Socio-economic rehabilitation programmes of LEPRA India--methodology, results and application of needs-based socio-economic evaluation.

There is now a better understanding of the scope and process of rehabilitation. The approach recognizes the impact of leprosy on the individual, aims to understand the needs and concerns of those affected, their families and community in the rehabilitation process, and that aims to restore the person to normal social life. LEPRA India has undertaken socio-economic rehabilitation (SER) activities in its projects in Andrah Pradesh and Orissa States in India with a holistic approach that has been evolutionary, developmental and participatory. A SER Officer (SERO) was posted to each project. A plan was formulated by the SERO with participation of all project staff. The main emphasis of the programme was on active participation of the affected person in the rehabilitation process. A needs-assessment study was conducted in the target population using a semi-structured questionnaire. Information was elicited about social and economic status, before and after the disease, and the current rehabilitation needs of the persons affected. The next step was meeting the needs through interventions by the SER staff. The impact of the programme on restoration of social and economic status of the affected persons was analysed. The paper stresses the importance of assessing the needs of persons affected by leprosy, structuring a rehabilitation programme with the active participation of the affected person and evaluating the impact of the interventions in restoring normal social and economic life.

Early detection of damage to nerves in leprosy.

Methods of examining and diagnosing damage to nerves commonly involved in leprosy are described. The equipment used is inexpensive, gives reliable and repeatable results and is useful in making objective assessments in terms of function in everyday living.

Denatured autologous muscle graft in leprosy.

Autologous muscle grafts were used to repair 12 mixed peripheral nerves (9 posterior tibial, 3 median) in 10 patients with leprosy who had total anaesthesia and analgesia of the area supplied by the nerve. Postoperatively, 7 patients reported improved sensation in the foot or hand, with a return of vibration sense and joint position sense in 11 and of perception of a 10 g pin in 5; the ability to sweat in the affected area was also restored in 7.

Mycobacteria in nerve trunks of long-term treated leprosy patients.

Mycobacteria were present in 4 out of 8 mixed peripheral nerve trunks from patients (3 BT and 1 BL) treated with DDS and/or MDT for periods ranging from 21 months to 8 years. Most of the bacilli appeared to be 'whole'. Nerve destruction with areas of granulomatous infiltration appeared more active than expected. Possible reasons for a continued presence of bacilli in treated nerves and its implications in 'relapse' are discussed.

Surgical reconstruction in intrinsic palsy of the fingers. A study of hundred cases.

The authors analyze the results obtained by surgical treatment in 100 cases of intrinsic palsy of the hand. The distribution of the surgical procedures performed under regional anesthesia was as follows: Brand II: 41, Palande's lasso: 14, Zancolli: 15, Palande: 13, Littler: 13, Brand I: 2. Results were satisfactory in 74 p. cent, unchanged in 20 p. cent and poor in 6 p. cent of cases. The analysis of this series showed a significant difference in the results obtained in the little finger with better results when Palande's and the lasso procedures were used (for the hand in the lumbrical and open positions). Indications have thus been defined according to the degree of nerve damage and environmental factors. Physical therapy is essential for good integration of tendon transfers.

Correction of intrinsic-minus hands associated with reversal of the transverse metacarpal arch.

In a hand with paralysis of the intrinsic muscles, the clawing of the fingers is associated with deformity of the distal transverse metacarpal arch. The arch is often flattened in the open-hand position and is sometimes reversed. Sixteen hands were operated on with a new tendon-transfer procedure aimed at simultaneous correction of the deformities of the arch and of the fingers. Adequate restoration of the arch was attained in all sixteen hands while the deformity and disability were corrected in 85 per cent of the fingers.

The deformity of thumb in ulnar paralysis.

The flexion/extension angles at the Metacarpophalangeal and Interphalangeal joints of the thumb in position of rest and pinch were studied in 68 thumbs of ulnar paralysis. In two thumbs the flexor pollicis brevis was not paralysed. Twenty five normal thumbs were used as controls for comparison. During the resting position the metacarpophalangeal joints showed less flexion and interphalangeal joints more flexion than normal. During the pinch position metacarpophalangeal hyperextension and interphalangeal flexion -- the Z deformity was seen in about 60% while marked interphalangeal flexion was seen in all thumbs of ulnar paralysis. It is suggested that this muscular imbalance is likely to affect thumb joints in time because of abnormal stresses due to altered forces and this can be prevented by restoring the muscle balance by surgery.

Surgery of ulnar nerve in leprosy.

Surgery on 64 ulnar nerves in leprosy was done for the treatment of ulnar nerve paralysis of less than one year duration. The initial treatment, indications for surgery, clinical findings, the operation procedures and findings are described in detail. Factors shown to have statistically significant bearing on nerve function before and after surgery are discussed. The results were: improved 32, further nerve damage stopped in 11; worsening in six while in the remaining 15 nerves there was no nerve function recovery. The main conclusions are that earlier the duration of paralysis and lesser the initial nerve function deficit better is the result. Lepromatous cases showed better result than non-lepromatous cases.

The role of muscle reeducation in dynamic tendon transfer surgery of the hand.

Tendon transfers are used commonly to improve function following damage to major nerve trunks. For the transfer to function dynamically and in harmony with other active hand muscles, proper integration of the action of the transfer into the available hand movements is essential. This concept is discussed together with the rationale and methods of pre and postoperative training that contribute immensely to the success of such operative procedures.

Plantar ulcers with osteomyelitis underneath. A bacteriological study.

39 consecutive cases of plantar ulcers with underlying chronic osteomyelitis admitted in the Sacred Heart Hospital during 1975/1976 were studied for the infecting organisms and their sensitivity to easily available antibiotics. Single organism was iasolated in only 10 cases, the infection in the rest being a mixed one. The commonest organisms were Staphylococcus, Streptococcus and Proteus mirabilis. In a few cases Pseudomonas and E-Coli were also isolated. Chloramphenicol was the most effective antibiotic in general and Streptomycin the least. 70% of the staphylococcus strains isolated were found to be resistant to Penicillin. Empirical use of antibiotics especially Penicillin and Streptomycin is hence deprecated.

Correction of paralytic claw finger in leprosy by capsulorrhaphy and pulley advancement.

Paralytic claw-finger deformity in leprosy was corrected by metacarpophalangeal capsulorrhaphy and fibrous flexor pulley advancement in ninety fingers in thirty-seven hands. Sixty-eight of these were analyzed after one to four years' follow-up. The overall results were: good in forty-two, fair in seventeen, and poor in nine. This procedure was done in selected patients in whom for various reasons good results of tendon transfer were not assured.
