The AA. describe two cases of serious juvenile myasthenia arisen after jennerian vaccination, antidiphteric and antitetanic vaccination in the first patient and after antitetanic vaccination in the second one. They put forward the hypothesis that in the two patients juvenile serious myasthenia is a complication due to vaccination for the following reasons: 1) the chronological relation between vaccination and the arising of myasthenic sumptomatology is very close; 2) the vaccine produces in the body a reaction of immunity type and myasthenia is nowadays generally considered as an immunity disease. Although the ways in which the vaccine inoculation can give rise to the disease are unknown, it can be supposed that in the occurrence at the same time of a genetic factor or of an susceptible area, the vaccine can act as a factor provoking the immunity answer of myasthenic type.

Sixty-four all-night sleep polygraphic recordings have been carried out in patients affected by Parkinson's syndrome, before and after treatment with an association of l-dopa plus a peripheral dopa-decarboxylase inhibitor. Without any drug, parkinsonian patients showed sleep disruption with frequent awakenings, reduction of total sleeping time and lessening of REM stage rather than slow sleep. Treatment with l-dopa plus inhibitor appeared to be able to restore good night sleep organization with a noteworthy increase in the REM stage.
The effect of preheated (37 degrees C) mixture of 0.5% bupivacaine and fentanyl on peridural anaesthesia have been studied in a double blind randomized clinical trial. Fifty patients of both sexes, ASA I-II, undergoing elective surgery for inguinal hernia repair or radical saphenectomy have been randomly assigned either to the control group (n = 25) receiving 0.5% bupivacaine-fentanyl at room temperature (20 degrees C), or to the group receiving the same anaesthetic mixture preheated for 12 hours up to 37 degrees C (n = 25). The latency period, extension, duration and quality of anaesthetic block were evaluated. A significative difference in the extension of anaesthetic block was detected at 6' (p less than 0.05 for extension up to T10 and T11, p less than 0.025 up to T12), and at 8' (less than 0.05 for extension up to T10). However no difference was observed between the two groups at 10' and later. The use of 0.5% bupivacaine-fentanyl mixture resulted in a short latency period, as previously observed, and its preheating does not seem to affect substantially the parameters evaluated in this study.

Different forms of experimental allergic encephalomyelitis were obtained in 31 guinea pigs. Four groups of animals were studied: 7 adult Hartley guinea pigs (Group 1), 12 adult Hartley guinea pigs (Group 2), 6 juvenile strain 2 guinea pigs (Group 3), 6 juvenile strain 13 guinea pigs (Group 4). The increment of the antigenic load induced different disease courses in the animals. The different disease courses were attributed to differential inactivation of the cell-mediated response to the injected antigens. The intraneural injection of serum withdrawn from guinea pigs affected by EAE induced demyelination in the optic nerve fibers of healthy rats. Areas of demyelination with naked axons were evident 30 days following the injection when Visual Evoked Potential (VEP) delays were of as much as 27 msec. Our experiments demonstrated the activity of EAE sera (probably antiGal-Cer antibodies) on the myelin sheats of nervous fibers.
