing asthma, are reported. Qualitative complement impairment that produced hereditary angioedema in the son of an asthmatic father indicated that the kinin system was the vasoactive mediator in both cases.

Reconstruction of upper cervical esophagus.

We describe a surgical technique for reconstruction of the upper cervical esophagus after a segment has been excised. The technique involves the downward rotation of a full thickness pharyngeal flap to close the defect. The flap is obtained from the posterior wall of the hypopharynx and oropharynx. We describe a case in which this technique was used.
