Mainly the Ender, Simon-Weidner nailing and the dynamic hip screw (DHS) of the AO compete along with other procedures for the treatment of proximal femur fractures by elderly individuals. The opportunity was taken to control the operative strain and postoperative development of 51 patients with Ender-nailing and 45 patients with DHS from the years 1985 to 1987. Prompt ability to walk and full load bearing capability is obtained by both procedures. Both procedures have slight strain due to operating time and operative trauma. Specific technical complications such as wandering of the nail and defective external rotational positioning were found mainly by the Ender-nailing. Reoperations were necessary in a few cases. The complication rate could be sunk further through the application of the DHS.
Pilon tibiale fractures with significant joint involvement (AO-classification B2/3 and C2/3) are considered one of the most unfavourable injuries of the lower extremity. It was possible for follow-up examinations to be performed on 50 patients with this injury pattern from the years 1984 to 1988. Primary plate osteosynthesis is primary blamed for causing a high infection rate. Remaining functional losses, uneven joints, defective positionings and early arthrosis are not avoidable with either a one step or a two step approach. The infection rate could be significantly lowered through a two step approach with primary stabilization through an external fixator, without having to accept further disadvantages. An early change of procedure in suitable cases between the second and third week guarantees an internal osteosynthesis with the best possible anatomic reposition of articular surface and axes. In chosen cases an end treatment is possible with an external fixator. The road is open for a primary arthrodesis of irreparably damaged articular surfaces.

The new AO-Universal-Nail was developed through intensive cooperation between fundamental research, technicians and clinical application. We are now able to transfer the concept of a Universal-Nail (initially for the femur) to the tibia as well. The existing technique of the medullary nail was maintained as far as possible. Special features to be highlighted: The new AO-bend obtains better impact behavior and a more anatomical stabilization of the bone cavity. The tapered tip eases the gliding of the tip of the nail along the inner wall of the dorsal corticalis and past the fracture. Proximal locking is obtained through three transverse extending holes. They offer many choices for optimal placement of the locking bolts. Interlocking from anterior to posterior has been logically abandoned, due to danger for nerves and vessels. Distal looking offers a large selection for stabilization through one a.p.-hole and two transverse holes for the locking bolts. The design from anterior to posterior entables the protection of the soft tissue. The Universal-nail can either be inserted with or without interlocking. A free decision is possible during the operation. The new AO-Universal-Nail for the tibia (as for the femur) maintained reliable aspects and was consequently converted to meet new demands and findings. The basic fundamentals of the AO, namely the observance and application of biomechanical principles and safe operation procedure were attained through simple instrumentation and have been proved successful for clinical application.

A long persisting chronic recurring ostitis disturbs above all the differentiated conversion of fibrous bone into strong lamellar bone. The more that other factors disturb the vitality and vascularity of the bone, the more dystrophy, sclerosis and so-called crenae develop. Together this leads to new pathological refractures during inflammatory bone processes. To avert these dangers adequate infection prophylaxis and therapy are necessary as well as consistent observance of biomechanical and biological principles.
MRT control examinations were carried out on 15 patients with allogenic bone-cartilage transplants in the knee. Spin-echo and gradient-echo sequences were obtained by using a flat surface coil. The results were compared with arthroscopic and operative findings. In 8 symptom-free patients, there was inclusion of the bone implant with narrow, but smooth, cartilage. 7 patients with recurrence of symptoms showed various complications such as oedema of the marrow, exudative synovitis, cartilage oedema, degenerative cartilagenous changes with regeneration of fibro-cartilage and localized cartilage defects. Spin-echo sequences were useful for showing bone, whereas gradient-echo sequences proved better for showing changes in the cartilage. MRT can be recommended as the method of choice for the non-invasive control of bone-cartilage transplants.
Between 1980 and 1986 16 male patients of between 30 and 65 years of age were treated operatively because of distal biceps tendon ruptures. The operative procedures are reported and the results of a follow-up examination two years later are communicated. The good results of the applied operative treatment allow to recommend the operative procedure. A special method of measurement of muscle power is presented.

The replacement of a loose total hip prosthesis requires a great amount of surgical skill and experience. Severe bony defects in the acetabulum should be filled by homogeneous bone grafts. Implantation of a Schneiderring together with the graft gives sufficient stability. The new shaft should be cemented very carefully, avoiding cavities between bone, cement and stem. The shaft of the femur should be reinforced by a long bridging plate.
