Accumulation of galactonate in liver of suckling rats perfused with galactose.

Livers of 14-day-old suckling rats were perfused for 30 min in the nonrecirculating (once-through) mode with media containing 0-60 mM galactose. Galactonate was detected in livers perfused with as little as 1 mM galactose and increased with higher perfusate galactose to 178 nmoles per gram of liver at 60 mM hexose. Tissue galactonate levels were as high as 60% of galactose-1-phosphate which also accumulated under the same conditions. Galactonate was also found to be present in the effluent perfusate. The perfused suckling rat liver appears to be a physiological model for assessing the role of galactose oxidation to galactonate as an alternate metabolic pathway of galactose metabolism.

[A case of simulation of ovarian cyst in volvulus of an ectopic spleen].

A case of volvulus of ectopic spleen mistaken for twisted ovarian cyst is described. Attention is drawn to six points: 1) the rarity of the condition; 2) the persistent uncertainty concerning its aetiopathogenesis, even though over a century has passed since the first case was reported; 3) the mechanism leading to twisting of the spleen stalk; 4) the absence of specific symptoms and the consequent impossibility of preoperative diagnosis in most cases; 5) the ready confusion with ovarian cyst; 6) the reasons for supposing that the reported case was the result of congenital factors.

[Esophageal voice: a critical analysis of the results obtained by different rehabilitative technics].

In order to evaluate the phonatory results obtained with the use of tracheoesophageal prostheses, 38 laryngectomees were examined, 20 of which underwent erigmophonic voice rehabilitation while the other 18 were rehabilitated with the application of a prosthesis. In all cases electroglottography and spectrography of the voice were performed. In the erigmophonic-voice subjects, electroglottography gave a normal glottic wave while spectrography gave a tracing with frequent interruptions, scarce harmonics, altered by the presence of background noises. In the tracheoesophageal prosthesis patients, the electroglottogram showed regular hypopharynx vibratory activity while the spectrogram appeared free of interruptions activity while the spectrogram appeared free of interruptions and presented a uniform distributions and presented a uniform distribution of suffixes. The data obtained demonstrate that better results are obtained with the use of tracheoesophageal prosthesis as it allows the exhaled air to be used in phonation and thus respects respiratory dynamics and pneumophonic synergism. It also makes it possible to achieve a clear, fluent voice. Furthermore, the results obtained with patients with this prosthesis were constantly satisfactory when the long exercises required were performed.

[The quality of medical records: a retrospective study in Lazio Region, Italy].

INTRODUCTION: Medical records have an important role in the communication among different care providers and in forensic medicine. In Italy, information on completeness and correctness of medical records is scanty, whereas future hospital accreditation could take into account their quality as a proxy of good medical practice. PURPOSE: We performed a retrospective study in order to assess the quality of medical records in the Lazio region. METHODS: From all 37009 hospital discharges for five different diseases in 123 hospitals (acute myocardial infarction (AMI), coronary artery bypass surgery, pneumonia, cerebrovascular disorders, breast surgery), registered in the Regional Hospital Information System, we selected a random sample of 2022 (5.5% of the total). Ten physicians, previously trained, reviewed the relative medical charts and filled in "ad hoc" questionnaires. RESULTS: A total of 1960 (97% of the target) charts were reviewed. Organization and structure of data recording strongly varied. Important differences were found across the diseases for various items: presence of anamnesis 98.1% (range: from 95.6% for breast surgery to 100% for AMI); presence of physical examination 92.7% (range: from 88.1% for breast surgery to 98.5% for AMI), completeness of the daily medical records was good in 70.8% (range: from 34.2% for pneumonia to 93.9% for cerebrovascular disorders). Variability among different type of hospitals was also observed, being teaching hospitals and some private hospitals more accurate. CONCLUSIONS: Quality of medical records tended to vary across different type of hospitals and different diseases. Actions for improving the quality should be undertaken as a priority. Efforts have to be done in restructuring charts, creating guidelines and training caregivers. The development and application of computer based health information systems should help solving these problems.
