In patients with differentiated thyroid carcinoma high serum TSH-levels enhance 131 J-uptake in thyroid remnant and/or metastases. An effective increase of TSH could be achieved by oral administration of thyrotropin releasing hormone (TRH) even after a short T4/T3-withdrawal period so that we recommend a TRH-stimulation in all patients before a diagnostic or therapeutic 131 J-application. Adverse reactions to TRH are infrequent and usually short timed so that in contrast to TSH-stimulation--TRH can be given to outpatients without any risk.

Pheochromocytoma scanning using 131J-meta-benzylguanidine was done in one patient with metastasizing paraganglioma, one patient with multiple endocrine neoplasia type IIb. No activity of tumour tissue could be demonstrated in the patient with metastasizing paraganglioma, whereas the pheochromocytoma could be clearly defined in the patient with multiple endocrine neoplasia type IIa. The female with multiple endocrine neoplasia type IIb showed a suspect space-occupying lesion of the left adrenal using computed tomography. Pheochromocytoma could be excluded by 131J-benzylguanidine scanning, selective estimation of catecholamines in adrenal venous blood as well as the glucagon stimulation and clonidine suppression.
Besides the known joint involvement as seen in the whole-body scan which uses 99m technetium-phosphate-complexes, skeletal scintigraphic investigations in 17 patients with psoriasis have demonstrated circumscribed or diffuse accumulative pathological concentrations of the radionuclide within the skeleton of the skull, the thyroid cartilage, the bony thorax and osseous long bones. These pathological accumulations of radionuclides of TcP-complexes which are distant from joint regions, point to proliferative phenomena in the bone in case of psoriasis especially in alterations of the collagenous metabolism.
In order to determine the radiation dose to children during bone scintigraphy, distribution and excretion measurements on 99mTc-pyrophosphate and 99mTc-diphosphonate were carried out in rats, piglets, children and adults. Because of the more intensive bone metabolism in children, uptake of 43 to 47% is higher than in adults, in whom uptake in the skeleton reaches 33 to 36%. Assuming homogenous distribution of uptake, radiation dose to the skeleton in children of the ages investigated was 51 to 55 mrd per mCi. Bearing in mind the non-homogeneous distribution in the juvenile skeleton, radiation dose at growth point reaches 85 to 95 mrd per mCi.
Since 1973 bone scintigraphy using 99mTc-phosphate-complexes was carried out in 382 patients with psoriasis. For comparison with the results of nuclear medicine, roentgenologic and clinical findings a group of 121 patients with psoriasis aged between 11 and 74 years was compared to a group of 42 patients aged between 20 and 49 years without roentgenologic and clinical signs of psoriatic arthritis. We found by means of isotope investigation that an essentially greater part of the bones adjacent to the joints was involved than was expected according to X-ray and clinical findings. In addition, in 205 patients with psoriasis whole-body scintigraphy, using 99mTc-MDP, was carried out sine 1977/78. In 17 patients we found an increased accumulation of activity in the region of extraarticular structures of the skull as well as of the skeletal thorax. According to these results we conclude that in addition to the clinically and roentgenologically defined psoriatic arthritis in patients with psoriasis and osteopathy may exist, which can only be demonstrated by skeletal scintigraphy and which is localized in bones adjacent to the joints but can also be demonstrated in the region of extraarticular bones.
Clinical and laboratory data are frequently unspecific in acute hematogenous osteomyelitis of infants. As prognosis is dependent on early treatment, a timely radiologic diagnosis is mandatory. Because of specialties in the vascular supply, the roentgenmorphologic alterations of acute hematogenous osteomyelitis in infants differ from the appearance of the disease in other age groups. The acute osteomyelitis in infants frequently leads to destruction of the epiphysis and purulent joint effusion. A subtle analysis of soft tissue lesions will already rise suspicion for acute hematogenous osteomyelitis before destructive or reactive osseous alterations prove the diagnosis by roentgenology. Nuclear medicine examinations of the bone have contributed significantly to radiologic diagnosis of acute hematogenous osteomyelitis in infants despite of few negative results. Osseous trauma and other diseases should be included in differential diagnosis. Especially in neonatals and premature infants, variations in the typical course and appearance of the disease may occur.
Three female patients out of 40 psoriatics examined with scintigraphical methods (Tc-diphosphonate) were found to have a diffuse and circumscribed pathological increase of nuclide at the skull. These abarticular signes of proliferation at the bone-area indicate an early affection of skeleton of patients with psoriasis which is similar to the joint-scan that is mostly obtained in positive bone-scan.

Clinical, X-ray, and scintigraphic (6mCi 99mtechnetium pertechnetate, 4mCi 99mtechnetium pyrophosphate) tests for joint involvement were conducted on 11 inpatients with psoriasis (4 female, 7 male), 6 of them cases of what is conventionally termed psoriatic arthropathy. These tests were performed both before and after a 12-day period of therapy with daily 4 x 1 coated tablets of Demoplas containing 150 mg phenylbutazone Scintigraphic tests showed that pathological radionuclide concentrations were present in at least one joint or group of joints in 10 out of 11 patients. Following therapy the previously pathological scintigraphic findings had stabilized completely in 4 patients and partially in 3 patients, but had not stabilized in 3 patients. The results of therapy with Demoplas as demonstrated by joint scintigraphy were thus good to satisfactory in 7 patients and unsatisfactory in 3 patients- possibly owing to the short duration of therapy. The improvement in scintigraphic results testifies to the therapeutic action of phenylbutazone on arthritic psoriasis. This can also be regarded as additional proof that psoriatic joint involvement is of an inflammatory and proliferative nature.

Tumorscanning of malignomas and their metastatic nodules in head and neck is an enrichment of diagnostic possibilities. For the examination such substances can be used, which are tumoraffine and well to mark with a radio-isotope. Therefore we have marked the cytostatic drug Bleomycine with 57Co. Scans were performed with the Anger-Camera 4 to 6 hrs after intravenous injection of 1 mCi 57Co-Bleomycine. 60 patients with different malignomas have been examined, 46 with tumor-localization in head and neck, the others in thoraxabdomen. 34 (85%) of the head and neck malignomas could be demonstrated; 21 (81%) were squamous cell carcinomas, 13 (69%) sarcomas. The possibilities of this method are shown; its clinical importance lies in the identification of primary tumors and their metastatic nodules, of relapses and tumorrests as well as the localization of distant metastasises.
The biological behaviour of four 99mTc-marked human albumen preparations, prepared by different methods, were studied by serial scintigraphy and excretion measurements. Three preparations consisted of particles of irregular albumen macroaggregates, the fourth was in the form of spherical albumen particles (microspheres). Forty-five patients with various pulmonary abnormalities were examined. The following parameters were used for comparison: half-life of pulmonary activity, changes in activity in liver and spleen as a measure for the formation of Tc-containing particle fragments, uptake in the thyroid and stomach and urinary excretion as an indication of the stability of the nucleid fixation. Despite marked differences in their biological behaviour, all four substances provide comparable information and lack of toxicity and are suitable for lung scintigraphy. Because of the rapid initial decline in activity in the lung, microspheres are more suitable for use with a gamma camera, whereas the other three preparations, in view of their long persistence in the lung can be conveniently used for the slower examinations with a conventional scanner.
