Traumatic dehiscence of incisions seven years after radial keratotomy.

We studied the clinical and pathologic features of a traumatically ruptured eye seven years after radial keratotomy. The radial keratotomy incisions were identified and topographically related to perforation sites. Three radial keratotomy incisions were perforated. Seven additional incisions were partially separated. Radial keratotomy incision depths ranged from 50% to 90% in the remaining incisions. All incisions showed disruption of Bowman's membrane, 15 of 16 showed malapposition of Bowman's membrane, and 14 of 16 showed epithelial plugging. In this case the healing and strength of radial keratotomy incisions were suboptimal seven years after the procedure.

Sectorial corneal infiltrates and pannus in herpes zoster ophthalmicus.

We report four patients with herpes zoster ophthalmicus who developed a dense pannus limited to a sector of the superior cornea that advanced to the central region of the cornea. The pannus appeared to develop in response to peripheral infiltrates in a subepithelial or anterior stromal location and not in association with disciform corneal edema, interstitial keratitis, or scleritis. The corneal pannus developed insidiously as a late complication without associated ulceration and with little or no anterior uveitis or conjunctival reaction. Additionally, all four patients developed anterior and mid-stromal infiltrates in a punctate or incomplete ring configuration at the leading border of the pannus that threatened the visual axis. In the three patients that were treated with topical corticosteroids, the infiltrates resolved with preservation of visual acuity. The other patient developed central scarring with a permanent reduction in vision. Patients with infiltrates and pannus in a sector of the cornea should be followed carefully and treated with topical corticosteroids when infiltrates appear to prevent progression to the central cornea.

The complement system and host defense against Pseudomonas endophthalmitis.

The authors examined the role of the complement system in host defense against Pseudomonas aeruginosa endophthalmitis. Guinea pigs received intravitreal injections of P. aeruginosa, and comparisons were made between bacterial counts from the vitreous of control guinea pigs and experimental guinea pigs that underwent systemic decomplementation with cobra venom factor. In group 1 (intravitreal injection of 42 organisms), bacterial counts were significantly higher in the vitreous of decomplemented guinea pigs versus control guinea pigs at days 1, 2, and 3 after intravitreal injection but not at day 7 when complement levels returned to normal. In group 2 (intravitreal injection of 102 organisms), bacterial counts were significantly higher in the vitreous of experimental guinea pigs versus control guinea pigs only at day 1 with no statistically significant differences thereafter. In group 3 (intravitreal injection of 150 bacteria), there were no significant differences in bacterial counts in the vitreous of experimental versus control guinea pigs. Our results in group 1 suggest that partially decomplemented guinea pigs show impaired host defense to P. aeruginosa and that this defense is restored as complement levels return to normal. Intravitreal injection of higher numbers of P. aeruginosa as in a group 3 overwhelms and obscures any beneficial effect of the complement system on host defense.

Inhibition of contact lens-induced corneal neovascularization in radial keratotomized rabbit eyes.

Cellulose acetate butyrate contact lenses were fitted for extended wear on 28 rabbits 6 weeks after radial keratotomy. Fourteen rabbits received 0.03% flurbiprofen to one eye and vehicle solution to the contralateral control eye, while the other 14 received 1% prednisolone acetate to one eye and vehicle solution to the contralateral control eye four times each day in a double-blind fashion. The eyes were photographed and graded weekly with a standardized slit-lamp grading method. After 6 weeks, inhibition of corneal neovascularization was found with flurbiprofen (p = 0.001), while there was a trend toward inhibition of corneal neovascularization with prednisolone acetate (p = 0.076). We also found a significant vehicle effect, with hydroxypropyl methylcellulose, the more viscous vehicle, suppressing corneal neovascularization more than polyvinyl alcohol (p = 0.004).
