Dysfunction of the Björk-Shiley prosthesis. Report of 32 cases.

We present 32 cases of dysfunction of the Björk-Shiley valvular prosthesis, representing 3.01% of the 1,063 BS prostheses implanted at this clinic. The complications observed were thrombosis of the prosthesis, suture dehiscence with or without endocarditis and disc-suture interference. In selected, favourable cases, we prefer thrombectomy with generous washing of the prosthesis and rotation of the disc, without removal of the disc from the prosthesis. Furthermore, when the dehiscence does not exceed 1/3 of the circumference of the ring, we have achieved very good results with single sutures supported on Teflon patches. Subsequent failures of this procedure were not of a technique nature, but rather due to the poor condition of the patients. In situations of clinical emergency, the diagnosis of valvular dysfunction may suffice as indication of surgery, further diagnostic measures being postponed in view of the data of the physical exploration.

Thrombectomy: surgical treatment of the thrombosed Björk-Shiley prosthesis. Report of seven cases and review of the literature.

Among 1,000 patients with 1,225 Björk-Shiley prostheses, we have detected 12 cases (0.98%) of late thrombosis of the Prosthesis, seven of which were successfully managed by thrombectomy. A review of other authors' experience with this procedure shows no deaths directly attributable to thrombectomy, although two patients had cerebral embolization during the procedure. For prevention of this complication, we propose that both the aorta and the left atrium be explored during thrombectomy of either the mitral or the aortic prosthesis so as to achieve complete exposure of the left ventricular outflow tract and complete removal of any thrombus located in the ventricle. We consider this the procedure of tract and complete removal of any thrombus located in the ventricle. We consider this the procedure of choice in thrombosis of the Björk-Shiley prosthesis. Replacement of the prosthesis would then be restricted to cases of prosthesis damage or those in which adequate anticoagulation cannot be maintained.

[Renovascular hypertension: results of surgical treatment with a three-year follow-up (author's transl)].

Between 1966 and December 1979, 58 patients with renovascular hypertension were surgically treated at our center. Follow-up was a minimum of one year in all patients and in all cases permeability of revascularization was assessed through angiography from one to six months after surgery. The mean follow-up was 39 months. Thirty-eight (65.5%) were male and 20 (34.6%) female. Age of onset was between 3 and 53 years (mean 36.3 years). Etiology was arteriosclerotic in 27 cases and fibrous dysplasia in all its manifestations in another 27, in 2 cases vascular compression due to retroperitoneal fibrosis and renal hypoplasia in the remaining two. In 4 cases lesions were bilateral. The cure rate was 37 patients (63.8%), Improvement in 16 (27.6%), while there was no improvement in 5 (8.6%). Surgical mortality rate was nil. The different techniques used, indications, long and short term complications and different results according to etiology are discussed.

Membranous obstruction of the inferior vena cava and hepatic veins. Budd-Chiari syndrome? A treatable disease.

Three cases of membranous obstruction of the inferior vena cava (IVC) are presented. Two of them underwent transcardiac membranotomy with good results. The third patient was treated symptomatically and died from gastrointestinal hemorrhage and hepatic coma two years after the diagnosis of Budd-Chiari syndrome. At autopsy, membranes were found in the inferior vena cava and hepatic veins. Differentiation of Budd-Chiari syndrome secondary to the presence of such a membrane is essential since these patients are potentially curable by surgical treatment. Cavography visualizes the obstruction and is the procedure of choice for diagnosis of this condition. Surgical treatment with transcardiac membranotomy is effective. Some hepatic abnormalities regress after surgery. Prophylactic postoperative treatment with diuretics seems advisable in these patients to prevent congestive cardiac failure.

Treatment of acquired chronic esophagopleural fistula by endoscopic cauterization.

Successful treatment of an eight-year-old esophagopleural fistula by means of endoscopic cauterization with an alkaline solution is reported. Previous therapeutic attempts with other more conventional measures had failed. The technique of the successful treatment is described. No complications could be attributed to its use.
