Therapeutic effects of the instant rheumatoid medicinal granules for treatment of acute rheumatoid arthritis--a report of 34 cases.

The Instant rheumatoid medicinal granules (IRMG [symbol: see text]) is a proprietary drug for oral administration after being dissolved in boiled water. It consists of the herbs for clearing away heat, detoxifying, eliminating dampness, promoting flow of qi, invigorating blood circulation and relieving pain, used for treatment of acute rheumatoid arthritis. Of the 34 cases treated, 5 cases were clinically cured, and 15 cases markedy effective, the total effective rate being 91.23%. IRMG also showed certain action on immune regulation.

[Serum apolipoprotein C II, C III and E levels in 437 male healthy subjects aged 40-70 in Chengdu area].

Apolipoprotein (apo) C II, C III and E levels of 437 male healthy subjects aged 40-70 [with fasting serum triglyceride (TG levels < 2.26 mmol/L, total cholesterol (TC) levels < 6.21 mmol/L and plasma glucose levels < 6.10 mmol/L] in Changdu area were determined by radial immunodiffusion assay (RID). The results showed that the concentrations of apoC II, C III and E were 41.6 +/- 13.1, 112.8 +/- 31.0 and 38.7 +/- 8.2 mg/L (mean +/- s) respectively. No difference in the serum levels of apoC II, C III and E was noted between the men of different ages. The results of the linear correlation showed that there was a positive correlation of apoC II, C III, E with TG, TC, LDL-C; of apoC II, C III with BMI; and of apoE with ages.

[Relation of serum apolipoprotein B100 levels to lipids and apolipoproteins A I, A II, C II, C III and E levels in the middle and old age normal males in Chengdu area].

This study was conducted in 438 normal men aged 40-70. The age groups were: 40-44, 45-49, 50-54, 55-59, 60-64, and 65-70. The results showed that the serum apolipoproteins levels (mean+/- s, mg/L) were: B100 levels 742.5 +/- 165.8, A I 1301.2 +/- 219.9, A II 299.4 +/- 49.0, C II 42.7 +/- 21.4, C III 113.3 +/- 34.0 and E 38.7 +/- 8.2. The levels of serum apoB100 increased with age. The serum apoB100 levels in the 5th and 6th age groups were significantly higher as compared with the groups 1, 2, 3 and 4 (P < 0.05 and P < 0.05). The fasting serum mean TG (1.39 +/- 0.45 mmol/L), TC(4.79 +/- 0.82 mmol/L) and LDC-C(1.62 +/- 0.40 mmol/L levels in the groups 5 and 6 were significantly higher as compared with the groups 1, 2, 3 and 4 (P < 0.01). Serum apoC II, C III and E levels were significantly increased in the groups 5, 6 (P < 0.05). The correlation analysis indicated that there was a positive correlation of apoB100 with serum TG, TC, LDL-C, apoC II, C III and E respectively (P < 0.01); and a negative correlation with HDL-C levels (r = 0.1312); and apoB100 correlated negatively with apoA I (r = -0.0706). The results suggest that serum TG, TC, LDL-C, apoC II, C III and E are the main factors related with the serum apoB100 levels.

Japan's growing economic activities and the attainment patterns of foreign-born Japanese workers in the United States, 1979 to 1989.

"This study examines the impact of the economic activities of Japan in the United States on the socioeconomic attainments of foreign-born Japanese male workers in 1979 and 1989. It demonstrates that working in wholesale trade, finance and manufacturing industries, three major sectors of Japanese investment in America, provided foreign-born male Japanese workers with the highest likelihood of assuming managerial positions. Moreover, the managerial occupation in turn provided the Japanese workers with the highest earnings returns. This pattern is consistent over time and by length of residence. The results suggest the importance of Japan's economic globalization since the 1970s in explaining the socioeconomic attainment patterns of foreign-born Japanese workers in the United States."

Histological and electron-microscopic observations on the mucosa of pediculate gastric wall graft transplanted to the intestines in Wistar rats.

OBJECTIVE: To study the machanism of the development of intestinal metaplasia through the transplantation of a flap of gastric wall with vascular pedicle to the intestines. METHODS: The pedicled gastric wall flaps of Wistar rats were transplanted to their duodenum, jejunum and colon respectively. After the operation the rats were killed at the 3th, 6th, 9th and 12th month respectively, and histological change of the gastric grafts was observed with optical and electron microscopy. RESULTS: Intestinal metaplasia was found in all the gastric grafts transplanted to the intestines and the alkaline phosphatase positive intestinal metaplasia was seen in the gastric graft mucosa transplanted to the duodenum and jejunum. CONCLUSIONS: The formation of intestinal metaplasia in the gastric mucosa may be related to a change of the microenvironment around the tissues, and gastric mucosa may differentiate into intestinal mucosa by the increase of pH value. The possible mechanism of the development of intestinal metaplasia was discussed.

[A study of loss of heterozygosity and mRNA expression at DCC locus in gastric cancers].

To evaluate the role of DCC gene alteration in the development and progression of gastric cancer, loss of heterozygosity (LOH) and mRNA expression of DCC gene was examined in surgically resected gastric cancer specimens using PCR-RFLP and RT-PCR analyses. LOH was detected in 18 (35.3%) of 51 informative cancers specimens. The frequency of LOH in stages III and IV (50.0%) was significantly higher than that in stages I and II (14.3%, P < 0.05%), but LOH was not correlated to histological types, tumor size, invasion depth and lymph node metastases. Expression of DCC mRNA in 26/51 cases was examined and the DCC expression was lost in 8/26 (30.7%) cancer, and no significant correlation was found between loss of DCC expression or LOH and clinico-pathologic parameters. LOH and loss of expression at DCC gene occur frequently in gastric cancers, LOH at DCC locus is a late event.

Familial amyloid polyneuropathy--clinical report of a family.

This paper reports a familial amyloid polyneuropathy (FAP) family in China. This family being investigated had 69 members of five generations. From the third generation, there have been 16 patients. The age of onset was about 3 to 5 decades. The initial symptoms were autonomic nerve symptoms, such as impotence, dyspepsia and diarrhoea, associated with the sensory loss of lower extremities. As the disease progressed, the upper extremities and motor ability were also involved. The duration of disease course was about 8-10 years, most patients died of infection and cachexia. Sural biopsy in 3 patients had showed positive Congo red staining. From the clinical view, this FAP family is similar to FAP I found in Japan. The true classification, however, should be confirmed by further genetic analysis.

[Measurement of serum apoA I and apoA II in 438 male healthy subjects aged 40-70 in Chengdu area].

Using the method of immunoturbidimetric assay (INA) for apoA 1 and the method of radial immunodiffusion assay (RID) for apoA II developed by ourselves, we measured the serum apoA I and apoA II concentrations in 438 male healthy subjects aged 40-70 in Chengdu area. The contents of serum apoA I and apoA II for male aged 40-70 were 130.1 +/- 22.0 mg/dl (n = 438) and 29.9 +/- 4.90 mg/dl (n = 437) respectively. The serum apoA I and A II levels showed no significant increase with age and had a normal distribution.

[A study on the loss of heterozygosity at the APC/MCC and DCC genetic loci in colorectal cancers].

To evaluate the role of APC, MCC and DCC genes in the development and progression of colorectal cancers, loss of heterozygosity (LOH) at these genetic loci was investigated in 41 surgically resected specimens of colorectal cancer by using polymerase chain reaction (PCR) and restriction fragment length polymorphism (RFLP) analysis. LOH at APC or/and MCC was detected in 38.9% (14/38) of the informative cases; at the APC in 28.0% (7/25) and at MCC locus in 36.4% (8/22). LOH at DCC genetic locus was detected in 55.3% (21/38) of the informative cases. LOH rates DCC gene in the cancer specimens with lymphnode metastasis (80.0%) and in Dukes' stage III and IV (71.4%) were significantly higher than that in specimens without lymphnode metastasis (39.1%) and in Dukes' stage I and II (35.3%) (P < 0.05). Our results suggest that LOH at APC/MCC and DCC is the common genetic alterations in colorectal cancer and DCC genetic LOH assay may be useful in predicting the prognosis of patients with colorectal cancer.

Na,K-ATPase beta subunit isoform expression in the peripheral nervous system of the rat.

Using the RNase protection assay (RPA) to study the distribution of isoforms of the non-catalytic (beta) subunit of Na,K-ATPase in the peripheral nervous system, we found both beta 1 and beta 2 isoform mRNAs in dorsal root ganglion (DRG), but only beta 2 mRNA in sciatic nerve. Using Western blot to measure accumulation of the polypeptides at a ligature on the nerve we found that beta 1 but not beta 2 polypeptide is carried by rapid axonal transport in the sciatic nerve. These results imply that beta 1 is the prominent isoform of Na,K-ATPase in neurons and beta 2 the prominent isoform in Schwann cells.

Posterior cruciate avulsion fracture associated with a large inverted medial tibial osteochondral fragment.

A case of traumatic avulsion fracture of the posterior cruciate ligament in association with a large osteochondral fracture of the medial tibial condyle is described. The osteochondral fragment was inverted 180 degrees. Many associated injuries of the posterior cruciate ligament have been described. This unusual combination of fractures to the best of our knowledge has not been reported before. The mechanism of injury and its clinical implications are discussed.

[Changes of HDL-receptor of hepatic cell membrane in rabbit model of calcium bilirubinate cholangiolithiasis].

HDL-receptor of the hepatic cell membrane and Fch level of the hepatic tissue in the rabbit model of calcium bilirubinate cholangiolithiasis were examined. The results showed: (1)the HDL-receptors' Bmax of the biliary obstruction and infection (BOI) and biliary obstruction (BO) groups decreased during the 1st week after operation (P < 0.01); they were only 69.09% and 49.09% of that in the control group respectively, and the lowered level continued until the 6th week; the Kd values of BOI and BO groups were not different from that of the control group during the 1st week, but they were significantly higher in the 6th week after operation (P < 0.01); (2)the Fch levels of the hepatic tissue were slightly decreased in all experimental groups (P < 0.05). We concluded that under the conditions of biliary obstruction and infection, there was a disorder of the HDL receptors of hepatic cell membrane. At the early stage of biliary obstruction and infection, the receptors decreased in number, and at the later stage, the affinity changed and less HDL entered into the hepatic cell. Then the precurser cholesterol pool for BA biosynthesis narrowed. Thus, BA secreted from the liver cells into genetic elements possibly promote the formation of the calcium bilirubinate gallstones.

[Neuro-specific enolase in acute ischemic stroke and related dementia patients].

Neuron-specific enolase (NSE) levels of cerebrospinal fluid (CSF) were measured in 39 patients with ischemic stroke and 15 controls. There was a significant increase of CSF NSE in patients with acute ischemic stroke as compared with the controls. The altered CSF NSE levels were well correlated with the infarct size in CT scan. The CSF NSE levels were higher in 6-patients who were diagnosed as multi-infarct dementia (MID) after 6-month follow-up than in 22 non-MID patients of this series. Our research supports the view that CSF NSE can be a useful biochemical marker for brain ischemia. The importance of CSF NSE for dementia related to ischemic stroke is worth further studying.

Computed tomographic osteometry of Asian lumbar pedicles.

STUDY DESIGN: The lumbar pedicles of 100 adult Chinese patients were studied with computed tomography to provide data on their dimensions and angular alignment. Data obtained were compared with published measurements of spines from white patients to determine similarities and differences between the lumbar spines. SUMMARY OF BACKGROUND DATA: The age range of the patients was 18 to 60 years. There were 63 men and 37 women. There was no demonstrable sex difference for dimensions or angular alignment of the pedicles. METHODS: The method of scanning and measurement was standardized. Levels considered abnormal by the radiologist were excluded from the study, and a single observer was used to avoid inter-observer errors. The data obtained were statistically analyzed using Student's t test to determine differences between men and women and left and right sides. Age differences were analyzed using the F statistic test. The pedicle inclination angle (transverse angle) and the pedicle dimensions were compared with published data of whites. RESULTS: The pedicles of Asians, when compared with those of whites, showed a similar pattern of changes with vertebral level. However, Asian pedicles had a larger pedicle inclination angle (transverse angle) from L1 to L4 (L1 = 16 degrees, L2 = 16 degrees, L3 = 19 degrees, L4 = 23 degrees), whereas L5 (29 degrees) was similar to many series of whites. CONCLUSIONS: There are differences between the lumbar pedicles of Asians and whites. Such differences need to be considered when posterior pedicle screw fixation of the spine is performed.

Computed tomographic osteometry of the Asian lumbar spine.

The computed tomography (CT) scans of 100 adult Chinese patients 18-60 years of age were studied to provide osteometric data on the Asian lumbar spine. The study population was composed of 63 men and 37 women. A total of 29 parameters were measured from each vertebral level. All the data were statistically analyzed for differences between men and women and left and right sides for paired structures. The data then were compared with data for white individuals where available. The results show that for Asians there is a significant sex difference (p < 0.001) for vertebral body and disk dimensions but not for the posterior elements or spinal canal. Compared with white individuals, Asians have a smaller vertebral body and a different spinal canal shape, with the lower part of the canal in relation to the vertebral body being narrowest. Such findings are of significance to the radiologist, to the surgeon, and in the design of instruments and implants for the Asian lumbar spine.

Pyogenic vertebral osteomyelitis: treatment by anterior spinal debridement and fusion.

Forty-three patients with pathologically proven vertebral osteomyelitis were studied between 1957 and 1990. Thirty-nine underwent anterior debridement and fusion, and four underwent anterior debridement only. The indications for surgery were uncertain diagnosis, persistent pain, failed conservative treatment with uncontrolled sepsis, and neurological involvement. Thirty patients were followed-up for an average period of 5 years, with a minimum of 2 years and the longest for 15 years. All their symptoms improved after surgery; only one patient subsequently deteriorated due to multiple level recurrence. All patients with neurological deficit improved. Bony fusion occurred in 93% of cases (average time to fusion, 6.8 months), and 90% of the patients were able to return to their original work 4-20 months after surgery. We feel that anterior debridement and spinal fusion allow for reliable microbiological and histological diagnosis, and rapid relief of symptoms and return to work. Primary bone grafting is successful despite the presence of infection.
