Effects of low dose rate irradiation on the division potential of cells in vitro. IV. Embryonic and adult human lung fibroblast-like cells.

Early and late passage human embryonic lung fibroblasts were compared with early passage adult lung fibroblasts with regards to their survival (number of population doublings), after low dose rate ionizing radiation. It was found that early passage embryonic cells are quite resistant to this type of radiation. Late passage embryonic and early passage adult fibroblasts are more sensitive to ionizing radiations. The results suggest that cell aging is accompanied by an increased sensitivity to low dose rate ionizing radiation and favor the idea that aging in vitro, expressed as a function of the fibroblast division potential, is correlated with aging in vivo.

Prognostic factors and treatment of multiple myeloma: interest of a cyclic sequential chemohormonotherapy combining cyclophosphamide, melphalan, and prednisone.

1. A type 2 therapeutic trial consisting of the administration of monthly cycles of chemohormonotherapy, each cycle combining weekly sequences of mephalan, prednisone, cyclophosphamide, and prednisone, has been achieved in 20 stage II or III myeloma patients. Tolerance of this regimen in treated out-patients was found to be excellent. Preliminary data indicate that the better survival rate in patients treated by this regimen is still not reached at a 30-month follow-up examination by three other nonrandomized control groups of patients receiving continuous therapy with prednisone alone, prednisone + cyclophosphamide, or prednisone + melphalan. 2. Analysis of the main prognostic factors of the four trials indicates that a) IgG-type myelomas are associated with a better prognosis than IgA type; nonexcreting myelomas are associated with the best prognosis, while Bence Jones myelomas are associated with a prognosis as poor as that of the IgA type; b) tumor volume as well as renal insufficiency, at the time of diagnosis, are also prognosis factors; this study confirms the prognostic value of the recently proposed clinical staging system based on these parameters but outlines that 10% of the patients died from a cause not directly related to myeloma plasmocyte proliferation. 3. In conclusion, these results point out: a) the possible advantage of using two alkylating agents instead of one at the beginning of the disease; b) the need to classify multiple myeloma according to prognosis before attempting therapeutic trials.

[Topographic diagnosis of urinary infection by determination of fraction 5 of lactate dehydrogenase].

Amongst the various methods suggested for the topographical diagnosis of urinary tract infection, the study of various fractions of lactico-dehydrogenase in the urine would appear to be particularly valuable. Study of 44 subjects with a significant urinary infection, compared with an identical group of subjects free of urinary infection, showed that in those with pyelonephritis fraction 5 was significantly (p less than 0.001) higher than in those with a lower urinary tract infection. However this was valid only in the absence of renal insufficiency. Combined study of immunological techniques (urinary antibodies revealed by immunofluorescence and serum antibodies) shows that these techniques are complementary and make it possible to exactly localise the site of a urinary tract infection. In relation to immunology, enzymology has the advantage of being suitable for use during periods at which bacteriological examination of the urine is negative, and hence is useful in following the influence of treatment.

[Serum and urinary antibodies in pyelonephritis (author's transl)].

Pyelonephritis is accompanied by complex immunological reactions, the study of which is just now being undertaken. Study of 216 patients with a significant monomicrobial urinary infection showed that in more than 95 per cent of cases of Gram negative infection the urine contained IgG type antibodies, often associated with IgA. These antibodies may be easily detected using an immunofluorescent technique, which would appear to be the best method available at present for distinguishing between pyelonephritis and an infection of the lower urinary tract. They may also be found in the serum, though less frequently. Detection by immunofluorescence is markedly more sensitive than using the classical passive haemagglutination technique. The biological significance of these antibodies remains imprecise, and their protective or facilitating role may be discussed. Their specificity must be determined, in order that sequential study of these immunological reactions may be of help in the conduct of treatment.

[Use of demeclotetracycline in the treatment of hyponatremia in cirrhotic ascitis].

The activity of demeclotetracyclin, and ADH antagonist, is studied in 11 ethylic patients with cirrhosis of the liver, under a large hydric diet (1500 cm3). The prescription of the cyclin (600 mg daily) is always determined by a fall of the urinary osmolarity (-36%) and by a dramatic improvement of the free water clearance (+ 60%); consecutively, we observe an increase of natremia in 8 out of 9 cases. Associated with Spironolactone (200 mg daily) the anti-ADH activity persists (the free water clearance becomes positive in 5 out of 10 patients), in spite of the natriuretic activity of anti-aldosterone ; a minimal fall of the natremia is observed in only 2 cases. The indication of Demeclotetracyclin in the curative or preventive treatment of the hyponatremia of the liver cirrhosis is discussed.

Antibodies in Escherichia coli urinary tract infection.

In 105 adults with E.Coli urinary tract infections, IgG coated bacteriuria was found in 8/9 with acute pyelonephritis (PN), 17/20 with chronic PN, and in only 2/76 with lower UTI. IgA was present in 66% of PN, but IgA secretory piece in less than 10%. These urinary IgG antibodies were, at least in part, synthesised in the kidney because serum IgG antibodies were detected by indirect immunofluorescence in only half the patients. 06, 018, 022 E.Coli serotypes were the three most frequently found O groups, but their prevalence in PN is not significant. The immunology of urinary tract infection (UTI) is still a subject of little interest in adult nephrology; but antibody production is a well-characterised event in pyelonephritis (PN), the study of which seems to be the best indirect procedure for localising the site of UTI. We have analysed the production of urinary and humoral antibodies, and their correlation with E.Coli serotypes in patients with E.Coli UTI.

[Bumetanide, a new diuretic active during chronic renal failure (author's transl)].

Bumetanide is a new diuretic, which acts in the ascending limb of Henle's loop. The use of very high doses (5 to 40 mg/24 h) in 32 patients with advanced chronic renal insufficiency gave a clear diuretic effect in half the patients with a creatinine clearance of less than 3 ml/min and in 2/3 of the cases with a creatinine clearance between 3 and 10 ml/min. The efficacy of high doses of bumetanide, as well as their good tolerance shows an action of this new diuretic similar to that of furosemide.

[Immunofluorescence study of bacteriuria. A new technic for the localization of urinary infection].

The detection by immunofluorescence of immunoglobulins fixed to the wall of urinary bacteria makes it possible to localise the site of the urinary infection. The presence of immunoglobulins was noted in 18 out of 19 patients with a high urinary tract infection involving the renal parenchyma. They were absent in 25 out of 28 subjects with a lower urinary tract infection. The three apparently discordant positive reaction involved 1 case of prostatitis and two cases in which the diagnosis of pyelonephritis was made later. In the case of renal involvement, these immunoglobulins, essentially IgG, are seen with greater frequency than humoral antibodies. Whilst the biological significance of these immunoglobulins remains uncertain, their existence probably being a reflection of parenchymatous inflammation, this nevertheless is a new and, apparently, reliable, method for the determination of the site of an infective process.

Hormonal control of isoperoxidases in lentil embryonic axis.

Detachment of the cotyledons from the lentil (Lens culinaris Med.) embryonic axis causes in the latter an increase in total peroxidase activity which is shown to be due to enhancement of specific cathodic isoperoxidases. Kinetin treatment of attached or detached axes promotes activity of essentially the same cathodic isoperoxidases. In addition kinetin enhances the activity of two anodic peroxidases and represses specifically that of a cathodic one. Abscisic acid inhibits the production of all isoenzymes in the presence or absence of kinetin. Cytokinin and abscisic acid actions are discussed in relation to the nature of a wounding hormone and the role of natural inhibitors in cotyledons during germination. Indoleacetic acid stimulates the activity of certain isoenzymes which are also stimulated by kinetin, whereas in others the effects of the two hormones are different. Specific inverse effects of indoleacetic acid and kinetin are demonstrated on the two most cathodic isoperoxidases. Indoleacetic acid-kinetin interactions on the cathodic isoperoxidases have been found in the literature and are discussed as a possible mechanism for explaining interactions of the two regulators on growth and other physiological processes.
