The development of priorities for health services research. The National Center, 1974-76.

The priorities published by the National Center for Health Services Research/DHEW in 1975 were the result of more than a year of consultation between federal officials and members of the research community. The process of priority development emerged from an interpretation of the history of the National Center, of health services research as an activity, and of the previous experience of various research agencies. This paper is a combined memoir and intellectual history of the priority development process in the National Center by a participant who, because of his position, was somewhat detached from events.

Enhancement by prolactin of carcinogen induced mammary cancerigenesis in the male rat.

Mammary tumours were induced in 3 groups of male Long-Evans rats by a series of 6 fortnightly gastric intubations of 7,12-dimethylbenzanthracene. Two weeks before the initial carcinogen treatment one group of rats was grafted with 3 pituitary homografts underneath the kidney capsule of each recipient (hyperprolactinaemia). A second group, 2 weeks before the initial carcinogen treatment and for the duration of the study (35 weeks), were injected 4 X weekly with 2-Br-alpha-ergocryptine (CB-154) (hypoprolactinaemia). A third group of rats served as controls. A significant increase in the incidence of mammary tumours and a reduced latency period of tumour appearance in the hyperprolactinaemia group, when compared with the controls, were observed in this study. Mammary tumour incidence and latency period of tumour appearance in the hypoprolactinaemia group, however, did not differ significantly from controls. Thus, an increased secretion of pituitary prolactin in rats appears to be an important enhancing endocrinic condition in carcinogenesis of the male mammary gland.

Mixed lymphocyte reactions and tissue transplantation tolerance.

The induction of tolerance of Lewis histocompatibility antigens in BN rats inoculated at birth with BNI Lewis F(1) hybrid bone marrow cells, as revealed by the prolonged survival of Lewis skin grafts, is accompanied by markedly decreased reactivity in the mixed lymphocyte interaction. Blood lymphocytes from animals inoculated with lymph node cell suspensions also display diminished proliferative reactivity to hybrid BN/Lewis cells in the interaction. However, these recipients are not tolerant of Lewis skin grafts. Blood lymphocytes from BN rats inoculated neonatally with Lewis thymocytes fail to display any level of unresponsiveness in vitro, and such animals are not tolerant of Lewis skin grafts. The results suggest that in rats skin and marrow cells have histocompatibility antigens that are absent or poorly expressed on lymph node cells and thymocytes.

Perinatal transport at Wesley Medical Center, Wichita, Kan.

Fixed-wing aircraft, helicopters, and ambulances all are part of the transport system at this regional perinatal care center, one of only two in Kansas. The neonatal transport teams are made up of respiratory therapists and registered nurses.
