Anterior chamber suture in pseudophakic retinal detachments requiring intraocular gas.

We have devised a simple technique to facilitate the use of vitreous cavity gas in complicated cases of pseudophakic retinal detachment with pupillary support intraocular lenses. A 10-0 monofilament suture is placed across the anterior chamber just prior to instilling gas in the vitreous cavity. This retaining suture prevents the pseudophakos from being displaced forward by the gas bubble, possibly damaging the corneal endothelium. The anterior chamber suture is removed easily at the slit-lamp examination several days postoperatively when intraocular gas is no longer present.

Ocular motility complications following intranasal surgery.

The ocular motility problem of two patients following intranasal surgery is examined. The patients were studied by eye movement records and ultrasonography. Surgery in both cases revealed a defect of the medial orbital wall with scar tissue incarcerated in the defect.

Ultrasonographic features of choroidal malignant melanomas.

In a 12-month period 26 eyes were enucleated after an ultrasonographic diagnosis was made of malignant melanoma of the choroid. Histologic features of the tumors were reviewed. The ultrasonographic diagnosis was correct in 100% (26/26) of enucleated eyes. Spontaneous vascular movement (SVM) was identified by standardized A-scan echography in 69% (18/26) of the tumors. Contact B-scan ultrasonography demonstrated choroidal excavation in 65% (17/26) of the melanomas. During the 12-month study period, choroidal excavation was also demonstrated in cases of metastatic choroidal tumors, hemangiomas of the choroid, and long-standing choroidal nevi. The standardized A-scan finding of SVM is a compelling sign in the diagnosis of choroidal malignant melanoma. In contrast, the B-scan finding of choroidal excavation is a less useful sign since it may also be seen in various other ocular lesions.

Ultrasonographic diagnosis of massive periretinal proliferation in eyes with opaque media (triangular retinal detachment).

A triangular pattern of retinal detachment, as seen by B-scan ultrasonography in eyes with opaque media has been correlated with advanced massive periretinal proliferation (MPP). The ultrasonographic triangular configuration was created by a proliferative membrane that united detached retina anteriorly. Thirteen eyes with opaque media and a triangular retinal detachment by ultrasound had severe MPP at vitrectomy or enucleation. Twelve eyes with clear media with advanced MPP were also studies by B-scan ultrasonography and a triangular detachment could be demonstrated in each case. The finding of a triangular retinal detachment by B-scan ultrasonography in eyes with opaque media is diagnostic of advanced MPP. Eyes with this type of retinal detachment have a poor surgical prognosis even with modern vitrectomy techniques.

Urinary estriol for assessment of fetoplacental function.

A total of 1,607 urinary estriol determinations were performed in 447 patients with normal and complicated pregnancies. In general, the test provided reliable information concerning fetal status, particularly in pregnancies complicated by toxemia, diabetes, and postmaturity. When measured consecutively, a drop of 50% or more in estriol level usually should be considered a significant index of fetal distress. Limitations of the test and several compensatory measures aimed at minimizing inaccuracy are discussed.

Bright-flash electroretinography for the evaluation of eyes with opaque vitreous.

Standard retinal function tests are of limited value in assessing retinal function in eyes with opaque vitreous. We developed a method for obtaining an electroretinogram (ERG) from eyes with significant vitreous opacity by utilizing a much brighter than normal stimulating light. Of 115 eyes with vitreous opacities and good ERG responses to this bright-flash photostimulator, 47% would have been nonrecordable with a conventional ERG light source. Bright-flash ERG was often helpful in evaluating eyes with vitreous opacities for vitrectomy and was sometimes the only source of information regarding potential retinal function.
