The effect of fraction size on control of early glottic cancer.

A retrospective analysis of 600 patients treated for head and neck malignancy at the Cooper Hospital/University Medical Center was undertaken. Patients who had surgical intervention (excluding biopsy) were withdrawn from this review. Fifty-eight patients with Stage I Glottic Laryngeal Carcinoma were identified and constitute the basis of this report. Various parameters were analyzed to assess their impact on local control. These include age, sex, serum hemoglobin, tumor bulk, differentiation, field size, total dose, total treatment time, and fraction size. Overall local control was 87% with a median follow-up of 63 months. The only factor that influenced local control was fraction size. Of 28 patients treated with 180 cGy fractions, seven (25%) had a local recurrence within 3 years. Twenty-eight patients treated with 200 cGy or greater fractions have had no failures to date. The difference in control rate when comparing the two treatment schema was significant (p less than 0.01). The median dose in the controlled 180 cGy group was 6660 cGy (range, 6300-7020 cGy). In the patients who failed in the 180 cGy group the median dose was 6660 cGy (range, 6480-6840 cGy). The patients receiving 200 cGy fractions or greater had a median dose of 6600 cGy (range, 6000-6950 cGy) and an average dose of 6507 cGy. The mean NSD in the 180 cGy group failing was 1787 RET (range, 1735-1843 RET). Patients who were controlled and received 180 cGy fractions had a median NSD of 1796 RET (range, 1743-1868). The mean NSD in the 200 cGy group was 1847 RET. The median TDF in the 180 cGy group of patients controlled was 102. Those failing also had a TDF of 102 (range, 101-105). Patients receiving 200 cGy fractions or greater had a median TDF of 109. It appears from this data that fraction size is a highly significant factor in our ability to control glottic laryngeal cancer.

Influence of mediastinal adenopathy on site and frequency of relapse in patients with Hodgkin's disease.

Between April 1969 and July 1979, 267 patients with surgically staged IA-IIB Hodgkin's disease and 31 with clinically staged IIE or IV Hodgkin's disease were treated; disease was limited to the area above the diaphragm. Three separate groups of patients with mediastinal adenopathy were identified. Patients with disease limited to the mediastinum had a favorable prognosis, and treatment with mantle followed by para-aortic irradiation has resulted in 85% relapse-free survival and 100% overall survival at 8 years. Surgically staged patients with large diameter mediastinal adenopathy (greater than 0.33 of the thoracic diameter) have a high relapse rate with radiation therapy alone. The use of combined modality therapy has reduced the risk of relapse in these patients, but the survival remains higher whether MOPP is used initially or at relapse. Clinically staged IIE patients or stage IV patients with extensive extranodal disease limited to the thoracic cavity were treated with chemotherapy and involved- or extended-field irradiation. These patients had a 5-year survival of 66% with large mediastinal adenopathy and 90% with lesser mediastinal disease.

Posthypophysectomy taste abnormalities: their relationship to remote effects of cancer.

Abnormalities in taste sensation have been studied in 15 patients consisting of five normal controls, five patients with diffuse neoplasm and five patients post hypophysectomy. Threshold recognition for salt and HC1 in the three groups studied was the same. Sucrose and urea recognition was higher in patients with neoplasm as compared to normal controls. Patients post hypophysectomy had a lower threshold recognition for sucrose than both normal individuals and patients with neoplasm. The threshold recognition has markedly shifted for both sucrose (decreased) and urea (increased) post hypophysectomy. These observations are in support of previous findings and suggest that the pituitary plays at least a necessary permissible factor in the development of abnormalities in taste as observed in patients with disseminated cancer.

Effect of hyperoxia and androgen on red cell 2,3-diphosphoglycerate and oxygen affinity.

Increased plasma erythropoietin (ESF) activity, a rise in RBC 2,3-DPG, and a 'right shift' of the oxygen hemoglobin equilibrium curve following androgen administration to mice in ambient or hyperoxic conditions, was demonstrated. If androgens had a direct effect on the RBC metabolism, increased 2,3-DPG would result in a facilitated release of oxygen to the tissue. This would have been accompanied by a decrease rather than an increase in the level of ESF. Hyperoxia has abolished detectable rising levels of plasma ESF and RBC 2,3-DPG following androgen administration. These levels were close to those seen in the ambient nontreated mice.

Tissue culture of bone marrow. V. Effect of 5Beta(H) steroids and cyclic AMP on heme synthesis.

In mice 5beta(H) steroids have been shown tostimulate heme synthesis in vivo. The current studies were undertaken to evaluate the effect of these steroids on heme synthesis by use of the "well" method of marrow culture. The 5beta(H) steroid metabolites 3alpha-hydroxy-5beta-pregnane-11, 20-dione, 3beta-hydroxy-5beta-pregnan-20-one, 3 alpha,17alpha-dihydroxy-5beta-androstan-3-one stimulated Fe uptake by human marrow explants cultured in wells. The dibutyrl analog of adenosine 3',5'-monophosphate (db cAMP) also stimulated Fe uptake. The effect of the latter compound was enhanced by theophylline. There was no additive nor synergistic effect between db cAMP and the 5beta(H) steroid metabolites. There was no inhibition of the steroid metabolite effect by either antierythropoietin serum or cyproterone. Uptake of fe was also enhanced by the cations Ca++ and Co++.

Androstanes

Effective erythropoiesis induced by 5beta-pregnane-3beta-hydroxy-20-one in squirrel monkeys.

The erythropoietic effect of 5beta-pregnane-3beta-hydroxy-20-one, a naturally occurring steroid metabolite of progesterone, was evaluated in the squirrel monkey by ferrokinetic studies. red cell survival, and blood volume measurements. The intramuscular administration of this steroid in pharmacologic doses shortened the (59)Fe plasma clearance and increased the plasma iron turnover, thereby indicating an increase in erythropoiesis. A normal (59)Fe red cell uptake was observed, and the bone marrow maturation time was not altered. Red cell survival was the same in the treated and control groups. After five weekly injections of the steroid, the monkeys increased their red cell mass by 57%. A significant increase in white blood cells and a slight elevation of platelet counts in the treated monkeys also suggest a possible direct stimulation of hemopoietic stem cells by the steroid metabolite. These observations indicate that some steroid metabolites can stimulate an early increase in iron turnover (within 48 h) that is not secondary to hemolysis. The increased red cell mass indicates effective erythropoiesis in primates.
