Abdominal wall lifting with subcutaneous wiring: an experience of 50 cases of laparoscopic cholecystectomy without pneumoperitoneum.

Abdominal wall lifting is a method to produce operative space between the anterior abdominal wall and the intra-abdominal organs during laparoscopic surgery. We devised a hanger lifting procedure for the anterior abdominal wall to avoid complications as well as reduce the costs related to the pneumoperitoneum. In our series, we performed 50 cases of laparoscopic cholecystectomy with this abdominal wall-lifting procedure. Though at the beginning we performed laparoscopic cholecystectomy by the pneumoperitoneum, we discarded the insufflator for this operation since starting the new procedure. There was no incidence of conversion to pneumoperitoneum and a fairly good operative view was achieved enabling a smooth laparoscopic cholecystectomy.

Ojigi tying spatula: a newly developed movable spatula for laparoscopic ligation.

We developed a new type of movable tying spatula for ligation of the cystic duct and artery in laparoscopic cholecystectomy. This spatula can be used to ligate any vascular or ductal structure before laparoscopic resection. This instrument should give strong support to surgeons performing laparoscopic procedures and eventually will replace the expensive clips and Endoloops.

Occupational skin disease in newspaper pressroom workers.

Studies of printing industry tradespeople have reported an increased problem of dermatologic abnormalities, including contact dermatitis and dermatitis attributed to solvent exposure. The current cross-sectional health survey of dermatological conditions were conducted in a follow-up of perceived skin abnormalities among newspaper pressroom workers. We surveyed 215 pressroom workers and 34 compositors at a large northeastern US newspaper printing facility. Our findings indicate that printing pressroom workers reported skin condition symptoms at a significantly higher rate than did the compositor referent group. Pressroom workers also were found to be at a significantly elevated risk of developing dermatitis after self-reported exposure to certain commonly used solvents. This emphasizes the need for proper work practices, product substitution where possible, and appropriate protective glove use by newspaper pressroom workers.

Development of a computer-aided surgery system: three-dimensional graphic reconstruction for treatment of liver cancer.

Simulation of the needle puncture and volume estimation for the tumors in the liver were carried out with the three-dimensional image reconstruction system, which consists of a medical image acquisition system, a data processing system, and a graphic display. A set of sliced-image data from a computerized tomography and/or a magnetic resonance imaging was used to reconstruct the liver, the vessels, and the tumors of the patients with liver cancer. A good agreement of anatomic locations of both the intrahepatic vessels and the tumors between the reconstructed liver model and the echography done intraoperatively was observed. Surgical simulations with these graphic models clearly indicated safety areas for needle puncture in the laser coagulation therapy. In addition liver volumes were calculated within 3% of error in comparison to the measured values. These results indicate that the computer-aided surgery system is a highly promising method that avoids cumbersome stereoscopic recognition of the anatomical location of the diseased area and the vessels, before and after surgery.

[Three-dimensional analysis of colonic adenoma and of carcinoma in adenoma with scanning electron microscope and sequential serial sectioning].

Three-dimensional structures were analyzed on 32 human colonic adenomas, 5 hyperplastic polyps, 4 normal colonic mucosa and 5 advanced colonic carcinomas with Scanning Electron Microscope (SEM) and with sequential serial sectioning. After SEM observation, the samples were softened with 2% NaHCO3 and observed with histological serial sectioning. By combining both techniques, that is combined analysis of outer structures for SEM and inner structures for serial sectioning, three dimensional analysis of the colonic adenoma and carcinoma in adenoma were successfully completed. The colonic adenomas were classified into 6 types based on the surface structure observed by SEM as Type I (circular type), Type II (long-elliptical type), Type III (branched type), Type IV (gyral type), Type V (wrinkle type) and Type VI (complicated type). Classified Types I, II, III and IV were observed in tubular adenoma, and grades of atypia seemed to be increased as increasing type numbers. Type V, in tubulo-villous adenoma and villous adenoma, were found out and showed severer atypia than observed in Type IV. Among 32 adenomas, 3 portions of focal cancers were detected, and the surface structures of them had highly complicated patterns as shown with Type VI.

Laparoscopic cholecystectomy: an approach without pneumoperitoneum.

Diagnostic pneumoperitoneum, which has been considered the first step of any laparoscopic procedure, is no longer an absolute necessity. We devised an alternative to pneumoperitoneum or abdominal insufflation by upward and outward traction on the anterior abdominal wall with a "hanger lifting method" using subcutaneous wiring. Fairly good room was produced intraabdominally, which was enough in which to perform the cholecystectomy procedure. We have successfully performed 40 cases of laparoscopic cholecystectomy with this procedure. No complication was experienced with this method and, moreover, excess instrumentation and complications related to pneumoperitoneum were avoided.

Levels of lead in blood and bone of women giving birth in a Boston hospital.

Blood lead levels declined among Americans aged 1-74 y between the years 1976 and 1991 (National Health and Examination Survey [NHANES III]). In 1990, umbilical cord blood lead levels were surveyed among 223 women who gave birth in a Boston hospital obstetrical service. In a survey conducted 10 y earlier, women had a mean umbilical cord blood lead level of 6.56 micrograms/dl (standard deviation = 3.19). In another subgroup of 41 women who were 1-4 wk postpartum, bone lead levels were surveyed with a K-x-ray fluorescence instrument and by analysis of venous blood lead levels. In 1990, umbilical cord blood lead levels were very low (i.e., mean and median of 1.19 [standard deviation = 1.32] and 1.0 micrograms/dl, respectively. In the subgroup of postpartum women, lead levels in cortical bone (i.e., tibia) and trabecular bone (i.e., patella) were also low (i.e., tibia mean and median of 4.5 [standard deviation = 4.0] and 4 micrograms/g bone mineral, respectively); patella mean and median of 5.8 [standard deviation = 4.5] and 5 micrograms/g, respectively). Maternal age was the only factor associated (i.e., borderline [p=.076]) with bone lead. Umbilical cord blood lead levels among women served by this Boston hospital from 1980 to 1990 declined dramatically, thus paralleling the National Health and Examination Survey. Bone lead levels were also low, but lead remains a reproductive hazard to women in select high-risk groups.

Workers' compensation reform: 180 days in Massachusetts.

While not the usual Occupational Medicine review article, this description of the 1991 workers' compensation reforms in Massachusetts reveals some of the difficulties and possibilities inherent in such an undertaking.
