Lower extremity scintigraphy: the foot and ankle.

Disorders of the ankle and foot are common and may pose a significant clinical problem. Acute injuries of the ankle and foot are routinely evaluated by radiographs and, if inconclusive, by magnetic resonance imaging (MRI) or computed tomagraphy (CT). In subacute or chronic injuries, where prolonged pain is unexplained, bone scintigraphy may play a significant role. The bone scan may differentiate soft tissue from bone pathology, and, being a sensitive test, it may indicate the region that needs further specific radiologic examination. It may also indicate the clinical significance of a radiologic finding. Moreover, in some clinical settings bone scintigraphy can be specific in the diagnosis. Careful attention to the technique enhances the efficiency of bone scintigraphy and SPECT allows better investigation of the hindfoot. The importance of ankle and foot injuries in the orthopedic practice and the better understanding of subacute and chronic injuries in athletes and in the general population has led to an increased use of specific tests such as MRI, CT, and bone scintigraphy. With improved technique and instrumentation it is no longer sufficient to indicate a focal abnormality in the ankle or foot on bone scintigraphy. More precise information about perfusion, blood pool, and specific location of a lesion can be obtained by high resolution and tomographic images. The role of bone scintigraphy in the diagnosis of such disorders will be presented with a focus on subacute traumatic injuries and related disorders, as well as its role in systemic inflammatory disorders that often involve the ankle and foot.

Quantitative SPET 99Tcm-DMSA uptake by the kidneys: age-related decline in healthy males.

To evaluate if 99Tcm-dimercaptosuccinic acid (99Tcm-DMSA) uptake by the kidneys is related to age and creatinine clearance in males with normal renal function, quantitative single photon emission tomography (SPET) of DMSA uptake by the kidneys was performed in 18 volunteers aged 20-79 years. The quantitative uptake of DMSA in the right kidney was 13.9 +/- 2.9% and in the left kidney 14.2 +/- 3.0%. There was no statistically significant difference between left and right kidney uptake (t = 1.2, N.S.). Global kidney uptake (right + left) was 28.1 +/- 5.9%. There was a statistically significant age-related decline in global DMSA uptake. The estimated DMSA uptake (%) was given by -0.27 x age + 42 (r = -0.87, P < 0.001). A good correlation was found between creatinine clearance and global DMSA uptake (r = 0.87, y = 2.8x + 28.3, P < 0.001). The results suggest that normal values of DMSA uptake by the kidneys are age-dependent.

Scintigraphy of posterior tibial tendinitis.

UNLABELLED: Our goal was to describe the typical scintigraphic pattern of posterior tibial tendinitis. METHODS: Bone scintigraphs were reviewed to study the scintigraphic characteristics of posterior tibial tendinitis in nine patients with posterior tibial tendinitis related to generalized rheumatic disease and in eight patients with isolated posterior tibial tendinitis. RESULTS: The scintigraphic pattern of posterior tibial tendinitis is elongated increased uptake in the blood flow and blood-pool phase along the anatomical course of the tibialis posterior tendon at the medial aspect of the ankle (malleolus region). Static images demonstrate increased focal abnormal uptake at the medial malleolus and in the navicular bone. CONCLUSION: Bone scintigraphy depicts a characteristic pattern of posterior tibial tendinitis. It is useful for the early diagnosis of idiopathic- or rheumatic-related posterior tibial tendinitis.

Increased renal function in kidneys with ureteropelvic junction obstruction: fact or artifact? Assessment by quantitative single photon emission computerized tomography of dimercapto-succinic acid uptake by the kidneys.

PURPOSE: We evaluated whether increased differential renal function on the pentetic acid (DTPA) renogram represents elevated renal function in obstructed kidneys. MATERIALS AND METHODS: In 10 patients with unilateral ureteropelvic junction obstruction and a differential function of 53% or greater on the DTPA renogram of the obstructed kidney we performed quantitative single photon emission computerized tomography of dimercapto-succinic acid uptake by the kidneys. RESULTS: The absolute uptake of dimercapto-succinic acid was significantly higher in the obstructed kidneys compared to the contralateral normal kidneys (30.0% +/- 7.8 versus 25.3% +/- 8.2, t = 3.6, p < 0.01, respectively). The elevated absolute uptake in the obstructed kidneys was due to an increased functional volume compared to the contralateral normal kidneys (126.0 +/- 69.7 cc versus 102.3 +/- 59.2 cc, t = 4.4, p < 0.01, respectively). CONCLUSIONS: The results suggest that increased relative renal function in hydronephrotic kidneys due to ureteropelvic junction obstruction is not an artifact of the DTPA renogram. It may represent a compensatory mechanism that sometimes may overcompensate resulting in a paradoxical hyperfunctioning kidney.

Detection of permanent damage in kidneys with vesicoureteral reflux by quantitative single photon emission computerized tomography (SPECT) uptake of 99mtechnetium labeled dimercaptosuccinic acid.

PURPOSE: We determined the level of functional damage to the kidney which inhibits further growth. MATERIALS AND METHODS: Renal functional volume and percent uptake of 34 kidneys with reflux were studied by performing sequential quantitative single photon emission computerized tomography 2 times at a mean interval of 3.2 years. RESULTS: Change in volume between the initial and repeat studies was 27.2 +/- 28.4% in all 34 kidneys. Change in volume was less than 7% in 8 kidneys and greater than 14% in 26. Receiver operating characteristic curve analysis indicated that 15.6% kidney uptake was associated with 75% sensitivity, 100% specificity and 94% accuracy in distinguishing kidneys with more than 7% from those with less than 7% change in volume. The value of kidney uptake of less than 15.6% was 100% in predicting less than 7% change in volume (positive predictive value) and the value of kidney uptake greater than 15.6% was 93% in predicting more than 7% change in volume (negative predictive value). CONCLUSIONS: Kidney uptake of less than 15.6% represents irreversible impairment of renal growth and indicates permanent damage in kidneys with vesicoureteral reflux.

Bone scintigraphy findings in Lisfranc joint injury.

Three-phase bone scintigraphy diagnosis of Lisfranc injury in a patient after foot trauma is discussed. Early diagnosis of Lisfranc joint injury is frequently missed and radionuclide bone scintigraphy may show a specific pattern where the x-rays are inconclusive.

Quantitative SPECT of technetium-99m-DMSA uptake in the kidneys of normal children and in kidneys with vesicoureteral reflux: detection of unilateral kidney disease.

UNLABELLED: Quantitative SPECT was used to evaluate renal functional volume (cc), percent of injected dose/cc (%ID/cc) and renal uptake (%) in 11 children with unilateral vesicoureteral reflux grade 3 or greater, and in 19 normal control children without reflux. METHODS: Studies were performed 4-6 hr after intravenous injection of 0.750-2 mCi of 99mTc-DMSA. RESULTS: Control kidneys (n = 38) had a volume of 99.7 +/- 29.5 cc. The %ID/cc was 0.27 +/- 0.08, and the uptake in one kidney was 24.8% +/- 3.9%. Global renal uptake (right plus left) was 49.6% +/- 7.3%. Functional volume of the control kidneys showed an increase with age, and the %ID/cc showed a steeper decrease with age, resulting in a trend of the kidney uptake to decrease with age. Kidneys with reflux had a decreased kidney uptake of 15.7% +/- 29.5%, compared to age- and sex-matched controls (t = 4.7, p < 0.001). The contralateral kidneys without reflux had a significantly increased total uptake of 33.4% +/- 6.8% as compared to controls (t = 3.44, p < 0.01). Global uptake by the kidneys was 49.2% +/- 8.6% and was not statistically different from controls (t = 1.0, ns). CONCLUSION: Our results suggest that SPECT quantitation of 99mTc-DMSA uptake in each kidney separately could be used as a noninvasive method to assess impairment and compensation of the function of the individual kidney in children with vesicoureteral reflux.

Imaging tumor hypoxia and tumor perfusion.

Tumor perfusion and oxygenation status have been suggested as factors which may influence treatment outcome in cancer patients. Nuclear medicine assays of tumor perfusion [99mTc-hexamethylpropylenamine oxime (HMPAO)] and tumor hypoxia [123I-iodoazomycin arabinoside (IAZA)] have recently been developed and described. We report on measurements of perfusion and oxygenation status of 27 tumors in 22 patients using these probes. An inverse correlation between tumor uptake of HMPAO and IAZA was measured (p < 0.05), with severe perfusion deficit usually associated with an increased uptake of the hypoxic marker. This trend was observed for limited stage small-cell lung carcinoma, squamous-cell carcinoma of the head and neck, soft-tissue sarcoma, brain metastases from small-cell lung carcinoma and adenocarcinoma of the prostate as a group, but not for glioblastoma multiforme. Whereas each imaging agent can yield information about the physiological status of tumor and normal tissue, the information resulting from their combined use could be important in cancer therapy.

Renal scintigraphy in initial evaluation of renal colic.

To determine the role of renal scintigraphy (RS) in patients with suspected acute urinary tract obstruction, a prospective study was performed comparing RS with emergency intravenous excretory urography (IVU) in 36 examinations. Thirty patients were diagnosed as having renal colic: 28 had dilation of the urinary tracts with (89%) or without (11%) visualized stone; 2 patients with stones did not have stasis at the IVU. An abnormal RS was found in 28 patients with abnormal IVU, while it was normal in the 2 nonobstructed patients (sensitivity = 93%). The radiopaque stones in these 2 patients were seen on plain x-ray film of the abdomen. The results of this study support the use of RS combined with a plain film of the abdomen in the initial evaluation of renal colic.

Ga-67 uptake unsuppressed by leukopenia and intense antibiotic therapy.

The authors describe a patient with myelofibrosis who showed intense Ga-67 uptake in spite of being severely leukopenic and receiving large amounts of antibiotics. They conclude that false-negative results associated with leukopenia or intensive antibiotic treatment may not always be correct.

Spectrum of scintigraphic appearance of liver hemangiomas.

Two hundred and twenty five liver hemangiomas in 166 patients were studied with Tc-99m labeled RBC and are the basis for this atlas. All hemangiomas showed various presentations of the perfusion blood pool mismatch, which is the basis for diagnosis. The size of the hemangiomas was the factor that determined the mixing of the Tc-99m RBC with the blood and, hence, the sequence of visualization of the lesion. Hemangiomas may appear as multiple lesions on liver scan and imitate metastases. Ninety-five hemangiomas appeared in 36 patients as multiple focal abnormalities. Hemangiomas are extremely variable in size. They may be huge (31 hemangiomas) and sometimes occupy most of the abdomen. Or they can be very small and are then detected only by SPECT (two cases). Thrombosed large hemangiomas appear as focal defects on Tc-99m RBC.
