T-cell clonality determination using polymerase chain reaction (PCR) amplification of the T-cell receptor gamma-chain gene and capillary electrophoresis of fluorescently labeled PCR products.

We compared the effectiveness of polymerase chain reaction (PCR) and DNA blot analysis (DBA) for detecting clonal T-cell populations and investigated whether a nonradioactive PCR method could be used in routine clinical diagnosis. We analyzed DNA from 117 cases for T-cell clonality by PCR amplification. DBA was performed on 77 of these cases. Denaturing polyacrylamide gel electrophoresis (PCR-PAGE) of radiolabeled PCR products and capillary electrophoresis (PCR-CE) of fluorescently labeled PCR products were used for PCR product separation and quantitation. Complete agreement was obtained between PCR-PAGE and DBA in 67 of 77 cases. One case was positive by DBA and negative by PCR-PAGE, and 3 cases were positive by PAGE and negative by DBA. Five cases indeterminate by DBA were positive by PCR-PAGE, and 1 indeterminate case was negative by PCR-PAGE. In the comparison of PCR-PAGE and PCR-CE, of 63 cases with height ratios less than 2.0, all were negative by PCR-PAGE. Of 52 cases with height ratios of 2.0 or more, 50 were positive by PCR-PAGE. We conclude that PCR-CE is analytically equivalent to DBA and PCR-PAGE for detecting clonal T-cell populations. The PCR-CE method is semiquantitative and, therefore, may be more objective than gel-based methods.

Analysis of T-cell receptor gene rearrangement in lymph nodes of patients with mycosis fungoides. Prognostic implications.

OBJECTIVE: To determine the prognostic value of analyzing lymph node (LN) DNA from patients with mycosis fungoides for the presence of a monoclonal T-cell population. DESIGN: Inception cohort study. SETTING: A tertiary care referral center in Seattle, Wash. PATIENTS: Fifty-five uniformly staged patients with the diagnosis of mycosis fungoides and who had a lymph node biopsy, 21 with clinically abnormal nodes and 34 with normal nodes. MAIN OUTCOME MEASURES: Lymph nodes were evaluated by Southern blot analysis for T-cell receptor beta-chain (TCRB) gene rearrangement and by histopathologic examination for the LN classification using the National Cancer Institute system. Patients were observed clinically for a mean (+/- SD) of 4.7 +/- 3.4 years. RESULTS: Patients with detectable TCRB gene rearrangement in lymph node DNA had an increased likelihood of a poor clinical outcome and a decreased probability of survival (P < .001 for both) compared with patients with the TCRB germline. Although patients with clinically enlarged nodes were more likely to have the TCRB gene rearranged, those with normal nodes and the TCRB gene rearranged also had a poor clinical outcome and a decreased probability of survival. Similar to those with the TCRB gene rearranged, most patients with advanced histopathologic changes (LN3 and LN4) had a poor prognosis. The presence of a rearranged TCRB gene, however, correctly predicted some patients with intermediate LN scores (LN2) who had a poor clinical outcome. CONCLUSIONS: Detection of a monoclonal T-cell population, as demonstrated by a rearranged TCRB gene on Southern blot analysis, in LNs of patients with mycosis fungoides is predictive of a poor clinical outcome and a reduced probability of survival. Lymph node TCRB gene analysis provides additional prognostic information for patients with mycosis fungoides with intermediate LN histopathology.

[Sun protection by optimally designed fabrics].

A rising incidence worldwide of skin cancer has been observed for years. A high cumulative exposure to UV radiation is a major factor in the development of such neoplasms. Suitable protective measures are therefore becoming increasingly important. Textiles provide simple, effective and medically safe protection against UV radiation. At present, however, in Europe--in contrast to Australia--the UV protection factor (UPF) for summer textiles is not stated. It is a largely unknown fact that by far not all textiles offer sufficient protection. Our goal was to study the factors which determine the UV transmission of fabrics and, based on these findings, to design materials which provide enhanced protection. A spectrophotometric method was used to determine the UV transmission by fabrics made of cotton, silk, polyester and viscose. The UV protection factors of the fabrics were computed on the basis of the transmission data. The UV protection factor is dependent on the type of fibre, yarn and surface design, weight per square metre, moisture content, colour, finishing method and degree of wear. To optimize the UV protection via textiles, a viscose yarn with a low UV transmission was used. This yarn makes it possible to design light-weight summer fabrics with optimized UV protection. This development will make it possible to offer clothing with high UV protection on the European marked. This clothing will not be more expensive than normal products, so that effective prevention should be more available.

CD7+ acute non-lymphocytic leukemia: evidence for an early multipotential progenitor.

Nineteen of 71 (26%) cases of acute myelogenous leukemia (AML) were found to express CD7, a cell surface marker found early during T lineage differentiation. These myeloid leukemias often expressed other lymphoid markers and frequently had rearranged T-cell receptor beta and immunoglobulin heavy chain genes. We propose that in CD7+ AML, the malignant transformation occurred in a CD7+ progenitor cell. CD7+ myeloid leukemic precursors may be capable of limited differentiation with loss of CD7 during the initial phase of the disease, but this capacity may diminish during the course of treatment such that CD7 expression persists.

Origin of malignant adenocarcinoma cell line induced by retrovirus-like particles from DMBA rat mammary tumors.

Intraperitoneal inoculation of neonate Sprague Dawley rats with cell-free extracts containing retrovirus-like particles from DMBA-induced rat mammary tumors resulted in a fivefold increase of benign mammary neoplasias in the survivor animals, in comparison to the spontaneous tumor incidence rate. In addition, four animals developed metastasizing abdominal adenocarcinomas. The ascitic cells of one of the abdominal tumors were established as a permanent tissue culture line (HH-1). After subsequent animal passage, cells of the permanent line HH-9 clone 14 showed increased malignancy manifested by the number of takes per animals injected, and by the number of remote metastases observed.

[Unusual displacements of the lower molars].

Two unusual cases of maleruption of mandibular third molars associated with the retention of adjacent teeth and the presence of supernumerary teeth, respectively, are described. Due to the position of the crowns to each other, this phenomenon is termed "kissing teeth" in our clinic.
