Prognosis of testicular tumour since the introduction of complex therapy.

Change of approach in dealing with testicular carcinoma has made it necessary to set up a central organization covering a larger geographic area in Hungary where 242 patients had been treated since 1980. Progress in the development of therapy-oriented methods cleared the way for reliable and accurate stage determination. Data analysis seems to indicate that correct therapeutic principles combined with plenty of surgical, radiological and chemotherapeutic experience may contribute a lot to improving the pathologic prospects. The mortality rates for the pure seminoma group and for the non-seminomatous plus mixed-cell group of patients were 7.5% and 26.5%, respectively. The results promise additional chances--even with increasing morbidity--to reduce the mortality rate and the incidence of complications.

Radical nephrectomy through total median laparotomy.

Twenty-two carcinoma patients aged less than 65 years were subjected to radical nephrectomy through total median laparotomy. This type of approach provides excellent access to both essential operation phases: vessel management and lymphadenectomy. Postoperative abdominal complications and hernia have not been encountered.

Multiple urolithiasis in bilharziasis patients.

A total of 780 patients were operated on for lithiasis. Among them 97 presented multiple urolithiasis which was often associated with Schistosoma infection. Bilharziasis, in accord with theoretical considerations, plays a part in the formation of calculi, especially in that of multiple urolithiasis.

The effectiveness of venous hypothermia in the left kidney.

Twenty renal parenchyma operations of the left kidney were performed in venous hypothermia. The hypothermic solution was perfused via the spermatic (ovarian) vein, outflow occurred through the vessels opened by the incision. The effectiveness of venous hypothermia was measured by the probe of an electrical thermometre inserted into the renal tissue. The temperature of the cooled kidney became constant between 20 and 23 degrees C by using a perfusion solution of 4-5 degrees C. The method can be used for its simplicity and safety also in extensive renal parenchyma operations, its only limitation is that it can be performed only on the left kidney.

Current diagnostic and therapeutic methods in the treatment of vesical tumours.

Reviewing the contemporary therapy of vesical tumour, the authors point to the basic principles that govern the daily practice, the precise application of which may go a long way to improving the results. Not ignorable is the experience that one part of these tumours defies discovery by multifocal and cystoscopic examination. The fundamental tenet of tumour surgery to remove the tumour by incision into sound tissue, must be respected. A brief review is given of the main lines that promise headway in dealing with the disease.

A new approach to the treatment of polycystic kidneys.

Maximum conservativism was the watchword up to 1979 in dealing with polycystic disease. In the period 1979 to 1985 a total of 45 patients were subjected to 80 instances of polycystic resection whereby larger cysts were removed by excision, smaller ones by ignipuncture, with particular attention to extirpation of the perihilar cysts. Post-operative incidence of hypertension dropped from 33 to 3%. The method described is rated as one of the feasible expedients to deal with polycystic disease in the early compensated stage.

Urological tuberculosis mimicking bladder tumour.

A total of 8 patients have been treated in the recent 10 years in whom urogenital tuberculosis simulated the picture of bladder tumour. This occurred in 3% of overall patients with urogenital tuberculosis, i.e. primary diagnosis proved to be erroneous in such percentage of them. The course of the disease of 2 patients is reviewed in detail. In order to establish the differential diagnosis between the two diseases, the importance of bladder biopsy following polycystography is pointed out.

Cytostatic therapy, its value and indications in the management of prostatic carcinoma.

Observations with cytostatic therapy of generalized prostatic carcinoma (PC) over 5 years are reported, cyclophosphamide (cpa) having been administered to hormone-resistant patients. A standard therapeutic and follow-up scheme, enabling the results to be assessed objectively, had been elaborated. Objective remission or stagnation, confirmed on this basis, was attained in 50%, relief of pain in 60% of the cases. Side effects were negligible. Withdrawal of cytostatics was required in 2 cases. The results indicate that secondary cytostatic therapy significantly potentiated the effect of hormone therapy in generalized PC, and in case of hormone resistance it offers the only chance of therapeutic benefit.

Organ-preserving surgery in kidney tumours.

After a survey of the international literature on organ-preserving surgery in kidney tumours, a surgical material of 12 cases is presented, in which a mean survival of 51 months has been attained. In similar cases medical treatment gave survivals of 7 months only. The factors affecting the survivals are analysed and resection in stage T1-2 is advocated on these grounds. Organ-preserving surgery is regarded as justified in stage T1N0M0, even if the other kidney is intact. The decisive importance of appropriate selection of the patients and of a meticulous surgical technique is emphasized.

Surgery of calculous hydrocalyx (calyceoplastic surgery).

A modified surgical technique for calculous hydrocalyx, based on partial calyceal resection with simultaneous correction of the morphological abnormality responsible for stone formation is described. Access to the calyx is gained by a modified marginopolar resection. The modification developed by the authors consists in carrying the resection line laterally to the vertical axis of the kidney. The parenchyma is closed by U-shaped "hair-pin" stitches. Preparation of the limbic, pelvic and ureteral areas is abstained from, thus avoiding scarring which might affect renal lymph and blood flow and interfere with urinary transport. The results obtained in 85 cases of calculous hydrocalyx over an 8-year period were satisfactory. The rate of calculous recurrences was not higher than 3.5 per cent.

Complex treatment and care of prostatic carcinoma patients.

The authors' 5-year experience in the management and care of prostatic carcinoma are summarized. Their method differs essentially from earlier practice. They have found a new diagnostic and therapeutic method by introducing the TECO irradiation therapy, extensively using bone scintigraphy, by introducing cytostatics, extensively applying the prostate-specific acid phosphatase and by performing rectal biopsy of the prostate. They describe their own observations on the diagnostics and therapy of prostatic carcinoma. They stress that none of the therapies is the method of choice, the use of the various kinds of treatment are defined by strict indications. They state that care of prostatic cancer patients is highly important because only observation of the course of the disease may ensure the evaluation of treatment results and the indication of the adequate therapeutic method.

Experiences on the local adriamycin treatment of bladder tumours.

Following repeated resections, local cytostatics have recently been increasingly applied for preventing the recurrence of superficial bladder tumours. Fifty-five patients at stages Tis, Ta, Tl were treated intravesically by adriamycin for 16 to 24 months. After the treatment period, the total number of tumour recurrences decreased by more than half. In 18 cases there were no tumour recurrences. The administered cytostatics did not produce any systemic side-effects. Chemocystitis and haematuria were the most frequently associated side-effects. Based on favourable experiences, local adriamycin treatment is looked upon as an effective prevention of recurrences.

[Primary carcinomas of the scrotum--5 cases].

Etiology, diagnostic difficulties and clinical feature of the scrotal carcinomata are described. In the course of 10 years 5 cases were observed. The patients at stage T3N2M0-1 survived still 4 to 8 months after operation, while the patients at stage T1 are all still alive. The significance of the early diagnosis is emphasized, in order to prolong the survival time.
