The clinical course of 117 patients who were treated for Stages IIB-IIIC sarcoma of soft tissues by the combined approach of radiation and surgery at MGH during the period 1971-1984 have been analyzed for an effect of adjuvant chemotherapy to improve clinical results. Thirty-two patients received adjuvant chemotherapy and 85 were treated by local methods alone. The chemotherapy protocols featured doxorubicin (25 patients) or VAC (seven patients). The no chemotherapy patients were controls matched for tumor grade and size and were treated over approximately the same period. The two groups were not well matched for age: median age of 38.5 and 54 years for the treated and control groups, respectively. There was no significant difference in survival. However, the time to first metastasis was longer in the treated group, P = 0.04. Furthermore, the local failure rate appeared to be lower in the treated group, P = 0.06.
One hundred fifty patients with bone marrow failure were treated in three groups with antithymocyte globulin (ATG; Upjohn, Kalamazoo, MI) in a multicenter trial. Patients were assessed at 3, 6, and 12 months after initiation of treatment by three criteria: transfusion independence, clinical improvement, and blood counts. Group I consisted of 77 patients with acute severe aplastic anemia, randomized to receive either ten or 28 days of ATG. There was no significant difference between the two arms of this protocol: 47% of all patients were clinically improved and 31% were transfusion independent at 3 months. Of the severely affected patients, 27% died before 3 months; most deaths occurred early in treatment. Factors associated with survival in severely affected patients included male sex, age less than 40 years, absolute neutrophil count greater than 200/microL, and idiopathic etiology. Neutrophil counts generally increased by 8 weeks after treatment, but patients continued to show improvement to 1 year posttreatment. In Group II, 44 patients with moderate or chronic severe aplastic anemia were randomized to receive either ten days of ATG or 3 months of high-dose nandrolone decanoate. No patient initially treated with androgens recovered, but 28% of ATG-treated cases achieved transfusion independence at 3 months. Group III consisted of patients with a variety of bone marrow failure syndromes. Patients with pancytopenia and cellular bone marrow showed response rates similar to those of patients with chronic or moderate aplastic anemia.
The colon motor response to a meal consisting of 100 mM of sodium oleate was assessed before and after neodecortication in male Sprague-Dawley rats. Recording probes were anchored surgically in the ascending and descending colon. Pressure changes were recorded on a dynograph using a low-compliance perfusion system. A motility index took into account the amplitude, duration, and frequency of contractions. Neodecortication increased the motility index of the distal colon in the fasting state. However, removal of the cerebral cortex did not affect significantly the colon motor response to a meal. Meal stimulation increased the motility index before and after neodecortication. These findings suggest that resting colonic motor activity is increased after neodecortication, probably through the loss of an inhibitory influence of the central nervous system; and the cerebral cortex is not required for the colon response to a meal in the rat.
In 1978, there were initiated two independent randomized, prospective trials of adjuvant doxorubicin hydrochloride (Adriamycin) following primary therapy for soft-tissue sarcoma. The virtual identity of these two protocols permits their combination for analysis. A total of 75 patients (42 men, 33 women) with soft-tissue sarcoma (stages IIB to IVA) were randomized, after receiving optimal regional therapy, to receive either doxorubicin hydrochloride (450 mg/m2) (37 patients) or observation (38 patients). Follow-up has ranged from 16 to 80 months (median, 49 months). Twenty-five patients (33%) died, and two patients receiving doxorubicin developed cardiotoxicity. No significant differences in local control, metastasis-free survival, disease-free survival, and overall survival were observed for the two treatment arms. Despite temporary prolongation of disease-free survival with doxorubicin in some subgroups, we conclude that there is no advantage to the use of adjuvant doxorubicin in the treatment of soft-tissue sarcoma.
Motor nerve conduction and excitability were measured on the tail nerve of anesthetized rats before and after the nerve was exposed perpendicularly to a static electromagnetic field of various intensities and durations. There was no significant change in either the distal latencies or the amplitudes of the compound muscle action potential (CMAP) measured from stimulating the tail nerve after it was exposed to the electromagnetic field with a density up to 1.2 Tesla (T) for a duration of 60 seconds. However, the nerve excitability expressed as changes of the amplitudes of the submaximally evoked CMAP increased significantly when the tail nerve was exposed to a magnetic field with a density higher than 0.5T for more than 30 seconds. The finding that an electromagnetic field increases motor nerve excitability suggests a possible mechanism of its therapeutic effects.
The influence of evacuating, mixing and returning ruminal contents on microbial populations, volatile fatty acid (VFA) concentrations and liquid flow rate was investigated with four ruminal-cannulated Hereford steers (247 kg avg wt). Ruminoreticular contents were sampled, then completely removed, mixed for 5 min and returned to the rumen. Subsequent samples were taken immediately, 1 h and 4 h later. Non-evacuated steers were sampled at identical time intervals either 1 d before or after evacuation. Averaged over time, there was no significant difference between evacuated and non-evacuated steers in total anaerobic, cellulolytic and facultative bacteria, protozoa, oxidation-reduction potential, VFA concentrations, and liquid flow rates. There were no treatment X time interactions and, except for holotrich protozoa and VFA, no differences from time of sampling. Ruminal evacuation does not appear disruptive to anaerobiosis or detrimental to ruminal microorganisms and digestive processes.
Young rats with lesions to either the globus pallidus, substantia nigra, median raphe, or pontine reticular formation have previously been reported to be deficient in learning a wide variety of laboratory tasks. In the current study, weanling rats subjected to one of these lesions were rested for three weeks, then examined for acquisition and extinction of a water-motivated straight alley task, and finally tested on luminous flux discriminations of increasing difficulty. All brain-damaged groups were slower than the controls in extinguishing the alley task and only the median raphe group failed to show an impairment on the discrimination problems. These results and others suggest that the foregoing lesions produce deficits in inhibitory and attentional processes. The possibility is discussed that young rats bearing these lesions might serve as a model for the investigation of the neurobiological and cognitive disturbances underlying certain classes of mental retardation in children.
1,2-Dimethyl hydrazine (DMH) is absorbed in the colon by passive diffusion in the 0.1-5.0 mM concentration range. Absorption of DMH is enhanced by both conjugated and unconjugated bile acids. The presence of hydroxy-fatty acids in the colon markedly increased DMH absorption while fatty acids of different chain length did not influence absorption.
The resting colonic motor activity before and consecutively after spinal cord transection was recorded in male Sprague-Dawley rats. Recording probes were anchored surgically in the ascending and descending colon. Pressure changes were recorded on a dynograph using a low compliance perfusion system. A motility index took into account the amplitude, duration and frequency of contractions. Following a baseline recording animals were subjected either to spinal cord transection at T4 level or a sham operation. The recording sessions continued regularly on alternate days for the observation period of 3 weeks. Transection of the thoracic spinal cord markedly reduced the motility index of the distal colon on the first postoperative day. However, the motor activity gradually returned to pre-operative values after 7 days. Sham surgery did not influence the motor activity. These findings suggest that colonic motor activity is influenced by spinal shock and probably by different neural mechanisms mediating proximal and distal activities of the colon in rats.
Circulating antibodies against certain nuclear acidic protein antigens have been shown to have diagnostic and prognostic importance in connective tissue disease. We describe a new precipitin system found in the sera of patients with systemic lupus erythematosus. The antigen, called MA, was prepared from calf thymus nuclei, and was shown to be distinct from other nuclear acidic protein antigens by physicochemical and immunologic techniques. MA antibodies were detected in the serum of 12 of 66 lupus patients and in none of 554 sera from normal controls or patients with other rheumatic diseases. Lupus patients having MA antibodies had more severe disease than did lupus patients with Sm or native DNA antibodies, manifested by recalcitrant skin rashes and a significantly greater incidence of hypocomplementemia, serious renal disease, hypertension, hepatosplenomegaly, lymphadenopathy, and neurological disease (P values range from 0.025 to 0.005). The presence of circulating MA antigen was demonstrated in three lupus patients immediately before a flare of nephritis. These data suggest that MA is a nuclear acidic protein antigen that may identify a subset of lupus patients with very severe disease. The presence of the antigen in the circulation before clinical flares suggests a possible biologic role for the MA system in an immune complex nephritis.
