[Parathyroid autotransplantation during expanded total thyroidectomy for cancer].

Autotransplantation of the parathyroid gland was performed in 28 out of 95 patients undergoing total thyroidectomy for carcinoma. The autotransplantation was supplementary in 15 patients who kept at least one parathyroid gland in the neck and substitutive in 12 patients who had total cervical parathyroidectomy. After 1 to 30 months' follow-up none of these patients had signs of hypoparathyroidism. The incidence of permanent hypothyroidism after total thyroidectomy was reduced from 7% to 2.1%. However, autotransplantation should be considered as a means of saving any cervical parathyroid that cannot be preserved and has been shown by histopathological examination to have escaped invasion by the carcinoma.

[Quantified EEG and CNV changes during inhalation of nitrous oxide].

The present study constitutes an approach to the action mechanisms of nitrous oxide (N2O) by assessing both the subjective effects induced by the inhalation of this anaesthetic gas, and their electrophysiological correlates, i.e., spontaneous EEG, analyzed by a Fourier algorithm, AEP and CNV. The experiment was conducted single blind in 9 volunteers under 3 recording conditions: inhalation of air, of a mixture of 25% N2O--75% O2, and of a mixture of 50% N2O--50% O2. A latin-square design was used. Polymorphic effects of N2O were evidenced on behaviour and on CNS: some of the behavioural manifestations could evoke a depression of SNC: muscle relaxation with difficulty of motor initiation, psychomotor slowing, slower EEG with diminution of the percent alpha component and increase in the percent theta and delta, loss of amplitude of both exogenous (N1) and endogenous (P300) components of the AEP. Conversely, other manifestations were observed that are compatible with an activation of the CNS: increase of sensory perceptions, appearance of perceptual illusions, maintenance of a high CNV amplitude and lengthening of its duration. This dual effect of N2O suggests a dissociative action at the different levels of the CNS.

[Results of the conservative surgical and irradiation treatment of 132 nonpalpable ductal carcinomas in situ of the breast].

PURPOSE: Retrospective analysis of results of treatment of 132 subclinical ductal carcinomas in situ, non-palpable. MATERIAL AND METHODS: Patients were treated with limited surgery and 70 Gy radiation therapy (70 Gy). RESULTS: With a median follow-up of 7 years, the total recurrence rate was 6%, and the actuarial rate at 5 years 4% and at 10 years 13% at. These have no influence on recurrence on the specific actuarial survival rate which was 100% at 10 years. In spite of five infiltrating recurrences of seven, no metastasis appeared 48 months after the salvage surgery. The global rate of breast preservation was 92% at 7 years. DISCUSSION AND CONCLUSION: Therapeutic indications were developed taking into account the present analysis and a literature review (2,338 in situ ductal carcinomas, palpable or not, treated with conservative surgery, with or without adjuvant radio-therapy).
