[The effect of occupational noxae in the tobacco industry on pulmonary function in exposed workers].

A study was carried out to determine respiratory hazards to workers in tobacco manufacture. The investigations at the workplace included analysis of the work technology and measurements of chemical pollutants and dust concentrations in the work rooms. Ventilatory lung function tests were performed in 95 workers exposed to tobacco dust and in 129 control workers. Results of workplace analysis showed the presence of tobacco dust levels exceeding the maximum permissible concentrations. The values of some lung function tests for exposed workers non-smokers were significantly lower than for control workers non-smokers. They could be attributed to the effect of occupational noxae on the respiratory system.

[Retroperitoneal hematoma].

Diagnosis and management of retroperitoneal haematoma is the problem of controversy in actual moment. It appears most frequently in the range of polytrauma or various traumas of abdomen and retroperitoneal organs. Here we report our experience in management of retroperitoneal haematoma. During the ten year period (from 1979 to 1986) we treated surgically, at the department for surgery, 58 injured patients with retroperitoneal haematoma. In 5 cases explorative laparotomy was done, and in other 53 cases there were injuries of intraabdominal and retroperitoneal organs. The haematomas were caused by the ruptures of spleen, liver, kidneys, pancreas, duodenum, small bowel with mesenterium, large bowel, bladder, retroperitoneal large blood vessels and pelvic fractures. In 17 cases retroperitoneal haematoma was associated with the injury of one organ. In 36 cases there were injuries of two or more organs. Retroperitoneal haematoma was caused by blunt trauma in most cases. During the management there were some diagnostic difficulties. In diagnosis we use: clinical status, of patients, radiography, angiography, ultrasonography, but the most secure was laparotomy. There are two treatment approaches, operative and conservative. Retroperitoneal haematoma was a consequence of ruptured solid organs and retroperitoneal blood vessels, and associated with injuries of intraperitoneal organs. All this, mentioned above, is the reasons for detailed exploration of abdominal cavity.

[Pefloxacin in the treatment of septicemia, purulent meningitis and salmonellosis].

Thirty two patients (16 female and 16 male), ranging in age from 13 to 80 years, were treated with pefloxacin (Abactal) at The Clinic of Infectious Diseases and Febrile Conditions. Pefloxacin was applied parenterally in 21 patients with serious infections. 15 of the cases had the signs of verified septicemia, i. e. bacteriemia; 5 patients developed purulent meningitis, i. e. meningoencephalitis; and in one patient bronchopneumonia occurred. Isolated causative agents (Staphylococcus aureus, S. epidermis and Gram-negative aerobic bacteria), were sensitive to pefloxacin. In 11 patients with enterocolitis Abactal was also included in the treatment because of the increasing occurrence of the resistance of Salmonella spp. to various chemotherapeutics and excellent "in vitro" effects of pefloxacin to those multiple-resistant species. Salmonella virchow was isolated from the stools of 6 patients. Salmonella enteritidis from 3 patients, Shigella sonnei from one, and in one case the causative agent was not identified. Pefloxacin was applied per os or parenterally in the dosage of 800-1200 mg in the period from 5-9 days. In all the patients (100%) eradication of the causative agents found in stools was done as early as the third or fourth day of the therapy. There were no adverse effects.
