Comparative studies of conversion disorders in childhood and adolescence.

In a syndrome validation study, four groups of patients suffering from conversion disorders, anxiety states, obsessive-compulsive disorder and emotional disorders, as well as a healthy control group, were compared. The following findings emerged: besides female preponderance and greater age, a higher rate of low SES subjects and social migration patients suffering from conversion disorders displayed a greater proportion of psychiatric and medical diseases in their parental heritage.

The relative importance of history and symptoms in child psychiatric diagnosis.

This study analyzed the relationship between case history and reported symptoms and child psychiatric diagnosis in a sample of 928 cases. In addition sex, age, socioeconomic status and intelligence were considered. In general, symptoms were more important than case history data and background factors. Antisocial symptoms contained by far the most information for diagnosis. However, the highest information gained by a combination of different variables was only 20%. Overall, case history data represent an nonspecific factor of vulnerability in child psychiatric disorders which are primarily determined by symptoms.

Convergence of parent checklists and child psychiatric diagnoses.

This study examined the correlation between the Childrens' Behavior Questionnaire by Rutter, Tizard, and Whitmore (1970) and clinical diagnoses in a total of 1,468 in- and outpatients. The following diagnoses were considered: conduct disorder, emotional disorder, mixed disorder of conduct and emotions, and the hyperkinetic syndrome. In general, correlations were low, but subscores indicating conduct or neurotic disorders had somewhat higher correlation with these respective disorders than the hyperactivity subscore. Sensitivity figures (i.e., percentage of true positives) were moderate to low, while specificity figures (i.e., percentage of true negatives) were high. It is concluded that convergence of parent checklists and child psychiatric diagnoses in unselected samples is only moderate.

[Maternal age and psychological health of the child. Study of a population in consultation].

The data of all first born or only children seen at the department of child and adolescent psychiatry at a university hospital from 1978-84 were analyzed according to maternal age at birth of the patient. Mixed emotional and conduct disorder as well as hyperkinetic syndromes are overrepresented in the youngest maternal age group (less than or equal to 17 at delivery). The difference to other groups remains significant for the former diagnosis even after matching for age, sex, socioeconomic status and whether or not the natural parents lived together at the time of presentation. Children of teenage mothers (less than or equal to 20 at delivery) were considerably disadvantaged in terms of socioeconomic status and family stability.

The validity of the hyperkinetic syndrome. A study in child psychiatric clinic attenders.

The total group of hyperkinetic children drawn from the pool of consecutive admissions to a child and adolescent out-patient department were compared with further groups admitted consecutively to the same institution in 1979 and 1980. These other groups consisted of normal controls, conduct disorders, emotional disorders or mixed disorders. The question of validity was studied in terms of several comparisons including sex, age, SES, family history, development characteristics, neurological findings and questionnaire scores. It was found that some data support the concept of concurrent validity of the hyperkinetic syndrome, namely the extremely asymmetric sex distribution in favour of boys, some indicators of developmental delay, the early onset of the symptoms and the higher rates of coordination deficits detected by neurological examination.

Computer analyzed EEG in methylphenidate-responsive hyperactive children.

Computer-based analyses of the EEG-records were performed in 18 hyperkinetic children. Thirteen of these patients displayed a positive response to clinical treatment with methylphenidate. Discriminant analyses of native EEG-records and differences between the spectrums from the EEG with and without medication made it possible to properly classify 73.3 or 80%, respectively, of all patients. Even based on a relatively small sample it is possible to make predictions as to the effectivity of stimulant treatment solely on the basis of EEG-findings.

Maternal age and autistic children.

Maternal age was examined in 52 autistic children. Two matched control groups were also investigated. Both were matched with the autistic group on age, sex, and birth order. In addition, the first control group was matched with the autistic group according to IQ, while the second control group was composed of normally intelligent children and matched with the autistic group on SES. Maternal age did not differ between the three groups. It is concluded that uncontrolled social class factors might account for raised maternal age in middle class mothers of autistic children found in other studies.

[Combined cytostatic chemotherapy of advanced non-small-cell bronchial carcinoma with doxorubicin and ifosfamide].

Fifty-three patients with inoperable non-small cell bronchial carcinoma were treated at four-weekly intervals with two cytostatic drugs, doxorubicin (50 mg/m2 on day 1) and ifosfamide (2000 mg/m2 on days 1-3). To avoid urotoxicity of ifosfamide, mesna, a uroprotective drug, was additionally given intravenously at a dose of three times 400 mg/m2 on days 1-3. All diagnoses had been histologically and/or cytologically confirmed. Adenocarcinoma was present in 22, large-cell undifferentiated carcinoma in 18, and squamous-cell carcinoma in 13. Distant metastases were present in 46, seven had a regionally localized tumour growth. There were one complete and 20 partial remissions (response rate 40%). Among a further 19 patients temporary growth arrest was registered. The remissions occurred in seven with adenocarcinoma, nine with large-cell carcinoma and five with squamous-cell carcinoma. Median remission was 8.3 months, mean survival time 10.5 months. Patients without response survived a mean of 5.5 months, patients with tumour progression for 1.3 months (Kaplan-Meier method). Most prominent among side-effects were cardiotoxicity and infection during the leukopenic phase. Urotoxicity was minor, due to treatment with mesna. The results suggest that doxorubicin and ifosfamide in combination can be considered an effective means, with acceptable toxicity, of treating advanced non-small cell bronchial carcinoma.

Criteria of functional operability in patients with bronchial carcinoma: preoperative assessment of risk and prediction of postoperative function.

In this prospective study of 433 patients undergoing surgery for bronchial carcinoma 3 main factors influencing the operative risk were identified: (1) extent of resection, (2) pulmonary function and (3) age. The 30-day mortality rate was 8.3% in the whole group which comprised a high percentage of patients over 70 years of age (27%) and with a disturbed pulmonary function (45%). The value of various functional criteria proposed in the extensive literature is assessed and compared with the results of the study in which the use of FEV1 and quantitative regional analysis by perfusion scanning and 'regions of interest' proved to be highly efficient, allowing, in addition, the prediction of postoperative lung function even in sleeve resections. A new formula for estimation of the additional loss of function in the early postoperative phase after lobectomies is proposed along with a flow sheet for routine preoperative evaluation of pulmonary function.
