[Aspects of clinical course and treatment of the gastroduodenal ulcer perforation in combination with hemorrhage].

The results of treatment of 51 patients with gastroduodenal ulcer perforation (GDUP) in combination with hemorrhage were analyzed. Frequency of this complication in the gastroduodenal perforative ulcers structure had constituted 2.5%. The GDUP clinical variants in combination with hemorrhage were delineated depending on the time of the particular complication occurrence, the peculiarities of their clinical course were depicted. Necessity of an early radical operative intervention was substantiated as well as application of active surgical tactics while postoperative hemorrhage occurrence. Postoperative mortality was 29.4%.

[Changes of the aluminum and silicon contents and cholinergic activity of the blood depending on the course of the gastroduodenal ulcer].

In 37 patients with gastroduodenal ulcer disease were revealed the changes of concentration of aluminium and silicon in blood, of acetylcholine in erythrocytes and plasma, the cholinesterase (ACHE) activity depending on the disease clinical course, the ulcer size; the interconnection between the blood cholinergic activity and the contents of aluminium and silicon; the influence of aluminium and silicon ions on the ACHE activity of plasma and erythrocytes, what is confirmed by the correlational analysis data and gives and arguments for application of chemical compounds with aluminium and silicon in treatment and prophylaxis of an ulcer disease recurrence.

[Relaparotomy in patients with perforated gastric and duodenal ulcers].

The article discusses the results of analysis of the causes of relaparotomy in 54 (3.1%) patients with perforating gastroduodenal ulcer in the period between 1966 and 1985, as well as the peculiarities of the course of complications, establishment of the diagnosis, and the methods and outcomes of surgical treatment. Postoperative mortality was 24.07%. The outcomes of relaparotomy were determined by early diagnosis of the complication, the time of the repeated operation, and the radical character of the first operative intervention. Mortality was less in relaparotomy undertaken in time (10.0%) than in late operations (41.7%). Mortality was highest among patients with postoperative peritonitis (46.7%). Postoperative peritonitis and intestinal obstruction were the most frequent conditions for which relaparotomy was carried out. Such factors as late hospitalization of the patients and generalization of peritonitis, the patients' age, doctors' errors, lateness and type of the first operative intervention played the principal role in the development of complication.

[Latent perforated gastroduodenal ulcers].

It was established from study of the case records of 98 patients that concealed perforating gastroduodenal ulcers take a phasic clinic 1 course. Each phase has a characteristic symptomatology. Special methods of examination increase the possibility of establishing the diagnosis. Concealment of the ulcers is caused predominantly by two main conditions: a small size of the perforation and localization of the ulcerous process on the anterior wall of the pyloroduodenal zone. The larger the perforation, the more probable is the presence of pneumoperitoneum. All patients with concealed perforating gastric and duodenal ulcers must be subjected to emergency operative treatment. The average duration of in-patient treatment depends on the time of hospitalization, timely diagnosis and operative intervention, and the presence of complications. Their prevention is therefore one of the most important problems.
