[Early jaundice after orthotopic liver transplantation. The role of ischaemia (author's transl)].

The purpose of this experimental study was to identify the cause of early cholestatic jaundice occurring after orthotopic liver transplantation. The role of ischaemia was investigated in dogs undergoing auto-transplantation of the liver preserved for two hours. The animals developed early cholestasis with lesions that were markedly different from the graft-rejection lesions observed in a control group, being restricted to the cells and canaliculi of the central lobular area. The results of this work have made it possible to avoid overdosage with immunosuppressive drugs in a patient who had recently undergone orthotopic liver transplantation.

Heterotopic liver transplantation in end-stage HBsAg-positive cirrhosis.

A patient with end-stage cirrhosis complicating HBsAg-positive chronic active hepatitis given a heterotopic liver transplant is alive and well 28 months after transplantation; liver function is normal, and there are no hepatitis-B related lesions on the liver graft despite positivity of HBsAg in the serum. This observation confirms that a heterotopic liver graft can offer long-term survival in man and suggests that this technique may be a more suitable procedure than orthotopic transplantation in end-stage cirrhotics.

Criteria for hepatic transplantation in cirrhosis.

Among the patients considered for hepatic transplantation, cirrhosis should be theoretically the best indication. Results so far are disappointing because the patients who have undergone a transplantation had end stage liver disease. When gastrointestinal bleeding, renal failure, coagulopathy and encephalopathy are used as criteria, one can accurately select patients who will die from cirrhosis within one month. Our results for those who had a liver graft in this group are poor. Criteria were then selected to identify a less dramatically ill group of patients with cirrhosis. These patients have residual poor liver function after surviving acute severe complications and have a spontaneous survival of less than one year. The results of liver transplantation in two patients in this group are encouraging.

[Spontaneous tolerance of liver allograft in the rat (author's transl)].

Several clinical and experimental arguments suggest that the liver has a privileged status regarding allograft rejection. The purpose of our study in inbred rats was to verify this notion in animals with precise histocompatibility relationships. Non auxiliary heterotopic liver transplantation, in which the graft ensures totally the hepatic function of the recipient, is equivalent in this view to orthotopic liver transplantation but has the major advantage to permit more easily the obtention of a low operative mortality. We compared the rejection of heterotopic heart allografts and heterotopic non auxiliary liver allografts in two semi-allogeneic donor-recipient combinations on inbred rats. Our main finding was that, contrasting with the constant acute rejection of heart allografts, about one-third of liver allografts were spontaneously tolerated. Test skin and heart allografts performed in rats with spontaneously tolerated liver allografts suggested that these animals were in a state of donor-specific non responsiveness. Spontaneous tolerance of organ allografts is very uncommon and our observation with liver allografts in inbred rats reinforce the concept that the liver has a privileged status facing rejection.

[The economical use of methods employed in the diagnosis of jaundice: decisional attitude as opposed to diagnostic attitude (author's transl)].

For the diagnosis of jaundice, a comparison is established between a classical diagnostic attitude and a decisional attitude which consists in guiding and restricting the investigations according to the therapeutic possibilities. It appears from the comparative study of the two groups of patients that the decisional attitude is better, resulting in a treatment which occurs earlier, is less expensive and probably of a better quality.
