[Aortic dissecting aneurysms: clinical masks, differential diagnosis].

101 necropsies of aortic dissection (AD) made in 1991-2000 have been analysed. Among the deceased men prevailed (76.3%), 23(22.8%) of them were under 50 years of age. An accurate intravital diagnosis was made only in 40.6 patients. The most common clinical masks were the following: coronary, valvular, vascular, gastrointestinal, pulmonary, thromboembolic, mediastinal, cerebral, renal and anemic. Most often AD masks as acute myocardial infarction. The knowledge of encountered clinical masks of AD promoted improvement of early AD diagnosis to correct AD surgically and reduce lethality.

Pathogenesis and morphogenesis of microcirculatory disorders in myocardial infarction. Clinical and ultrastructural examination.

Major advances in the understanding of myocardial infarction (MI) have been made on animal models of ischemia and infarction. We performed ultrastructural examinations of the hearts of 36 patients who died during the acute phase of MI. Tissue for investigation was obtained by express necropsy immediately after death at the clinic. All components of the microcirculatory system of the heart were damaged during the acute phase of MI. Coronary thrombosis led to arteriolar spasm and intravascular blood coagulation in the vicinity of the damaged artery. Microvessels reduction in ischemic, necrotic, and near-infarct zones was caused by endothelial cell injury and rheological disturbances. Thrombocytes played a decisive role in thrombus formation and arteriolar constriction. Leukocytes nearly always impaired the microhemodynamics in ischemic zones due to their size and rigidity. Microcirculatory disorders seemed to precede myocardial cell injury and death.

[Spectrographic study of the hair].

Estimation of microelemental composition of hairs and method of its determination are presented in the article. Two dozens of microelements were established to be present in hairs. Some samples contained vanadium, tin, and bismuth. This may be connected with unfavourable ecological influence. About a quarter of samples revealed presence of nickel, chrome and manganese., Barium and strontium were found in a half of the cases. Lead did not manifested in the most part of the samples. It may be ousted by other elements or accumulated in other organs. Calcium, aluminum, copper, iron, titan, lantan and zinc were common. Thus, study of microelemental composition of hairs may provide information on the state of metabolism and influence of environment on the body.

[The pathomorphological variants of changes in the aortic and arterial intimae: their terminology and nature].

Results of the complex study of extra- and intracranial arteries, arteries of lower limbs and aorta at various levels are presented. Autopsy material was obtained from young (aged below 44) patients who died of ischemic heart disease, cerebrovascular pathology, circulatory disorders of lower extremities. Attempt was made to summarize data about various lesions of inner arterial membrane: circular and focal musculo-elastic hyperplasia of hyalinized plaques, plaques poor on lipids, protein deposits on intima. Unification, different arterial lesions by the term "atherosclerotic plaque" prevents from detection of subtle morphogenetic mechanisms, metabolic and immunomorphological peculiarities of arterial wall.

[The duration of courses of manual therapy in vertebrogenic neurological and somatic pathology].

The authors have developed and validated algorithms, programs, regimens and techniques of manual and instrumental treatment which produced positive effects in 99% of patients with neurological syndromes of spinal affections. The criteria of the treatment discontinuation were alleviation of painful syndrome by 25-50%. This occurred, as a rule, after 3-4 sessions. The course averaged 10 sessions and more. The treatment was discontinued after stabilization of subjective improvement.

[Myxoma of the heart: aspects of its histogenesis].

15 cases of heart myxomas are presented. Their origin from remains of embryonal mesenchymal tissue is hypothesized. Predominant localization at the foramen ovale area, where remains of embryonal tissue can be found, and peculiarities of histological structure resembling embryonal mesenchyma may be proofs of the hypothesis. Microscopic structure of myxomas is homogeneous: branching cells of histiocytic origin amidst basophilic or eosinophilic stroma presented by myxoid tissue, capillaries and vessels of various gauges at different stages of functional activity (shut-down, compressed, dilated, filled with erythrocytes).

[Nonspecific aortoarteritis with involvement of the pulmonary arteries].

Considering the common aspects of tissue structure of the aorta and pulmonary artery the authors suggest that in unspecific aortoarteritis in the form of generalized arteriopathy, involvement of pulmonary artery is real. Analysed were 45 autopsy cases. In 31 cases vessel changes of two kinds were seen: destructive-proliferative and sclerotic. Not all cases of sclerotic changes may be explained by lesser circulation hypertension and, thus, the reported data evidence frequent involvement of the pulmonary arteries with development of generalized pathology of the vascular system.

[The autopsy: its importance for modern clinical medicine].

Autopsies are absolutely necessary for the progress of modern clinical medicine and for solving many problems: diagnostic, control of treatment quality, detection of infectious, hereditary and systemic diseases, establishing causes and mechanisms of sudden death, qualification of unclear diseases and syndromes, classification and nomenclature of nosological entities, detection and study of spontaneous and induced pathomorphosis. The pathologist performing autopsies should except necropsy also make macrochemical examination and send the material for microbiological and biochemical examination. Special attention should be paid to establishing the diagnosis and clinicoanatomical epicrisis. Undoubtful is the role of autopsy in teaching and training, postgraduate education, accumulation of archives and their retrospective analysis, creation of macro- and micromuseums of human diseases. Physician's errors, general requirements and cadaveric tissue observation are also discussed.

[Accumulation of anomalous glycopolymers in the walls of main lymphatic vessels of the extremities in patients with chronic lymphedema].

Redistribution of lectin receptor sites in the mam-lymphatics of human extremities in primary and secondary lymphedema in comparison with normal lymphatics has been investigated using conjugates to peroxidase of LCA, PNA, RCA, SBA, WGA and LAA. Chronic lymphedema caused no essential differences in lectin binding to tissue structures of intima and adventitia: endotheliocytes were strong reactive both with LCA, PNA, WGA and LAA; all lectins used bound fibrillary structures in subendothelial layer and adventitia. On the opposite, tunica media of main lymphatics in primary lymph stasis displayed accumulation of LCA-, PNA-, SBA- and LAA-binding deposits round smooth myocytes. When PNA used, the appearance of a specific "wide loop shaped reticulum" within lymphatics media in lymph stasis was observed. We consider, that lectin receptor sites accumulation phenomenon reflects some changes in smooth myocytes metabolism, possibly caused by dystrophia of these cell population with consequent incomplete glycosylation of synthesized glycopolymers.

[The results of a study of chemical-induced disease in children in Chernovtsy].

A clinical study of children inhabiting Chernovtsy and Chernovtsy Province, examination of the qualitative composition of inorganic admixtures in the hair and their microscopy indicates that the revealed symptom complex allowed to diagnose mild forms of chemical disease in children with focal alopecia. The criteria were: mucosal lesions (conjunctivitis, pharyngitis, glossitis, cheilitis); alopecia; hematological syndrome: eosinophilia, lymphocytosis, relative neutropenia; neuro-psychic disorders; cutaneotrophic disorders; presence of black inclusions in longitudinal and transverse light microscopy. Association of focal alopecia with the above symptoms even in absence of changes of the qualitative composition of inorganic microadmixtures allows to think about the chemical etiology of the diseases apparently caused by compounds of different chemical substances with inclusion of thallium.

[The antiphospholipid antibody syndrome: its pathogenesis and clinico-morphological manifestations].

Lupus anticoagulant was determined in lupus erythematosus as an antibody inhibiting prothrombin activation in phospholipid coagulation tests without specific suppression of any coagulation factors. The available information concerning properties and mechanisms of action of lupus anticoagulant are rather controversial as there exist a group of similar coagulation inhibitors that represent antiphospholipid antibodies. In spite of a considerable increase of a coagulation test time prothrombin time, partial thromboplastin time, the presence of the lupus anticoagulant is rarely followed by haemorrhagic diathesis. On the contrary, recurring thromboembolic complications, pathology of gestation are observed in patients with this or other antibodies against phospholipids. The appearance of the antiphospholipid antibodies is described in infections, haematological processes, systemic vasculitis, tumours, may be provoked by a long use of some medicinal preparations. Circulation of the lupus coagulant and other antibodies against phospholipids is of a chronic type while in infections diseases it is of an acute transitory character.

[The skin manifestations of congenital and acquired hemostatic disorders].

The authors review the literature data on skin symptoms of the most prevalent disorders induced by congenital or acquired defects of blood coagulation. The range of skin changes in hemostasis disorders includes hemorrhages of 5 various types, thromboses, pigmentation disorders, and pustular involvement. Early detection of skin symptoms of hemostasis disorders will improve the diagnosis and timely correction of such conditions.

[The chronic action of lead on the vascular system: the problem of ecological pathology].

Problem of chronic effects of the lead (L) on the cardiovascular system is presented from the viewpoint of the ecological pathology. The review summarizes: sources of L in the environment, industries in which L is widely used in the technological process, occupations in which L intoxication is possible, routes of L penetration into the organism and the organs of its accumulation, mechanisms and variants of chronic L effects on the vascular system. In case of chronic L toxicity the vessels of various organs and system are affected; the pathologic process is of a vasculitis or arteriosclerosis type and is predominantly observed in the arteries of small and mean caliber. The mechanisms of vascular L effect is not sufficiently studied.
