[Evaluating effects of some medicine on tinnitus with animal behavioral model in rats].

OBJECTIVE: To develop a valid behavioral animal model of tinnitus and to determine the animals whether have the perception of tinnitus and to evaluate effects of nimodipine and Chinese herbs on tinnitus using the behavioral model. METHODS: Adult male albino rats (n = 60) were fell evenly and randomly into ten groups. Animals in Group I and II were administrated sodium salicylate 350 mg/kg/d (i.p.). Instead, animals in control groups (Group III and IV) were saline injected. Pure tone (8 kHz, 65 dBSPL) was given to rats in Group V and VI instead of sodium salicylate. Nimodipine (1 mg/kg) were administrated to animals in Group VII and VIII. Gushenshuerfang, one kind of Chinese herbs were administrated to animals in Group IX and X. Medicine or pure tone were being given to all animals in Group I, III, V, VII, IX before conditioned reflex. Animals in Group II, IV, VI, VIII, X began to have medicine or pure tone after conditioned reflex. Thirsty animals (deprived of water for several days) were breeded in sound-proofed room and listened a persistent white noise (55 dB SPL) and trained daily with our instrument system, according to the procedure based on Pavlovian conditioned suppression paradigm. Subjects were permitted to lick water at an automatic dispenser during daily training sessions and the number of licks was recorded automatically. Noise offset (silence) was the conditioned stimulus and the electrical shock was the unconditioned stimulus. After two or three days training, subjects learnt rapidly to associate noise offset with shock and then established a behavioral conditioned reflex. Because of shock fear they decrease or even stop licking with the presentations of conditioned stimulus. However, subjects gradually forgot or ignored the fear without any shock present in extinction test sessions. RESULTS: Animals' licking recovered to their pre-training levels respectively at the following days: Group I--5 days, Group II--2 days, both Group III and IV--4 days, Group V--5 days, Group VI--2 days, Group VII--5 days, Group VIII--4 days, Group IX--5 days, Group X--4 days. There were much significant differences among all groups and all training days (F test, P < 0.001). Above results provided sufficient evidence for that tinnitus was induced by salicylate and tinnitus presented with noise offset. In group I, tinnitus became additional conditioned stimulus. Therefore, the fear of animals was much severe and the numbers of licking were much fewer. Extinction lasted longer. In group II, animals ignored the noise offset much easily and the extinction lasted much shorter. Moreover, results from group V and VI, pure tone instead of salicylate, indicated that sensation (tinnitus) sounded like pure tone. The groups VII, VIII, IX, X demonstrated that nimodipine and Gushenshuerfang, one kind of Chinese herbs can cure the tinnitus induced by salicylate. CONCLUSION: All behavioral evidence indicated that animals perceived tinnitus induced by salicylate and this animal model could be used in evaluating curative effects of some medicine on tinnitus.

[Mitochondrial DNA4977 deletions associated with human presbycusis].

OBJECTIVE: To determine whether or not the mtDNA4977 deletions contribute to human aging and involve in the development of presbycusis. METHODS: 67 sides of archival temporal bone sections, 20 temporal brains, 21 cochlear nucleus, 20 hearts and 22 livers were harvested and the total DNA was extracted. The presence of mtDNA4977 deletions was examined using nest polymerase chain reaction(PCR). RESULTS: Our results showed: (1) the highly conserved mitochondrially-encoded tRNA and ND1 segments were amplified from all tissues, as well as mtDNA4977 deletions related to aging. (2) The incidence of carrying mtDNA4977 deletions in all tissues in aged group was significant higher than that of young control group (P < 0.05). (3) The incidence of carrying mtDNA4977 deletions in temporal bone sections and cochlear nucleus with presbycusis patients was significant higher than that of aged normal hearing control group (P < 0.05). However, the incidence of carrying mtDNA4977 deletions in heart and liver were no significant difference between presbycusis patients and the control group. CONCLUSION: These findings indicated that mtDNA4977 deletions in human contribute to presbycusis, as well as aging.

[Supracricoid subtotal laryngectomy for the treatment of advanced laryngeal cancers].

OBJECTIVE: This procedure was designed to eradicate the advanced laryngeal carcinomas while preserving the laryngeal physiologic function. The indications and contraindications for the operation were also discussed and evaluated in this paper. METHODS: Twenty-one patients with advanced laryngeal cancers underwent this procedure during the period from 1988 through 1996 in our department. All of the patients were not amenable to conventional partial horizontal and vertical laryngectomies. In those patients, there were 9 supraglottic cancers, 10 glottic cancers and 2 transglottic cancers. Among them, 16 cases had stage T2 lesions and 5 cases had stage T3 lesions. In this procedure, the hyoid and thyroid cartilages were excised in toto, along with both true and false vocal cords, paraglottic space as well as the partial or total epiglottis and the whole preepiglottic space. The cricoid cartilage and at least one arytenoid cartilage were spared. The new larynx was reconstructed by suturing the cricoid and/or epiglottis to the base of the tongue. RESULTS: Of 21 patients, 95% were able to eat normally without aspiration, 90% were decannulated with near normal speech function. In two patients, the tracheotomy tube was reserved, one developed a stenosis, and another had recurrent aspiration pneumonia. No patients experienced local recurrence. Nineteen patients were followed up for at least 3 years, 13 patients achieved 5 years tumor-free survival. CONCLUSION: The term supracricoid subtotal laryngectomy best describes the surgical approach and the reconstruction model of the new larynx. As a useful alternative to total laryngectomy in selected advanced laryngeal cancers, this procedure may greatly improve the quality of life and prolong the life without recurrence.

[Thiol reagent thimerosal-induced Ca2+ mobilization in isolated guinea pig cochlear outer hair cells].

OBJECTIVE: To understand mechanism of cochlear outer hair cells (OHCs) intracellular Ca2+ mobilization further. METHODS: Intracellular calcium of isolated guinea pig was investigated using thimerosal, a--SH group oxidizing agent, and fura-2 fluorescence ratio imaging microscopy. RESULTS: In the presence of thimerosal, intracellular Ca2+ concentrations ([Ca2+]i) of OHCs were elevated in a dose-dependent manner. Even in Ca(2+)-free medium, Ca2+ response was still induced. The effects of thimerosal on [Ca2+]i were completely blocked and reversed by (DTT). Neither 1-100 mumol/L ryanodine nor 5-20 mmol/L caffeine altered the effects of thimerosal. Pretreatment with pertussis toxin (PTX) for 30 min did not affect the thimerosal-induced increase in [Ca2+]i The increase in [Ca2+]i when Ca2+ was added during thimerosal application in Ca(2+)-free medium was almost completely blocked by 500 mol/L LaCl3, while nifedipine did not inhibit further increase in [Ca2+]i caused by thimerosal. CONCLUSION: Oxidation of the -SH group of the OHC membrane can induce a Ca2+ release from intracellular Ca2+ stores, which are ryanodine- and caffeine-insensitive, and Ca2+ influx through non-specific Ca2+ channels, but not the nifedipine-sensitive Ca2+ Channels. The possible oxidation of--SH group gated Ca2+ channels in OHCs are worthy of further study.

[Value of preservation of uvula in uvulopalatopharyngoplasty].

OBJECTIVE: To explore the effect of the modified Uvulopalatopharyngoplasty (UPPP) where uvula is preserved in the operation, and whether the postoperative complications are diminished. METHODS: Thirty consecutive patients with the obstructive sleep apnea syndrome (29 men, 1 woman) were analyzed by questionnaire, polysomnography and oral cavity measurement before and 6 months after uvulopalatopharyngoplasty to assess the surgical outcome and value of preservation of uvula in the operation. RESULTS: Eighty-seven percent of patients reported improvement of snoring and daytime sleepiness. Fifty-three percent of patients showed a decrease of at least 50% in the apnea and hypopnea indices. Among the mild and moderate obstructive sleep apnea (AHI < 50), 73.3% of them showed a decrease of at least 50% in the AHI. No velopalatal insufficiency occurred. The result of oral cavity measurement showed that the uvula, which was preserved completely during the operation, began to contract 2 weeks after the operation. Three to six month after operation, uvula contracted to the normal length and the anatomical shape of the pharyngeal cavity became normal. CONCLUSIONS: The results of the uvula preserving UPPP indicate that this approach not only improves the effect of the UPPP, but also avoids the postoperative complications. The uvula that preserved completely during the operation will contract to the normal length progressively.

Peroxynitrite, the breakdown product of nitric oxide, is beneficial in blood cardioplegia but injurious in crystalloid cardioplegia.

BACKGROUND: Peroxynitrite (ONOO(-)) has been implicated as a primary mediator in the deleterious effects of nitric oxide (NO) in crystalloid solutions, possibly due to a lack of detoxification mechanisms, leading to the formation of.OH. In contrast, ONOO(-) may exert cardioprotective effects in blood environments secondary to detoxification and the subsequent formation of NO-donating nitrosothiols. This dichotomy in physiological effects of ONOO(-) may exist between crystalloid and blood cardioplegia (BCP) environments. In the present study, we tested the hypothesis that ONOO(-) is cardiotoxic in crystalloid cardioplegia but cardioprotective in BCP in ischemically injured hearts. METHODS AND RESULTS: In anesthetized dogs on cardiopulmonary bypass, global 37 degrees C ischemia was imposed for 30 minutes, followed by 60 minutes of intermittent 4 degrees C hyperkalemic crystalloid (Plegisol) or BCP with (+) or without (-) 5 micromol/L authentic ONOO(-). After 2 hours of reperfusion, left ventricular (LV) function (end-systolic pressure-volume relations, in percent of baseline) was 56+/-3% in Plegisol-, which was further reduced in Plegisol+ to 40+/-4%.* In contrast, postischemic systolic function was better in BCP+ groups than in BCP- groups (96+/-2%* versus 82+/-2%, respectively). Differences in functional recovery could not be attributed to differences in hemodynamics. LV end-diastolic stiffness was significantly increased with the addition of ONOO(-) in both Plegisol (298+/-26% versus 466+/-30%*) and BCP (201+/-22% versus 267+/-13%*) groups. Consistent with increased LV chamber stiffness, myocardial edema was increased in BCP+ compared with BCP- (78.9+/-0.3% versus 76.4+/-0.3%*) and in Plegisol+ compared with Plegisol- (81.1+/-0.3% versus 79.6+/-0.4%*). Creatine kinase activity was significantly increased in Plegisol+ (48+/-6) compared with that in Plegisol- (31+/-6) but was unchanged in BCP- (14+/-2) relative to BCP+ (18+/-1). Nitrotyrosine (ng/mg protein) accumulation in LV myocardial biopsy samples confirmed myocardial exposure to ONOO(-) or its metabolites (Plegisol- 1.2+/-0.1, Plegisol+ 3.31+/-0.3*, BCP- 1.4+/-0.2, BCP+ 2.9+/-0.2*). CONCLUSIONS: We conclude that (1) the postcardioplegic cardiodynamic effects of ONOO(-) depend on its environment and (2) ONOO(-) in crystalloid solution impairs postcardioplegia systolic and diastolic functional recovery, whereas (3) ONOO(-) in BCP increases functional recovery. This environment-dependent dichotomy in the effect of ONOO(-) may affect the benefits of NO-related adjuncts to crystalloid or BCP solutions (*P<0.05 versus group without ONOO(-)). :II-384-II-391.)

Protective effects of idebenone and alpha-tocopherol on beta-amyloid-(1-42)-induced learning and memory deficits in rats: implication of oxidative stress in beta-amyloid-induced neurotoxicity in vivo.

Amyloid beta-peptide (A beta), the major constituent of the senile plaques in the brains of patients with Alzheimer's disease, is cytotoxic to neurons and has a central role in the pathogenesis of the disease. Previous studies have suggested that oxidative stress is involved in the mechanisms of A beta-induced neurotoxicity in vitro. In the present study, we examined whether oxidative stress contributes to learning and memory deficits caused by continuous intracerebroventricular infusion of A beta-(1-42). In the A beta-(1-42)-infused rats, spontaneous alternation behaviour in a Y-maze and spatial memory in a water maze task were significantly impaired, as compared with A beta-(40-1)-infused control rats. The retention of passive avoidance learning was also significantly impaired by treatment with A beta-(1-42). Potent antioxidants idebenone and alpha-tocopherol prevented the behavioural deficits in Y-maze and water maze, but not passive avoidance, tasks in A beta-(1-42)-infused rats when they were repeatedly administered by mouth once a day from 3 days before the start of A beta infusion to the end of behavioural experiments. Lipid peroxide levels in the hippocampus and cerebral cortex of A beta-(1-42)-infused rats did not differ from those in control animals, and neither idebenone nor alpha-tocopherol affected the lipid peroxide levels. These results suggest that treatment with antioxidants such as idebenone and alpha-tocopherol prevents learning and memory deficits caused by A beta.

Mutual regulation between the intercellular messengers nitric oxide and brain-derived neurotrophic factor in rodent neocortical neurons.

The diffusible factors, nitric oxide (NO) and brain-derived neurotrophic factor (BDNF) are both suggested to be intercellular messengers that have similar synaptic activities and developmental influences in the brain. In the present study, we have analysed their mutual regulation with respect to their production in rodent neocortical neurons. Some of the cultured rat neocortical neurons exhibited immunoreactivity for both neuronal NO synthase (NOS) and the BDNF receptor trkB. Neuronal NOS appeared to be activated autonomously and produced NO in culture as monitored by nitrite accumulation. Inhibition of the endogenous NO production in culture by a NOS inhibitor, NG-monomethyl-L-arginine (NMMA), enhanced basal expression of BDNF mRNA and protein. Similarly, cerebroventricular administration of another NOS inhibitor, N-omega-nitro-L-arginine methylester (L-NAME), but not D-NAME or saline, increased BDNF content in the neocortex. In the opposite direction, however, BDNF appeared to function as a positive regulator for NO synthesis. Addition of BDNF upregulated the neuronal NOS expression as well as NO production in neocortical culture. In agreement, BDNF knock-out mice exhibited significant impairment of neuronal NOS expression in the neocortex. Taken together, these observations suggest that the trans-synaptic signalling molecules, NO and BDNF, influence the production of each other and mutually regulate the strength of their intercellular communications.

Wigner rotations and Iwasawa decompositions in polarization optics.

Wigner rotations and Iwasawa decompositions are manifestations of the internal space-time symmetries of massive and massless particles, respectively. It is shown to be possible to produce combinations of optical filters which exhibit transformations corresponding to Wigner rotations and Iwasawa decompositions. This is possible because the combined effects of rotation, phase-shift, and attenuation filters lead to transformation matrices of the six-parameter Lorentz group applicable to Jones vectors and Stokes parameters for polarized light waves. The symmetry transformations in special relativity lead to a set of experiments which can be performed in optics laboratories.

Regulation of nerve growth factor release by nitric oxide through cyclic GMP pathway in cortical glial cells.

In the present study, we found that S-nitroso-N-acetyl-DL-penicillamine, a spontaneous nitric oxide (NO) generator, dose-dependently inhibited basal nerve growth factor (NGF) release from mixed glial cells. To elucidate the function of endogenous NO in the regulation of NGF release, the mixed glial cells were stimulated with lipopolysaccharide (LPS) or LPS plus interferon-gamma (IFNgamma). The results showed that LPS alone induced NGF release and moderate NO production. However, costimulation with LPS plus IFNgamma greatly enhanced NO production but significantly suppressed LPS-induced NGF release. When N(G)-monomethyl-L-arginine, an NOS inhibitor, was added to the culture, the suppression of NGF release by IFNgamma was significantly reduced. Quantitative reverse transcription-polymerase chain reaction demonstrated S-nitroso-N-acetyl-DL-penicillamine was also able to inhibit the LPS-induced NGF mRNA expression. To understand the different contributions of astroglia and microglia to this phenomenon, both cell types were purified. We found purified astroglia produced high amounts of NGF but low amounts of NO. However, purified microglia produced a large amount of NO but very low amounts of NGF after stimulation with LPS or LPS plus IFNgamma. Our data also indicated the second messenger cyclic GMP, but not cyclic AMP, was able to inhibit basal NGF release. In vivo experiments confirmed that NGF protein level was significantly enhanced in rats treated with L-N(omega)-nitro-arginine methyl ester and in endothelial NO synthase mutant mice. Taken together, we conclude NO derived mainly from microglia down-regulates NGF release from astroglia at the transcriptional level by stimulating cyclic GMP pathway.

Long-term survival and function of intrahepatic islet allografts in baboons treated with humanized anti-CD154.

Clinical islet cell transplantation has resulted in insulin independence in a limited number of cases. Rejection, recurrence of autoimmunity, and impairment of normal islet function by conventional immunosuppressive drugs, e.g., steroids, tacrolimus, and cyclosporin A, may all contribute to islet allograft loss. Furthermore, intraportal infusion of allogeneic islets results in the activation of intrahepatic macrophages and endothelial cells, followed by production of proinflammatory mediators that can contribute to islet primary nonfunction. We reasoned that the beneficial effects of anti-CD154 treatment on autoimmunity, alloreactivity, and proinflammatory events mediated by macrophages and endothelial cells made it an ideal agent for the prevention of islet allograft failure. In this study, a nonhuman primate model (Papio hamadryas) was used to assess the effect of humanized anti-CD154 (hu5c8) on allogeneic islet engraftment and function. Nonimmunosuppressed and tacrolimus-treated recipients were insulin independent posttransplant, but rejected their islet allografts in 8 days. Engraftment and insulin independence were achieved in seven of seven baboon recipients of anti-CD154 induction therapy administered on days -1, 3, and 10 relative to the islet transplant. Three of three baboons treated with 20 mg/kg anti-CD154 induction therapy experienced delayed rejection episodes, first detected by elevations in postprandial blood glucose levels, on postoperative day (POD) 31 for one and on POD 58 for the other two. Re-treatment with three doses of anti-CD154 resulted in reversal of rejection in all three animals and in a return to normoglycemia and insulin independence in two of three baboons. It was possible to reverse multiple episodes of rejection with this approach. A loss of functional islet mass, as detected by reduced first-phase insulin release in response to intravenous glucose tolerance testing, was observed after each episode of rejection. One of two baboons treated with 10 mg/kg induction therapy became insulin independent post-transplant but rejected the islet graft on POD 10; the other animal experienced a reversible rejection episode on POD 58 and remained insulin independent and normoglycemic until POD 264. Two additional baboon recipients of allogeneic islets and donor bone marrow (infused on PODs 5 and 11) were treated with induction therapy (PODs -1, 3, 10), followed by initiation of monthly maintenance therapy (for a period of 6 months) on POD 28. Rejection-free graft survival and insulin independence was maintained for 114 and 238 days, with preservation of functional islet mass observed in the absence of rejection. Prevention and reversal of rejection, in the absence of the deleterious effects associated with the use of conventional immunosuppressive drugs, make anti-CD154 a unique agent for further study in islet cell transplantation.

Parental occupational exposure to hydrocarbons and risk of acute lymphocytic leukemia in offspring.

Parental exposure to hydrocarbons at work has been suggested to increase the risk of childhood leukemia. Evidence, however, is not entirely consistent. Very few studies have evaluated the potential parental occupational hazards by exposure time windows. The Children's Cancer Group recently completed a large-scale case-control study involving 1842 acute lymphocytic leukemia (ALL) cases and 1986 matched controls. The study examined the association of self-reported occupational exposure to various hydrocarbons among parents with risk of childhood ALL by exposure time window, immunophenotype of ALL, and age at diagnosis. We found that maternal exposure to solvents [odds ratio (OR), 1.8; 95% confidence interval (CI), 1.3-2.5] and paints or thinners (OR, 1.6; 95% CI, 1.2-2.2) during the preconception period (OR, 1.6; 95% CI, 1.1-2.3) and during pregnancy (OR, 1.7; 95% CI, 1.2-2.3) and to plastic materials during the postnatal period (OR, 2.2; 95% CI, 1.0-4.7) were related to an increased risk of childhood ALL. A positive association between ALL and paternal exposure to plastic materials during the preconception period was also found (OR, 1.4; 95% CI, 1.0-1.9). The ALL risk associated with parental exposures to hydrocarbons did not vary greatly with immunophenotype of ALL. These results suggest that the effect of parental occupational exposure to hydrocarbons on offspring may depend on the type of hydrocarbon and the timing of the exposure.

[Evaluation of effectiveness of hepatitis B plasma derived vaccine and a survey of hepatitis C virus infection in preschool children in Chengdu, Sichuan].

This study was undertaken to evaluate the effectiveness of hepatitis B plasma-derived vaccine and investigate the character of hepatitis C virus infection in preschool children. The anti-HBs, HBsAg and anti-HCV of 985 plasma samples were examined by ELISA. The results showed that the positive rate of HBsAg in the children (aged < 7 yr) in the communities was 0.89% and the effectiveness of the vaccine was 91.38%. The positive rate of anti-HBs decreased with the increase in age; the positive rate of HBsAg did not increase with age. The positive rate of HBsAg was as high as 4.69% in the children from hospitals, so supervision and enhanced vaccination should be administered properly in this group. The positiverate of anti-HCV was 1.18% in the communities, but 3.17% in hospitals. It is worthy to investigate how children get infected with HCV.

[Apoptosis in parathyroid of uremic patients with secondary hyperparathyroidism and the influence of Ca2+ and calcitriol on APO].

OBJECTIVE: To examine whether the alteration of apoptosis is pre sent in parathyroid (PT) tissue of hyperparathyroidism (HPTH). METHODS: DNA strand breaks were sought by TdT-mediated dUTP nick end-labeling assay (TUNEL) and DNA gel electrophoresis in 4 specimens of normal PT tissue, 7 of primary HPTH (primary HPTH) and 9 of secondary HPTH tissue (secondary HPTH). RESULTS: The data showed that the percentage of apoptotic cells was (1.3 +/- 0.4) % in normal PT, (3.9 +/- 0.7) % in primary HPTH and (5.6 +/- 0.8) % in secondary HPTH (P < 0.04), primary HPTH vs normal PT; P < 0.001, secondary HPTH vs normal PT respectively. There was no significant difference in the apoptotic incidence between primary and secondary HPTH. In vitro study, a greater amount of apoptotic TUNEL-positive cells was found 24 hours after exposure to high Ca(2+) (1.9 mmol/L) than to low Ca(2+) (0.5 mmol/L) concentration and 96 hours after treatment with high calcitriol (10(-7)mol/L) than with low calcitriol (10(-11) mol/L) concentration. However, no DNA ladder feature was observed in electrophoresis of DNA extracted from cultured parathyroid cells of HPTH. CONCLUSION: It was shown that apoptosis did not decrease, but actually enhanced, in hyperplastic parathyroid tissue of patients with primary or secondary HPTH and that a high extracellular concentration of either Ca(2+) or calcitriol was capable of enhancing apoptosis in human parathyroid cells of secondary HPTH complicating chronic renal failure.
