Effect of concurrent zidovudine use on the resistance pathway selected by abacavir-containing regimens.

OBJECTIVES: Abacavir (ABC) selects for four mutations (K65R, L74V, Y115F and M184V) in HIV-1 reverse transcriptase (RT), both in vitro and during monotherapy in vivo. The aim of this analysis was to compare the selection of these and other nucleoside reverse transcriptase inhibitor (NRTI)-associated mutations by ABC-containing therapies in the presence and absence of concurrent lamivudine (3TC) and/or zidovudine (ZDV) and to assess the effect of these mutations on phenotypic susceptibility to the NRTIs. DESIGN: This study was a retrospective analysis of the patterns of NRTI-associated mutations selected following virological failure in six multicentre trials conducted during the development of ABC. METHODS: Virological failure was defined as confirmed vRNA above 400 HIV-1 RNA copies/mL. RT genotype and phenotype were determined using standard methods. RESULTS: K65R was selected infrequently by ABC-containing regimens in the absence of ZDV (13 of 127 patients), while L74V/I was selected more frequently (51 of 127 patients). Selection of both K65R and L74V/I was significantly reduced by co-administration of ZDV with ABC (one of 86 and two of 86 patients, respectively). Y115F was uncommon in the absence (seven of 127 patients) or presence (four of 86 patients) of ZDV. M184V was the most frequently selected mutation by ABC alone (24 of 70 patients) and by ABC plus 3TC (48 of 70 patients). Thymidine analogue mutations were associated with ZDV use. The K65R mutation conferred the broadest phenotypic cross-resistance of the mutations studied. CONCLUSIONS: The resistance pathway selected upon virological failure of ABC-containing regimens is significantly altered by concurrent ZDV use, but not by concurrent 3TC use. These data may have important implications for the efficacy of subsequent lines of NRTI therapies.

Elective cerclage vs. ultrasound-indicated cerclage in high-risk pregnancies.

OBJECTIVE: To compare pregnancy outcome after elective vs. ultrasound-indicated cervical cerclage in women at high risk of spontaneous mid-trimester loss or early preterm birth. METHODS: This was a retrospective study comparing two management strategies in women with singleton pregnancies who had at least one previous spontaneous delivery at 16-33 weeks of gestation. One group was managed by the placement of an elective cerclage at 12-16 weeks and the other group had transvaginal ultrasound examinations of the cervix at 12-15+6, 16-19+6, and 20-23+6 weeks and cervical cerclage was carried out if the cervical length was 25 mm or less. RESULTS: A total of 90 patients were examined, including 47 that were managed expectantly and 43 treated by elective cerclage. In the expectantly managed group, 59.6% (28/47) required a cervical cerclage. We excluded from further analysis three patients who were lost to follow-up and three because of fetal death or iatrogenic preterm delivery. Miscarriage or spontaneous delivery before 34 weeks' gestation occurred in 14.6% (6/41) of the elective cerclage group, compared with 20.9% (9/43) in the expectantly managed group (chi2 = 0.219, P = 0.640). CONCLUSION: In women at increased risk of spontaneous mid-trimester or early preterm delivery, a policy of sonographic surveillance followed by cervical cerclage in those with a short cervix reduces the need for surgical intervention without significantly increasing adverse pregnancy outcome.

Current evidence for the use of paediatric antiretroviral therapy--a PENTA analysis. Paediatric European Network for the Treatment of AIDS Steering Committee.

UNLABELLED: The introduction of combination antiretroviral therapy has been associated with a dramatic clinical improvement in children with human immunodeficiency virus infection. However, the uptake of antiretroviral therapy has been variable across Europe. The Paediatric European Network for the Treatment of AIDS Steering Committee has performed a systematic literature review of paediatric antiretroviral therapy trials. An analysis of the evidence base for the commencement and maintenance of antiretroviral therapy was produced. Suggestions for when to commence antiretroviral therapy, which drugs to start with and how to monitor and sequence drug regimens are given. CONCLUSION: The aim of these guidelines is to help in obtaining equity of access to a uniformly high standard of care for children with human immunodeficiency virus infection in all European countries.

Modelling the costs of paediatric HIV infection and AIDS: comparison of infected children born to screened and unscreened mothers.

OBJECTIVE: To estimate the cost of managing children with HIV/AIDS in the UK from a health service perspective. DESIGN AND METHODS: Epidemiological, resource use and unit cost data were combined within a decision analytic model. A Markov model was developed to predict the prognoses of HIV-infected children under different assumptions about natural history, treatment efficacy and the timing of antiretroviral therapy. Resource use estimates for various stages of HIV/AIDS were based on published data relating to the UK health service and clinical judgement; unit cost data were taken from a London centre. RESULTS: The base-case results suggest that the cost of caring for an HIV-infected child is higher if the mother's infection was known about at or before the child's birth (antenatal screened cohort): lifetime costs ranged from 46 427 pound sterling to 119 502 pound sterling per child in the screened cohort and from 38 691 pound sterling to 86 014 pound sterling in the unscreened cohort. However, the screened cohort benefited from longer life expectancy (base-case, 11.66 versus 10.09 years) and AIDS-free life expectancy (base-case, 7.13 years versus 6.22 years). Results are sensitive to assumptions about natural history and treatment efficacy: for example, if antiretroviral therapy was initiated at birth, and assuming optimistic natural history parameters, discounted costs could increase to 215 077 pound sterling and the additional lifetime cost of a child born to a screened mother could be 72 491 pound sterling. CONCLUSIONS: Results reflect the marked uncertainty regarding the cost of, and prognosis for, children with HIV/AIDS in the new era of more potent antiretroviral combination therapies. These results are part of an assessment of the relative cost-effectiveness of alternative antenatal HIV testing strategies.

Prevention of mother-to-child transmission of HIV-1 infection: alternative strategies and their cost-effectiveness.

OBJECTIVE: To estimate the cost-effectiveness of alternative interventions to reduce the risk of mother-to-child transmission of HIV. DESIGN: A model capturing the sequential nature of mother-to-child transmission in utero, at delivery and postnatally was used to determine how the effects of bottle-feeding, elective Cesarean section (CS) and zidovudine (ZDV) would combine to prevent mother-to-child HIV transmission. Parameter estimates were derived from the literature, UK health service costs applied, and incremental cost effectiveness ratios (ICER) estimated for alternative risk reduction strategies. Results can be transposed to other cost assumptions or currencies. RESULTS: In a woman who breast-feeds her baby, has a vaginal or emergency CS delivery and takes no ZDV, the estimated transmission risk is 31.6% (range, 23.7-38.1%), at a cost of 400 UK pound per woman; this falls to a risk of 3.7% (range, 1.7-6.9%) when bottle-feeding, ZDV therapy and elective CS are all implemented at a cost of 1968 UK pound per woman. From a public health perspective the ICER of ZDV and elective CS each depend on the acceptance rates of the other. In women counselled against breast-feeding, ZDV with 100% acceptance of elective CS has an ICER of 11 342 UK pound (95% confidence interval (CI), 7084-21 515 UK pound]. However, the ICER of CS ranges from 9248 UK pound (95% CI, 5072-46 913 pound sterling) at zero ZDV acceptance to 27 895 UK pound (95% CI, 10 018-154 462 pound sterling) at 100% ZDV acceptance. CONCLUSIONS: Considering the estimated cost of caring for an infected child, ZDV appears to be cost-effective under any of the circumstances examined. However, elective CS may not be cost-effective in populations where the uptake of ZDV is high, and a more precise estimate of its efficacy is required.

Regression of AIDS-related Kaposi's sarcoma during therapy with thalidomide.

A 14-year-old girl with HIV infection and subcutaneous Kaposi's sarcoma (KS) received thalidomide therapy for oral ulcers, resulting in regression of KS lesions, disappearance of KS-associated herpesvirus (KSHV) DNA from blood, and reduced viral load in tumor tissue. Administration of grauntocyte colony-stimulating factor resulted in clinical exacerbation of KS and reappearance of KSHV DNA in blood.

Persistence of antibody responses to Haemophilus influenzae type b polysaccharide conjugate vaccine in children with vertically acquired human immunodeficiency virus infection.

BACKGROUND: Recurrent bacterial sepsis is common in pediatric HIV infection and immunization against Haemophilus influenzae type b (Hib) is recommended. Long term persistence of anti-Hib antibody and the need for, or timing of, a booster dose has not been adequately studied. METHODS: Immunogenicity during a 12-month period following immunization with Hib-tetanus conjugate vaccine (ACT-HIB; Merieux) was evaluated in 48 vertically HIV-infected children and 36 uninfected children, born to HIV-positive mothers. A titer of anti-Hib polysaccharide antibody of > or = 0.15 microgram/ml was considered to indicate short term and > or = 1 microgram/ml long term protection. RESULTS: At 1 month postvaccination 36 (100%) uninfected and 42 (88%) HIV-infected children achieved titers of > or = 1 microgram/ml. However, by 1 year titers had dropped below this value in 18 (43%) infected compared with only 4 (11%) uninfected children (chi square, 9.7; P = 0.002). Although the rate of fall of antibody titer was greater in uninfected than in infected children, this was no longer the case after adjustment for the 1-month postimmunization titer. The rate of antibody titer decline was not significantly related to HIV disease status or to either the age-related CD4 count at the time of immunization or the change in age-adjusted CD4 count during the 12 months after immunization. CONCLUSIONS: Not only was the initial antibody response to Hib conjugate vaccine decreased in children with HIV infection and AIDS but also 1 year later only 57% of the initial responders had persisting titers above the level associated with long term protection. The need for reimmunization of children with HIV infection against Hib requires further evaluation.

Vertical transmission of Kaposi's sarcoma.

AIDS related Kaposi's sarcoma is commonly seen in homosexual men, only occasionally in men and women with heterosexually acquired HIV, and extremely rarely in children. The case of an HIV infected mother and her vertically infected child who both developed visceral Kaposi's sarcoma is reported. It is proposed that the putative Kaposi's sarcoma agent may also be transmitted vertically.

An assessment of tonometry and regional splanchnic blood flow during aortic cross-clamping in the pig.

We aimed to evaluate the tonometer in the assessment of gastrointestinal ischaemia induced by an infrarenal aortic cross-clamp. Nine anaesthetized pigs were cannulated for haemodynamic monitoring and radionuclide labelled microsphere (RLM) injection. Gastric and sigmoid tonometers were positioned. After haemodynamic stabilization an infrarenal aortic cross-clamp was applied. Animals were sacrificed at the completion of the study and tissue sampled from the stomach and sigmoid colon for regional blood flow measurements. Measurements were made pre-clamp, post-clamp, pre-release and post-release. Haemodynamic parameters, gastric intramucosal pH (pHi) and blood flow did not change throughout the experiment. Arterial pH increased during cross-clamp and returned to baseline post-release. Arterial bicarbonate fell post release. Sigmoid blood flow fell during cross-clamp. The sigmoid pHi fall, delayed until pre-release, remained low post-release. Although there was a consistent fall in sigmoid pHi, 63% of post-clamp values remained within the baseline range. We conclude that maintaining haemodynamic parameters around baseline values resulted in maintenance of gastric mucosal perfusion as indicated by a steady gastric pHi. However, below the aortic cross-clamp, delay between change in sigmoid colon blood flow and change in pHi and wide variation in sigmoid pHi limits the value of an individual pHi measurement in detecting ischaemia.

Review of efficiencies and patient satisfaction in Australian and New Zealand day surgery units: a pilot study.

A pilot study was performed in eight Australasian day surgery facilities with a purpose of identifying common trends and differences. A prospective study was designed in which information was collected on 826 patients over a two-week period. Patients were well matched for age, anaesthetic type and mean surgical time. Three facility types were identified and results were statistically corrected for any differences that ASA status, age and surgical time may have made. Patient preoperative waiting time, recovery room times, delayed discharge time and unanticipated admission rates showed favourable outcome trends for free-standing facilities compared with hospital-integrated facilities where day patients had a shared recovery with inpatients. Similar trends were seen with patient opinions of waiting times and recovery periods. In summary, this pilot study has demonstrated the impact of different facility types on efficiencies and patient satisfaction both of which have important cost implications and relevance to those involved in continuous quality improvement processes in day surgery.
