[Lipid electrophoresis in the alcoholic. Features and usefulness in the diagnosis of various degrees of alcoholic liver diseases].

Reference is made to the importance of assessing serum lipoproteins by conventional agarose gel electrophoresis in the preliminary diagnosis of the degree of severity of a liver disease. A comparison is made between lipidogram changes in 48 alcoholic patients with the liver histologies following needle biopsy according to Menghini. The conclusion is drawn that in acute and chronic alcoholic liver disease there is a characteristic lipidogram profile, marked by a double "camel's hump" in the bands due to predominance of the beta and prebeta-lipoprotein fractions. This, in turn, is related to the liver steatosis observed bioptically. In macro and micronodular cirrhosis, however, examination of the electrophoretic lipid fractions is frequently unhelpful. Attention is drawn to the appearance of the alpha fraction as a presumably pointer to the condition in question. It is felt that this conventional, simple evaluation of lipoproteins is a valuable diagnostic test in assessing the seriousness of alcoholic liver disease.

[Pseudo-angina pectoris and acute formation of an esophageal diverticulum. Description of 2 cases].

After a short introduction aiming at outlining the etiopathogenesis of fore thorax acute pain occurring in quite healthy individuals, the authors describe two cases of individuals who complained of retrosternal pains to be related, in their opinion, to acute neoformation of a diverticulum of the esophagus. The Authors conclude by remarking how such newly formed diverticulum of the esophagus is to be included among the etiopathogenetic causes of both fore and back thoracic pains and they underline the necessity of an X-ray examination of the esophagus on subjects where the cause of acute thoracic pain in uncertain.

[The preleukemic syndrome. Aspects of the problem and consideration of 5 cases].

Chronic acquired peripheral cytopenia, usually the result of ineffective myelopoiesis, is the main feature of the dysmyelopoiesis described as the preleukaemic syndrome. Probably attributable to a stem cell disorder, this condition may well progress to acute myeloblastic leukaemia. Five cases in which a retrospective diagnosis of preleukaemia was possible are presented. Each patient was followed from the onset of the disease and throughout its entire course. The possibility that the preleukaemic picture might have been secondary to exposure to marrow-damaging agents or an initial paraneoplastic syndrome, or was some other disease (Björkman's acquired sideroblastic anaemia, refractory anaemia with partial Dreyfus myeloblastosis, nocturnal paroxystic haemoglobinuria) was ruled out in all cases.

[Behaviour of the LAP (leucine-amino-peptidase) in the serum of blood in subjects affected of cirrhosis of the liver in stage ascitical (author's transl)].

The AA. have studied behaviour of the LAP (leucine-amino-peptidase) in the serum of blood in subjects affected of cirrhosis of the liver in stage ascitical, of cause toxic-alcoholic excluding about of the casuistry the cirrhosis of the another cause and the cirrhosis that are compled white an obstruction in and extrahepatic standard biliary cirrhosis 1 degree e 2 degree. After the exposition of the casuistry in the registred normality of the test (seric-LAP) in some cases of hepatic-alcoholic cirrhosis, the AA. concluded considering the LAP an indicator only of the obstruction in and extra-hepatic, consistent with hepatic-organ well operating; do not be a secure test to follow development of a chronic hepatitis of any kind, even if in the hepatic disease is present an infiltration and an overturning of the hepatic-lobule, like it is normally in the hepatic alcoholic diseases.

Tympanometry in children with allergic respiratory disease.

The results of pneumatic otoscopy and tympanometry were compared in 102 children who were being followed in allergy clinic for allergic rhinitis or asthma. Forty-five abnormal tympanograms were found in 30 children, including six type B tympanograms, 20 type C and 19 with reduced compliance. Acoustic reflexes were absent or elicited only at thresholds greater than or equal to 100 DbHL in 45 ears of 34 children. Agreement between two examiners and tympanometry improved significantly during the study from 62% to 75% and from 62% to 79%, respectively. Tympanometry is an important adjunct to the evaluation of allergic children and supplies objective information useful in teaching proper evaluation of signs of middle ear disease.

[Agranulocytosis induced by rifamycin SV in a patient with Hodgkin's disease. Immunologic considerations and pathogenetic interpretations].

Alter a comprehensive introduction, intended to vocalize the pharmacologically-induced leukopenia problem, it is herein described an agranulocytosis case, Rifamicin SV-induced, in an abdominal Hodgkin subject. In the process determinism, much probably immunological, we deem two factors, at least, having acted in convergence: the Rifamicin haptenic activity and the immunosuppressive action of the same. We also believe that, in the pathogenesis of the hematological situation observed, a fundamental role must be ascribed to the immuno-deficient conditions peculiar to patients affected by Hodgkin's disease.

[Serum levels of pseudocholinesterase in alcoholic cirrhosis patients. Correlation with the extent of anatomo-functional damage. Unfavorable prognostic index].

Following an introduction on the question of pseudocholinesterase (CHE) in cirrhosis of the liver and acute and chronic hepatopathy, a personal study of 35 chronic cases at various stages of the disease is reported. The patients were submitted to the usual haematochemical liver function tests as well as to liver needle biopsy with anatomo-functional staging of the liver trouble, which was largely due to alcohol poisoning. The CHE level in these patients was monitored until death. On the basis of the results, measurement of CHE is identified as an index not only of liver function (this is of course well known) but of prognosis infausta, there being a confirmed correlation between survival and CHE serum level.
