Experimental laser phototherapy of the Morris 7777 hepatoma in the rat.

The tumoricidal effect of the activation of hematoporphyrin derivative (HpD), by an argon-ion-dye-laser (wavelength 630 nm), was investigated in the Buffalo rat bearing subcutaneous implants of the Morris 7777 hepatoma. Tumor growth was monitored by measuring the tumor volume with constant force calipers. In control animals and those that were pretreated with HpD alone (10 or 20 mg per kg by i.p. injection) or laser light alone (2,000 J at 100 mW), a predictable exponential growth pattern of the cancer was observed. Animals were pretreated with HpD (10 mg per kg by i.p. injection) 48 hr prior to the fiberoptic, intratumor delivery of laser radiation (2,000 J at 100 mW), when the tumor had reached a volume greater than 1.5 cm3. Forty-eight hours after combined laser and HpD treatment, the hepatoma underwent coagulation necrosis, and the tumor volume rapidly increased from a mean value of 1.8 +/- 0.7 cm3 S.D. to a value of 5.8 +/- 1.5 cm3 S.D., compared with a mean of 3.7 +/- 0.7 cm3 S.D. in animals who had not received laser phototherapy. Rats treated with HpD and laser light survived longer (mean survival time 48 +/- 12 days S.D.) than did the other animals treated with the laser alone or HpD alone (mean survival time 31 +/- 16.5 S.D.). Tissue biodistribution studies with tritiated hematoporphyrin derivative, given with doses of 10 and 20 mg per kg of HpD, showed higher concentrations of the dye in the liver, kidneys and spleen than in the tumor at 48 hr after administration of the dye.(ABSTRACT TRUNCATED AT 250 WORDS)

Identity, cognitive structure, and long-term tranquillizer use: a multidimensional approach.

The article discusses different theoretical perspectives that are relevant in analysing long-term tranquillizer use. The theories mentioned are, e.g., behavioral theory models, cognitive models, humanistic and existential models, transpersonal identity models, psychodynamic reasoning, sociological and anthropological perspectives. A multidimensional model focusing on the connection between identity structure and long-term tranquillizer use is presented. The article argues for the use of mixed methodological strategies, e.g., quantitative and qualitative methods. The implications of the 'actor-spectator paradox" for the analysis of data are also discussed in some detail.

Evaluation of programs for the treatment of benzodiazepine dependency.

Programs for the treatment of benzodiazepine dependency differ in a number of ways. Their outcome goals, for example, vary. In some of the programs the reduction or the elimination of benzodiazepine use is the main objective. In others a reduction in anxiety and depression among patients is regarded as much more important than decreasing benzodiazepine use. These differences in goals make comparative evaluations difficult. Further, the type of clients recruited to the programs are often dissimilar. However, it is still possible and valuable to make comparative evaluations of the numerous programs. The aims of published evaluations differ. Most evaluations are directed towards outcome data. Only in a few studies has the aim been to analyze the dynamic treatment process. To achieve more reliable and valid evaluations it is necessary to get data which describe the process and the results from both the clients' and the therapists' perspectives and combining different research strategies.

Toward an integrative approach in the analysis of dependency problems.

This article takes some preliminary steps towards an integrated analysis of dependency problems e.g., long-term tranquillizer use, alcohol dependence, problematic use of narcotics. It argues for the need to outline important theoretical, epistemological, and methodological prerequisites in the analysis of the complex dynamic developmental processes involved in dependency problems. The dynamic process leading to dependence can be studied by the aid of an artificial science neural network approach in combination with a mixed method strategy including a clarification of a combination of different epistemological positions. It is intended that the empirical output of this complex strategy will provide a starting point for a new theoretical analysis which, in turn, will lead to new and more relevant input variables in the neural network approach that will help us to extend our knowledge of the dynamic processes leading to dependency.

Intrathecal vincristine: an analysis of reasons for recurrent fatal chemotherapeutic error with recommendations for prevention.

PURPOSE: Accidental intrathecal vincristine instillation is usually a fatal error. The authors report an analysis of a patient and suggest means with which to reduce such errors. PATIENTS AND METHODS: A 7-year-old girl with recurrent acute lymphoblastic leukemia was inadvertently injected intrathecally with 1.5 mg vincristine. A detailed analysis of the events leading to this error and a review of all reported cases in the English literature were undertaken. RESULTS: Reasons for errors reported by us and other institutions included mistaking vincristine for an intended intrathecal drug, assuming vincristine was an additional drug to be injected, not checking physician orders, mistaken route of administration, and mislabeling of syringes. CONCLUSION: Intrathecal injection of vincristine may be the end-result of a series of systems errors. Protocol recommendations to reduce the likelihood of this error are presented.

How effective are UV opaque face shields in UVB phototherapy cabins?

A few hours after servicing illuminated lamps inside a whole body ultraviolet radiation B (UVB) phototherapy cabin, a technician developed erythema to the anterior neck with subsequent peeling, despite wearing a UV-opaque face shield. Measurements using polysulphone film badges attached to various sites on the head and neck of a mannikin were carried out to explore the spatial distribution of UVB exposure. It was found that the lower face and neck can receive sufficient exposure to result in erythema, the reason being that the fluorescent lamps will extend to about 1.5 m inferior to the head of an upright person and so result in irradiation from below. It is important, therefore, that if operators or service personnel need to be in an illuminated UVB cabin for several minutes or more, adequate protection should be provided to the chin and neck, in addition to a UV-opaque face shield.

Research implications for alternative health therapies.

Nontraditional health practices have gained in popularity during the past 10-15 years. Unfortunately, the effectiveness of many nontraditional approaches have been inadequately investigated. Furthermore, tradition research protocols often have fallen short in providing satisfactory strategies for these types of investigations because the nature of the personal belief system between patient and provider has not been examined. The authors discuss the implications for triangulation techniques in nursing research related to nontraditional treatments. Six types of triangulation are defined and methods for utilizing these techniques are explained.

In vitro assessment of vancomycin HCl compatibility after coinfusion with a specialized amino acid formulation.

Vancomycin usage at British Columbia's Children's Hospital has increased substantially in the Special Care Nursery as a consequence of a study demonstrating a reduced morbidity and mortality in neonates with necrotizing enterocolitis when treated with vancomycin and cefotaxime. The inability to place more than one peripheral intravenous access necessitates interruption of parenteral nutrition to infuse vancomycin, resulting in a reduction of the planned daily intake of these neonates. This is clinically significant with the administration of vancomycin because of the long administration period required for this drug (60 minutes). This study was designed to assess the physical and chemical stability of vancomycin with a standard neonatal parenteral nutrition solution, Vamin A, when coadministered through the same intravenous line. To simulate the actual clinical setting, the dose of vancomycin and the infusion rate of Vamin A were chosen to represent those commonly used in a 1-kg neonate. Physical compatibility was assessed using effluent obtained after coinfusion of vancomycin with parenteral nutrition solution. Duplicate samples were visually checked for color changes and precipitate. High-pressure liquid chromatography (HPLC) and pH testing were used to assess chemical compatibility of vancomycin. The results of physical compatibility revealed no color change or precipitate. No changes in pH were observed. HPLC determination confirmed that there were no significant time-dependent changes in vancomycin stability. The samples were studied over 24 hours to determine the rate of degradation of vancomycin, if any, under various temperature conditions. The concentrations were not significantly different from each other at the different temperatures studied. Interference screw fixation strength of a quadrupled hamstring tendon graft is directly related to bone mineral density and insertion torque.

The purpose of this study was to determine whether bone mineral density of the host bone, measured using conventional dual photon absorptiometry techniques, and insertion torque can predict part of the ultimate failure strength of interference screw fixation of quadrupled hamstring tendon grafts. The semitendinosus and gracilis tendons were harvested from 10 human cadaveric knees, mean age 66.5 years (range, 53 to 81). The bone tunnel was sized within 0.5 mm of the graft. The graft was fixed with a biodegradable screw (7 x 25 mm for the femur, and 9 x 25 mm for the tibia) directly against the tendon and at the joint surfaces. Tibial fixation and femoral fixation were tested to failure using a materials testing system. Bone mineral density was measured in the metaphyseal region of the tibia and femur. The results of multiple regression analyses showed that both insertion torque and bone mineral density were related to the maximum load the graft withstood. These two variables explained 77.1% of the maximum load observed. We concluded that bone mineral density measurements of the host bone site are an important determinant of postoperative graft strength and thus have an important, but previously unrecognized, clinical role in establishing individual postsurgery rehabilitation protocols. Insertion torque in this study was a useful predictor of graft fixation strength.

Modulation of CD99/MIC2 expression of human AHTO-7 osteoblasts by carcinoma cell line-conditioned media.

BACKGROUND: Prostate cancer metastases induce a predominantly osteoblastic response in bone tissue, resulting in new bone formation and associated morbidity; however, the mechanisms of these tumor-host responses are not fully understood. MATERIALS AND METHODS: Supernatants of prostate (PC3, DU145, LNCaP), breast (BT20, ZR-75-1), colon (SW620, Colo 320DM), pancreatic (ASPC1, Capan-1), renal cell (ACHN) and hepatoma (HepG2) cell lines were tested for their capacity to modulate proliferation, activity of alkaline phosphatase (ALP) and CD99/MIC2 expression in AHTO-7 (large T antigen transfected human trabecular osteoblasts) cells in vitro. RESULTS: Osteoblastic stimulation is not restricted to prostate cancer derived conditioned media CM and high activity is found in CM from Capan-1, HepG2 and ACHN lines. Furthermore these CM down-regulate the expression of the CD99/MIC2 antigen in comparison to medium by AHTO-7 cells as detected by HBA-71 immunofluorescence, with the exception of prostate cancer-derived CM. Induction of the differentiation marker ALP was detected in response to CM derived from Capan-1, BT-20 and Colo320DM. Stimulation of the proliferation of AHTO-7 cells (105-138% of control), induction of ALP (1.17-5.29-fold) and down-regulation of CD99 (13.6-57.5%) exhibited no correlation. CM derived from PC3 and LNCaP metastatic prostate cancer cell lines specifically resulted in the retention/stimulation of the expression of CD99/MIC2 in AHTO-7 cells in contrast to all other cell lines tested. CONCLUSION: The CD99/MIC2 antigen, which is expressed on human osteoblasts and osteosarcoma cells, seems to constitute a new and independent response marker of osteoblasts in the triggering of osteoblastic reaction by prostate cancer cells.
