[Technic of ventral spondylodesis of the lower cervical spine].

Anterior fusion of the lower cervical spine is usually performed without difficulty. The surgical technique is simple and normally involves little risk. The indication and technique depend completely on the clinical picture; otherwise, the results are unavoidably poor. A slipped cervical disc is operated on using the Smith-Robinson or Cloward procedure. If extensive decompression with resection of dorsal osteophytes is required, the Cloward technique is preferable due to the superior view of the spinal canal. In dislocations or dislocation fractures of the cervical spine requiring decompression and fusion from the anterior approach, the spine should be stabilized with a bone graft and plate. An anterior approach increases the instability due to damage to the posterior ligaments in flexion injuries. Fusion is usually performed with autologous iliac or fibular grafts, which show good ingrowth but different load-bearing properties. Multisegmental fusions or defects following tumor resections can be bridged with deep-frozen homologous grafts. To achieve immediate stability PMMA implants may be necessary. Implants of bioactive glass ceramics may prove useful in the future.

[Chronic spondylogenic cervical myelopathy. Pathogenesis, prognosis, therapy].

The term "spondylotic cervical myelopathy" is not generally used in the same manner. It is not correct at all to use the expression "spondylotic cervical myelopathy" for an illness inducing damage of the cervical spine. This however happens in literature--especially in neurochirurgical ones very often. It is additionally confusing when the specific course of illness induces pathogenetical statements. "Acute cervical myelopathie" is not identical to a spinal cord compression caused by disc-protrusion or slipped disc, just as "chronic myelopathy" is not identical with spondylogenic cervical cord lesions. "Cervical myelopathy" describes an syndrome, exactly an impairment of cervical spinal cord. This syndrome "cervical myelopathy" can base on many causes. Encephalomyelitis disseminata, a tumor in the region of the cervical spine, a myelopathy induced by radiation and also a so-called "whiplash injury" of the cervical spine should be mentioned for example.

[The Erlangen hip brace, a principle of rehabilitative treatment (author's transl)].

Painful, loosened total hip prostheses and instability following their removal require up to now occasionally support by means of a brace. The paper deals with design, way of action and indication of a new, light-weight, dynamic brace to controle the motion of hip joint. It is evident that this brace can be used not only to manage the loosening of a total hip prosthesis or conditions following the resection of the head of femur, but also in osteoarthrosis of hip joint.
