Evaluation of cytokines regulating bone turnover in the serum of post-menopausal and senile women.

The authors evaluated the Insulin-like Growth Factor-1 (IGF-1) stimulating the formation of bone tissue, and interferon gamma (IFN gamma), inhibiting bone resorption, in the serum of women, 13 of fertile age, 34 of post-menopausal age, and 14 of senile age. Values for IGF-1 in the serum were considerably low in patients of post-menopausal and senile age, and presented highly significant differences with values for subjects of fertile age. The values for IFN gamma did not present significant differences between different age groups. It may be assumed that post-menopause and during senile age physiological osteopenia may be favored by a decrease in the secretion of IGF-1.

Granular acute lymphoblastic leukemia in children. "Aieop Cooperative Group for Cytology of Acute Leukemias".

BACKGROUND: Granular acute lymphoblastic leukemia (ALL) is a rare morphological variant of ALL, characterized by cytoplasmic azurophil granules or inclusions, positive for aspecific esterase and acid phosphatase, with heterogeneous features at the ultrastructural level. METHODS: In an attempt to determine whether the presence of granules or inclusions marks a biologically distinct variety of ALL with peculiar clinical features, a prospective morphological review was undertaken of children entering AIEOP protocols for ALL in the period from 1985 to 1989. RESULTS: Of 531 cases examined, 16 (3%) were found to have greater than 1% granular bone marrow blasts, with 7 cases (1.3%) having greater than 10%. The presence of granules or inclusions was associated with the immunophenotype of "common" ALL. There was no clear association with FAB type L1 or L2 nor with particular clinical or hematological findings at presentation. Complete remission was achieved in all cases; one patient died of infection in remission at 3 months and 2 patients relapsed after 12 and 32 months, respectively, while the others are still in remission after a minimum follow-up of 24 months. CONCLUSIONS: In conclusion, granular morphology seems to have no prognostic importance in children ALL.

B-cell acute lymphoblastic leukemia (B-ALL): a report of 17 pediatric cases.

BACKGROUND AND METHODS: B-cell acute lymphoblastic leukemia (B-ALL) presents FAB L3 morphology and surface Ig, CD19, CD20, CD24. This pattern may show some morphological and immunological heterogeneity. Seventeen pediatric cases of B-ALL at onset, treated in twelve AIEOP Centers (Italian Association for Pediatric Hematology and Oncology), are presented. RESULTS AND CONCLUSIONS: Clinical and hematological features were characterized by low WBC counts at presentation, high M/F ratio, older age and association with extramedullary involvement. The overall survival curve at 78 months is 40%. All patients showed blasts positive for surface Ig (sIg), DR, CD19, and CD24. Ten/17 cases presented the classical features of B-ALL: FAB L3 morphology, sIg+ restricted to light chains, CD20+, cytoplasm mu (c mu)-, CD10-, TdT-. The remainder showed some differences in this pattern, such as non-L3 morphology (3 cases), absence of CD20 (3 cases), CD10+ (4 cases), TdT+ (3 cases), c mu+ (1 case), lack of surface light chains (1 case). This rare ALL subset seems to be characterized by a high phenotype heterogeneity, indicating various degrees of differentiation.

Effectiveness of differentiation inducers in combination on human neuroblastoma cells in vitro.

The effects of epirubicin and retinoic acid (RA) as differentiation inducing agents on human neuroblastoma cell lines were investigated. We have compared the response of neuroblastoma cells to epirubicin alone, to RA alone and to combined treatment, with respect to neuritic processes outgrowth, acethylcholinesterase activity, growth inhibition and antigenic expression. The obtained data indicate that the combination of the two agents is able to produce a synergistic effect on differentiation and on growth inhibition.

Histomorphometric evaluation and pathogenesis of prosthetic loosening.

On the basis of a vast experience deriving from the histological study of explants of hip joint prostheses, the authors emphasize the role of histomorphometric testing in allowing for a quantitative evaluation of periprosthetic cellular reaction, of the bone structure, and of the wear phenomena of the implant, in order to gain a better understanding of the pathogenetic mechanisms responsible for the phenomenon of loosening. In agreement with what has been reported by other authors, they describe what they were able to observe by image analysis, microradiography, x-ray diffraction, and microdurimetry in various types of explanted prostheses. In particular, they describe data on the features of the wear particles, the periprosthetic cellular component, the thickness of the interfacial membrane and of the joint neocapsule, the volume of bone-ingrowth, the contact surface, and the structure of the bone-implant interface. Based on the changes observed using histomorphometry, a pathogenetic hypothesis was formulated according to which physical phenomena closely related to the type of implant used, such as wear and metal ion release, and non-physiological stress exerted on the bone, cause a series of cellular chain reactions. Within this context, macrophages, lymphocytes, osteoblasts and osteoclasts are in a continuous state of activation and, both directly and by means of the production of cytokines and other substances, they are responsible for bone resorption and thus loss of continuity between the bone and the implant, resulting in prosthetic loosening.

Evaluation of minimal residual disease in high risk childhood acute lymphoblastic leukemia using an immunological approach during complete remission. AIEOP Cooperative Group for Immunology of Acute Leukemias.

BACKGROUND: Sensitive methods for detecting residual disease may complement conventional morphology while monitoring the response to treatment in leukemia patients. METHODS: We studied minimal residual disease (MRD) by selecting via a colony assay a peripheral blood (PB) cell population enriched in putative malignant cells and detecting occult leukemic cells by double immunologic analysis performed on colonyforming cells (CFC). Using this combined technique we assayed the PB of high risk children with acute lymphoblastic leukemia (ALL) in order to demonstrate possible differences in "the residual tumor cell burden" among leukemia patients and to correlate these with a 3-year clinical follow-up. RESULTS: In all 22 patients positive results were obtained for up to 18 months following induction chemotherapy at times of apparent hematologic remission. Common ALL (cALL) patients exhibited a mean of 9.6% cAlla+ cells (range: 2% to 30%), whereas cALL antigen (cALLA) and cALLA/Tdt or CD1a/Tdt combination were never found in the colonies derived from healthy individuals. Six out of 22 cALL patients expressed a mean of 16.5% cALLA+/Tdt+ CFC (range: 2% to 35%). Five T-ALL children presented a mean of 24% CD1a/Tdt+ cells (range: 8% to 44%). Extensive follow-up indicates a correlation between the percentage of CFC Tdt+/lymphoid marker+ cells (more than 10%) and subsequent clinical relapse. In one patient relapse occurred after 16 months with a dramatic increase in the number of leukemic cells. In contrast, the decline in malignant cells, observed in two cases, predicted a favourable course. Five patients tested before autologous bone marrow transplantation (ABMT) presented high number of positive cells and relapsed at various times. CONCLUSIONS: We conclude that this approach to the study of MRD could be valuable in monitoring the efficacy of chemotherapy, as well in evaluating the quality of purged marrow.

Gamma radiation-induced differentiation on human neuroblastoma cells in culture.

We have studied the effect of gamma radiation on differentiation in neuroblastoma cell lines AF8 and SJ-N-KP. Growth inhibition and morphological and biochemical differentiation have been examined following radiation exposure to 1-10 Gy. Gamma radiation induced marked growth inhibition and morphological differentiation in a dose-and time-dependent manner in both cell lines, and induced biochemical differentiation in AF8 cells.

[Assessment of various immunologic parameters in silicone implant carriers].

The Authors evaluated some immunological parameters in women carrying silicone gel-filled breast implants for over one year. Peripheral blood samples from 22 patients were examined in order to assess both the antigenic pattern of lymphocyte subpopulations by cytofluorimetric analysis, and the cell proliferation of PHA-stimulated lymphocytes by the uptake of tritiated thymidine. These tests were performed at the time of the sample withdrawal and after in vitro reexposure to silicone extract for 48-72h. Changes in lymphocyte subpopulations and functional response were observed when patients were divided into groups according to the type of surgery, i.e. breast augmentation or reconstruction, or to the degree of periprosthetic capsular contracture. These results suggest the possibility of an interaction between silicone and the immune system, which cannot be disregarded for the explanation of the silicone related complications.

Evaluation of several immunological parameters in patients with aseptic loosening of hip arthroplasty.

The authors evaluated several immunological parameters in patients with loosened hip arthroplasty in order to determine the state of sensitization with regard to the metallic constituents of the prosthesis. The results obtained by in vivo epicutaneous testing did not reveal a correlation between loosening and sensitization, as the patch test was positive in only 1 out of 16 cases examined. An evaluation of the lymphocyte subpopulation on peripheral blood demonstrated that patients with prostheses in Cr, Co, Mo, Ni alloy present significant lymphopenia, with a reduction in subpopulations CD4 and CD8 and a decrease in in vitro cytotoxic activity. The meaning of these modifications could be interpreted to be either a toxic effect products released from the implant, or as the recruitment of lymphocytes in the site of loosening, due to lymphocyte sequestration phenomena consequent to a cell-mediated hypersensitivity reaction.

The effects of orthopaedic cements on osteoblastic cells cultured in vitro.

The authors have examined the response of osteoblastic cells in contact in vitro with different acrylic cements used in orthopaedic surgery. The solid and liquid components of the cements were mixed in sterile environments according to the instructions provided by the manufacturers, and placed in contact with the cells 1/2, 1, 4, 6 and 24 hours after triggering of polymerization. The experimental model, that tends to define the kinetics of the release of toxic substances from acrylic cements, allowed to verify 24 hours after polymerization that the three cements had lost their toxic effects, which instead had been present to varying degrees for the first 1-6 hours. Cytomorphological observations did not reveal cellular damage, which instead was very clear when functional tests were used.

[In vitro tissue factor production by human endothelial cells put in contact with polyethylene terephthalate with different texture].

The aim of this study is the evaluation of the in vitro tissue factor production by endothelial cells cultured in the presence of Woven or Knitted Dacron. The spectrophotometric evaluation of the protein content of the cultures and the both indirect assay and enzyme immune assay of tissue factor in cellular lysates were carried out after 72 hour contact between the cells and the materials under examination. The endothelial cell contact with Knitted Dacron did not determine variation in protein content, but it induced a significant increase in tissue factor production. The endothelial cell contact with Woven Dacron determined no significant variations neither in the protein content nor in the tissue factor concentration. It is concluded that Knitted Dacron, through the induction of tissue factor synthesis, can favour the extrinsic pathway of coagulation and therefore the production of thrombi. Woven Dacron, not inducing tissue factor formation, does not activate the extrinsic pathway of coagulation, as for the mechanism considered.
