[Experimental research of biological inertness of nickel-titanium alloy with memory of the form].

Research was conducted on 26 adult outbred male and female mice, which were implanted with nickel-titanium wire in the paravertebral zone. The tissues surrounding the implants were studied macroscopically and histologically 2-8 months after the implantation. It was found that the morphological changes of tissues after nickel-titanium wire implantation were detected by microscopical study at the time intervals greater than 2 months after the operation. They included the formation of a thin connective tissue capsule in close vicinity to the implant. No signs of reactive changes in the distant tissues and organs were detected. It is concluded, that the nickel-titanium alloy possesses high degree of biological inertness and may be used for making fixating devices with the memory of mechanical shape that are employed in cranioreconstructive surgery.

Saccular aneurysms of meningeal artery: case report.

OBJECTIVE AND IMPORTANCE: A rare observation of double saccular aneurysms of the meningeal artery is presented. CLINICAL PRESENTATION: This 22-year-old man was referred to the Neurosurgical Institute with a suspicion of an aneurysm of the anterior communicating artery. Bilateral angiography of the carotid arteries was performed 1 week after the subarachnoid hemorrhage, but the aneurysms were not visualized. Routine angiography of both carotid arteries and selective studies of the left vertebral artery were performed again, and angiography of the right carotid artery revealed an aneurysm. The patient's neurological state at the time of admission was normal. Fundoscopic examination revealed papilledema and conjunctival injection of the left eye. INTERVENTION: The patient was treated using a right pterional approach. One aneurysm had caused spontaneous subarachnoid hemorrhage. The aneurysms were removed using a direct approach, with histological examination of dura matter fragment containing both aneurysms. The results of the patient's 2-week follow-up examination were normal. Follow-up angiography of the right carotid artery showed absence of the aneurysm with a clip on the branch of meningeal artery. CONCLUSION: Saccular aneurysms of the meningeal artery can be manifested by subarachnoid hemorrhage, and intradural arterial aneurysms are similar to saccular cerebral vessel lesions structurally.

Arteriovenous malformation of the cervical spinal cord with unusual drainage into the cranial cavity.

A case of arteriovenous malformation of the cervical cord 20 years in duration, studied angiographically, using computerized tomography scans and magnetic resonance images, as well as findings at autopsy are presented. The malformation was fed by the right vertebral artery, which entered a giant saccular aneurysm-like structure located subdurally on posterior spinal cord surface, and terminated there. Plenty of vessels of various diameter were found in the spinal cord substance at this level (C4-C6). Blood flow drained via a grossly enlarged vein at the anterior spinal cord fissure into cranial cavity, where varicose posterior fossa veins could be clearly seen and a single vertebral artery was present.

[Pathology of congenital defects of the central nervous system vessels].

The author's classification of vascular malformations in the central nervous system is presented approved by neurosurgeons, pathologists and in forensic medicine. The description of the cavernous angiomas pathology is given showing possibility of their radical surgery. The notion of micro-malformation is formulated. The pathogenetic concept of Sturge-Weber disease with recommendations for the clinics is given. The author's model of the malformation morphogenesis in the vessels of the central nervous system is presented and the ways for the decrease in the incidence of these malformations are indicated.

[Theory and practice in developing a system of artificial intelligence for recognizing histologic specimens].

A system is described for an automatic analysis and recognition of a fresh and reference histologic material. The system includes a microscope, TV camera and personal computer. The system proved applicable in identification of two tumor types: astrocytomas and oligodendrogliomas. The system can be used in pathology, histology, embryology, cytology, histochemistry, clinical laboratory microscopy and in borderline fields.

[Color television system for the microscope].

The characteristics of colour TV system made on the basis of microscope MBI-15 with the use of imported details are presented. The system can be used for clinico-anatomical conferences, scientific lectures, in teaching, and allows to avoid laborious preparing of colour slides. The design made it possible to keep information (images) in form of figures (for example, by means of a personal computer) and video-cassettes. This may be used in computer morphometry and quantitative analysis in histochemistry.

[Primitive trigeminal artery combined with arteriovenous malformation of the brain].

Analysis of the brain circulation and consequences of the intravascular surgery in a woman of 22 with a combined pathology--a primitive trifacial artery with an arteriovenous brain malformation--are presented. The arteriovenous malformation was cured surgically with the use of polystyrene emboli some of which penetrated through the primitive trifacial artery into the branches of carotis interna. The possibilities of the above combinations should be taken into consideration when the method of the intra-vascular surgery is to be chosen.

[Arteriovenous shunting of cerebral blood flow in basal meningo- cerebral developmental anomalies and paraganglioma of the glomus jugulare].

The authors describe a case of angiographically verified arteriovenous shunting of the cerebral blood flow coupled with basal ++meningo-cerebral developmental abnormality and paraganglioma of the glomus jugulare in a 52-year-old patient. Such a unique combination made it possible to suggest the existence of potential mechanisms of blood flow shunting either via tumorous vessels or vessels (arteriovenous shunts) of the basal ++meningo-cerebral developmental abnormality. In the latter case, the shunts were likely to occur latently while the clinical manifestations of the disease might be related to the tumor growth, impairment of the venous outflow and disclosure of arteriovenous shunts.

[Sturge-Weber disease].

Morphological changes in the vascular malformations of pia mater and neighbouring parts of the brain surgically removed in 9 patients with Sturge-Weber syndrome were studied light- and electron-microscopically. On the basis of the results obtained the concept of Sturge-Weber syndrome pathogenesis is formulated. According to this concept the disease develops due to the brain hypoxia resulting from the long-lasting congestion in the pia mater with angiomatosis. Respectively, the surgical treatment of the disease is founded and the necessity of such a treatment at young age is outlined. The authors' own data on the nearest (up to one year) and remote (up to 15 years) results of such treatment are presented.

[Intracranial cavernous angiomas].

Clinico-anatomical characteristics of 9 intracranial cavernous haemangiomas (CH) are given, 7 of them being found in the brain and 2 in the arachnoid membrane. The average age of patients with clinical manifestations of CH was 23.5 years. The emphasis is made on the appearance of the first CH symptoms in the females during the gestation, delivery or on the oestrogen treatment. The blood supply of CH is shown to be brought about from the small vessels on their periphery. The role of concomitant intracerebral cysts as an additional symptom is underlined. 6 clinical variants and 2 histologic types (typical and atypical) are distinguished. The suggestion about general embryogenetic roots of all intracranial angiomatous vascular malformations is put forward. CH, in spite of difficulties in their clinical diagnosis, are, as distinct from other vascular malformations, favourable object for the radical surgical treatment.

[Micromalformations of the cerebral vessels and their role in the development of stroke].

30 micromalformations (MMF) of brain vessels that were not diagnosed either clinically or at the autopsy were found histologically in 25 autopsy cases and in 3 patients after the surgical operation. MMF were, in half of the cases, the cause of the stroke which was mistakingly considered a complication of the arterial hypertension, cerebral atherosclerosis or rupture of saccular aneurysm of the brain base arteries. Clinically, MMF were either latent or manifested as recurrent disturbances of the brain circulation of the ischemic or (more frequently) haemorrhagic type. When clinically manifested MMF were revealed at the age of 15-32 years; at the age of 65-74 years they were found incidentally at the autopsy. Their predominant localization was in the brain stem, contiguous ventricular wall and in the cerebellum with its tunic. The following histological types can be distinguished: telangiectasia, venous, cavernous, arteriovenous, nonclassified, varicosis. Clinico-anatomical characterization of each type is presented. The medico-legal importance of MMF and the principal possibility of their treatment (when duly diagnosed) is pointed out.

[Intracranial oncogenic aneurysm].

The paper deals with a death from cerebrosubarachnoidal hemorrhage in a 22-year-old male. Autopsy revealed fusiform aneurysm at the junction of right posterior connective and cerebral arteries whose wall was histologically examined. The examination indicated that the wall was invaded by tumor cells from the endothelium, the internal elastic membrane and vascular muscle layer being destructed. The primary cause of the tumor was a testis removed 40 days before the death and diagnosed as having chorionepithelioma.

[Multiple primary tumors of the brain (rare case)].

The author describes clinical and morphological findings about a combination of 2 brain tumors--preventicular astrocytoma of the right hemisphere and giant cell tumor of the pons (giant cell sarcoma?, giant cell glioblastoma?) which gave a metastasis into the area of the interventricular foramen. The author notes the rarity of such a combination and the difficulty of clinical interpretation of the case which became clear only after autopsy. The difficulties of the histological verification of pons tumor and its possible histogenesis are discussed.
