The efficacy of anti-serotoninergic agents in the treatment of erectile dysfunction.

We investigated 3 agents with antiserotoninergic activity (oral trazodone, ketanserin and mianserin) in a double-blind randomized placebo controlled design in terms of the effectiveness in erectile dysfunction. Positive response rates to 30 days of treatment were 65.2% with trazodone, 19.1% with ketanserin, 31.6% with mianserin and 13.6% with placebo. Response to trazodone treatment was significantly better than with placebo (p = 0.0004) and the other 2 agents (p = 0.03 and p = 0.002 for mianserin and ketanserin, respectively). The effectiveness of ketanserin and mianserin seemed to be greater than that of placebo but the differences did not reach statistical significance (p = 0.63 and p = 0.17, respectively).

Reconstruction of penoscrotal transposition.

OBJECTIVE: To assess the results of surgical correction of incomplete penoscrotal transposition and its impact on subsequent definitive urethroplasties and the final outcome. PATIENTS AND METHODS: The medical records of 22 patients who had undergone surgical correction for incomplete penoscrotal transposition at one centre in the last 8 years were evaluated retrospectively. The surgical technique is described and the outcome after correction is noted. RESULTS: All cases had severe hypospadias and approximately half had associated anomalies. No major complication occurred following surgery and the appearance of the external genitalia was satisfactory as judged both by the surgeons and patients. Operative scars did not constitute major handicaps for the subsequent urethroplasties. Sixteen patients have completed reconstructive surgery of whom 12 have glandular and four coronal meatuses. CONCLUSION: Reconstruction of penoscrotal transposition yielded satisfactory results in terms of cosmetic appearance and functional external genitalia, and is recommended before definitive urethroplasty.

Treatment of clinically fixed lymph node metastases from carcinoma of the penis by chemotherapy and surgery.

A 59-year-old patient with carcinoma of the penis was referred for therapy of clinically fixed and ulcerated bilateral inguinal lymphadenopathies. He was treated initially by partial penectomy and subsequently by weekly infusions of methotrexate, bleomycin and cisplatin. This treatment resulted in a decrease in the size of bilateral, lymphadenopathy and re-epithelization of the overlying ulcer. Subsequently a left-sided inguinal lymphadenectomy was performed. At follow-up 18 months later the symptoms had disappeared and no distant metastases were observed.

A case of transitional cell carcinoma of the bladder presenting with brain metastases.

A patient with brain metastases from transitional cell carcinoma of the bladder is presented. This patient had multiple intracranial metastases. The disease progressed very aggressively despite systemic chemotherapy and he died in a very short time. In a review of the literature it is noticed that the incidence of brain metastases from transitional cell carcinoma of the bladder has increased recently. This is probably because of the current complicated therapeutic approach to bladder carcinoma, including radical surgery and combination chemotherapy for high stage disease.

The significance of serum lipid-bound sialic acid in bladder tumours.

Tumour markers with high sensitivity and specificity for bladder cancer are still lacking and several markers have been investigated up to now. Serum lipid-bound sialic acid (LSA) was claimed to be a tumour marker by some investigators and this study tried to assess the value of LSA as a tumour marker in patients with bladder cancer. The results obtained from 27 patients and 30 controls showed that serum LSA levels remained within the suggested normal limits of the method used but they were significantly higher than in the controls. The value of serum LSA is questionable as an initial tumour marker for bladder cancer, but variations from the basal level may indeed be helpful in monitoring the efficacy of the therapy and in detecting relapses during follow-ups.

Diagnostic value of creatine phosphokinase in testicular torsion.

In an experimental study on 36 male guinea pigs testicular torsion was induced in 10, epididymitis in 10, surgical trauma in 6 and only ether anaesthesia was given to 10 animals. Serum creatine phosphokinase (CPK) levels at 0, 4, 8, 20, 28 and 40 hours were determined in each group. Serum CPK levels were found to rise significantly in the torsion group. Determination of serum CPK levels may be an auxiliary alternative in the diagnosis of testicular torsion.

Hydatid cyst of the seminal vesicle.

Hydatid disease has a high incidence in our country and causes significant complications. The authors present a case of unusual localization of hydatidosis, hydatid cyst of the seminal vesicle, and a review of the literature.

Correlation between intracavernous papaverine injection, Doppler analysis and cavernosography in impotent patients.

Forty-four patients with impotence were evaluated by intracavernous papaverine injection, penile brachial index (PBI) determination and dynamic cavernosography. The correlation between these diagnostic procedures and their diagnostic value were investigated. It was concluded that the papaverine test is an efficient diagnostic procedure and can be used as the first step for the routine work-up of impotent patients.

Penile fracture is a rare entity. Twelve patients with penile fractures were treated surgically with a very low complication rate. Our results were compared with those reported in the literature.

Bilateral perineal ectopic testes.

Perineal testicular ectopia is seen very rarely, bilaterality is even more scarce and three cases have been reported in the literature. Here we present the fourth case with bilateral perineal ectopic testes.

Spontaneous perirenal haematoma secondary to polyarteritis nodosa.

Spontaneous perirenal haematoma (SPH) is an unusual but serious complication of polyarteritis nodosa (PAN). We present 3 patients with PAN-associated SPH. One was diagnosed pre-operatively and underwent emergency nephrectomy in order to stop the persistent bleeding; the second patient had surgical drainage of the haematoma and the third recovered without undergoing surgery. The diagnosis is discussed.

Urinary hydroxyproline levels in patients with prostatic carcinoma.

In this study daily urinary hydroxyproline (HOP) levels were evaluated in 26 patients with advanced prostatic cancer and 15 patients with BPH. In patients with prostatic cancer--the ones with bone metastases proved by bone scanning--urinary HOP levels were found to be 69.58 mg/l/day and in those without metastases the levels amounted to 22.55 mg/l/day. In patients with BPH, serving as controls, urinary HOP was 12.80 mg/l/day. Urinary HOP Levels in cancer patients were statistically higher than in the control group. This difference was even more significant in patients with bone metastases. The method detects small metastatic foci of low activity, therefore it may be used also in smaller centres and for effective monitoring of therapy.

Erectile dysfunction following treatments of benign prostatic hyperplasia: a prospective study.

The effects of open prostatectomy, transurethral resection, transurethral vaporization, doxazosin and finasteride on sexual functions of men were investigated in a total of 305 patients with benign prostatic hyperplasia. The sexual functions of the patients were assessed with a questionnaire before treatment and 3 and 6 months after the treatment. A total of 212 (70%) patients were judged to be potent before the treatment. At 3 months, open prostatectomy and transurethral resection caused erectile dysfunction in 2 of 40 (5%) and 5 of 89 (6%) potent patients, respectively. At 6 months, one of the patients from the former and 2 of the patients from the latter groups who developed erectile dysfunction at 3 months stated improvement. Transurethral vaporization caused loss of erectile functions in 4 of 14 potent patients (29%) at the 3-month follow-up and, one of these patients recovered erectile functions at 6 months. Only one of the 33 patients (3%) using doxazosin stated that he lost his erectile functions both at 3 months and 6 months. At 3 months follow-up, finasteride caused loss of erectile functions in 8 of 36 potent patients (22%). Four of these patients underwent surgery (transurethral resection) after 3 months of finasteride use. At the 6-month follow-up, 4 more patients suffered from loss of erectile functions. We suggest that finasteride and transurethral vaporization have the greatest potential of impairing the sexual functions among the treatment options investigated in this study and that they must be carefully offered to the potent patients.
