Abnormal smooth pursuit eye movements in schizophrenic patients are associated with cerebral glucose metabolism in oculomotor regions.

The purpose of this study was to test the hypothesis that abnormal smooth pursuit eye movements in schizophrenic patients would be related to cerebral glucose utilization in specific oculomotor regions. Eye movements were assessed with infrared oculography in 11 unmedicated schizophrenic patients and 13 normal comparison subjects. For the patients only, regional cerebral metabolic rate of glucose utilization was measured with positron emission tomography. Abnormal pursuit tracking in the patients was associated with relatively decreased metabolism in the frontal eye fields and increased metabolism in the caudate nuclei. The results are consistent with the hypothesis that these cerebral regions are involved in the pathophysiology of abnormal pursuit as related parts of a cortical-subcortical oculomotor circuit.

Technical improvements in the repair of acute postinfarction ventricular septal rupture.

Postinfarction ventricular septal rupture (VSR) is a high-risk complication following myocardial infarction (MI). Surgical treatment has evolved to improve an otherwise poor prognosis. Certain subsets of patients remain a formidable challenge. The presence of cardiogenic shock has consistently been found to have the highest risk. Over a 10-year period, our technique of repair has evolved from established procedures to one we believe confers superior results. Endocardial patching to viable myocardium reinforced with an epicardial patch not only corrects the shunt but maintains ventricular geometry and avoids tension on friable muscle. We report on a series of nine consecutive patients in cardiogenic shock. The operative mortality was 22%, none due to low cardiac output syndrome, shunt recurrence, or bleeding. All patients have been followed with transesophageal echocardiography at a mean period of 14 months (range 3-31 months). One patient is in New York Heart Association (NYHA) Class I, four are in NYHA Class II, and two in NYHA Class III.

The structure of schizotypy: a pilot multitrait twin study.

This report of a pilot study examines 29 pairs of twins from a population-based registry on whom four domains of schizotypy have been measured: personal interview using the Structured Interview for Schizotypy, self-report questionnaire formed from eight published self-report scales, attentional battery of eight individual tests, and root mean square error on smooth pursuit eye tracking. Analyzing the twins as individuals revealed two independent dimensions of clinically rated schizotypy (positive symptom schizotypy and negative symptom schizotypy) and two independent dimensions of self-rated schizotypy (positive trait schizotypy and trait anhedonia). Positive symptom schizotypy was highly correlated with positive trait schizotypy, but not with attentional dysfunction or eye-tracking error. By contrast, negative symptom schizotypy was significantly related to trait anhedonia, attentional dysfunction, and eye-tracking error. Correlations in monozygotic and dizygotic twins suggested that genetic factors were important in all four domains of schizotypy. Except for eye-tracking error, the results are more consistent with a dimensional than a "disease" model of schizotypy. Replication of these results with a larger group of subjects is needed.

Erratic eye tracking in schizophrenic patients as revealed by high-resolution techniques.

Using high-resolution infrared oculography with digital recording and analysis techniques, we tested several types of eye movements in 19 schizophrenic patients and 11 normal controls. Abnormal slow pursuit eye movements, seen in about half of the patients, were characterized by erratic inaccuracies in position, velocity, and phase. Tracking errors were quantitatively assessed by their root mean square (RMS) error. Position RMS errors fell into two clearly separated groups, with 10 of 19 patients clustering about the normal controls and the remaining 9 having much higher errors than normal. Although several of these poor trackers had an excess of saccades or low pursuit gain, these abnormalities were not primarily responsible for the large erratic tracking errors. Saccades in response to unpredictable target jumps had normal latencies (reaction times) and velocities, but were more hypometric and variable in accuracy than those of controls. These saccadic abnormalities did not correlate with the patients' position RMS errors during slow pursuit.

Complications of ascending aortic intraaortic balloon pump cannulation.

Choice of a route of cannulation for intraaortic balloon counterpulsation during cardiopulmonary bypass is related to accessibility. In those patients in whom it is impossible to pass the intraaortic balloon pump (IABP) into the common femoral artery, ascending aortic cannulation is a rapid and direct method of insertion. Eight patients are described in whom ascending aortic IABP cannulation was undertaken to enable weaning from cardiopulmonary bypass after cardiac surgical procedures. The following problems were encountered: graft infection, aberrant cannulation of the left subclavian artery, left coronary artery embolism, and inability to close the sternum due to mechanical tamponade. A technique is described for insertion of the IABP using a polytetrafluoroethylene (Impra) graft and closed-chest decannulation. Although considerable morbidity and mortality are associated with ascending aortic cannulation, it is simple, fast, and effective, and should be considered for all patients requiring postoperative IABP support in whom peripheral vascular disease makes access difficult.

Restriction endonuclease analysis of Campylobacter strains with particular reference to Campylobacter fetus ss. fetus.

Forty-three strains of Campylobacter fetus ss. fetus isolated from sheep abortions in New Zealand, and reference strains of C. fetus ss. fetus (four), C. fetus ss. venerealis (two), C. jejuni (one) and C. coli (one) were examined by restriction endonuclease analysis with the enzymes BstE II and Xho I. DNA fragment patterns of C. fetus, C. jejuni and C. coli differed strikingly from each other, but there were many similarities in the patterns obtained for all strains of C. fetus ss. fetus (47) and C. fetus ss. venerealis (three). The 43 local strains of C. fetus ss. fetus were clearly divided into four types. Two of these types produced patterns nearly identical with those of overseas reference strains.
