The effect of supplemental niacin on production and metabolite changes associated with early lactation were measured. Differences of milk production between treatments were not apparent, but serum nicotinic acid concentrations remained higher for those cows fed niacin (12 g/day). Changes of serum metabolites did occur when cows received a single oral dose of 12 or 120 g niacin. Significant alterations of nicotinic acid, free fatty acids, beta-hydroxybutyrate, blood urea nitrogen, serum glutamic-oxalacetic-transaminase, sodium, and chloride occurred within 24 h after initiation of treatment. These metabolites returned to basal concentrations within 60 h after administration.

The skeletal dysplasias are a unique and heterogeneous group of disorders with skeletal manifestations including deformity, disproportion, and short stature. The purpose of this article is to describe the use of the Ilizarov technique in the management of these orthopedic problems in 15 patients with skeletal dysplasias. The technique is particularly appropriate in those patients requiring correction of angular deformity and limb lengthening. The greatest number of complications were encountered in those patients undergoing extended limb lengthening. Nevertheless, all patients achieved their preoperative goals.
A series of four patients presented with a common but unique finding: inability to hold the head in an upright position. Each complained of pain with attempts at passive extension of the neck. All subsequently were diagnosed with spinal cord neoplasms. The literature indicates that the more classic symptoms of spinal cord tumors are often slow to develop and exist for months before being accurately diagnosed. The "hanging head sign" may provide an earlier diagnosis of spinal cord lesions.
Clinical observation of several hundred Ilizarov patients has demonstrated prolonged muscle weakness in the involved limb after treatment. The etiology may be either neuropathic or myopathic. Direct intraoperative neural injury is unusual but generally is detected immediately postoperatively. We wished to determine the etiology of this clinical problem. Six consecutive patients completing tibial lengthening by the Ilizarov method were examined by electrodiagnostic methods. All patients were clinically normal. Electrodiagnostic testing showed abnormalities in six of six deep peroneal nerves. Five of six demonstrated abnormalities in the superficial peroneal sensory responses. In addition, two of six patients demonstrated abnormalities related to the posterior tibial nerve. Concomitantly, sequential perioperative limb compartment pressure studies were performed in nine patients undergoing 12 tibial applications of the Ilizarov technique. Increases in mean compartment pressures to between 20 and 30 mm Hg were demonstrated after tibial osteotomy and in the first 18 hours postoperatively.
The Ilizarov method of gradual incremental distraction of long-bone deformities has provided dramatic functional improvement in children, but its cost in terms of pain may be high. The intensity of pain experienced and its progression with time was measured prospectively in 23 patients (age 11-20 years) by the Adolescent and Pediatric Pain Tool (APPT) and the Hospital for Sick Children (HSC) Pain Scale at four test intervals. These instruments were composed of visual analogue scales, body diagrams, and word lists. Results indicated high levels of pain extending over several months, in sharp contrast to the general orthopedic pain experience. The prolongation of pain throughout the entire correction and consolidation processes must be recognized to ensure appropriate management, particularly in the outpatient setting.
BACKGROUND: A method for measuring the downstream effects of a continuing education conference on nursing practice was developed and tested. METHOD: In collaboration with conference faculty, the authors developed an assessment of knowledge and nursing experience related to the course content. Knowledge about the conference content was measured before the conference and at its conclusion. Experience was measured before the conference and 3 months after. RESULTS: Increases were found in both knowledge and experience, indicating that knowledge gained at the conference was integrated into nursing practice. CONCLUSION: Based on positive findings from this pilot, the method will be tested further.

Between April 1987 and January 1990, 11 patients with congenital fibular deficiency underwent tibial lengthening of 12 involved limbs by the Ilizarov technique. The average follow-up was 23.2 months. The mean lengthening was 8.3 cm, representing an average lengthening of 31%. No knee or ankle subluxation occurred despite preoperative cruciate ligament laxity and ankle abnormalities in all patients. The most significant complications included regenerate bone deformation and delayed consolidation. With the Ilizarov technique, the limb length discrepancy, angular and rotational deformities, and foot and ankle deformities frequently associated with congenital fibular deficiency can be addressed simultaneously.

Congenital forearm pseudarthrosis is a rare clinical entity; only 27 cases have been reported in the English literature. The radius and ulna are affected with nearly equal frequency. Involvement of both bones is extremely rare. Most cases have been associated with neurofibromatosis, which appears to affect adversely the ability to obtain union by conventional corticocancellous grafting. We present a series of six patients treated by various methods. In two of our patients we obtained promising results, as did Allieu et al., by utilizing free vascularized fibular transfer. This may provide an attractive solution to a condition which is historically refractory to conventional operative management.
